[Operative procedure for simultaneous spontaneous pneumothorax].

We have treated 542 cases of spontaneous pneumothorax during the past 20 years, among them 40 cases of bilateral simultaneous spontaneous pneumothorax and 55 cases of non-simultaneous bilateral pneumothorax were included. This paper discusses when the bilateral operation for bilateral simultaneous spontaneous pneumothorax can be carried out and operative approach to bilateral simultaneous thoracotomy. The following results were obtained: Contralateral pneumothorax after an operation for unilateral spontaneous pneumothorax occurred in 25% of teenagers. In cases of spontaneous pneumothorax with …

Intercostal Nerve Cryoablation is Associated with Reduced Opioid Use in Pediatric Oncology Patients.

INTRODUCTION: Intercostal nerve cryoablation reduces postoperative pain in adults undergoing thoracotomy and children undergoing pectus excavatum repair. We hypothesize that cryoablation is associated with decreased post-thoracotomy pain and opioid use in pediatric oncology patients. METHODS: A single-center retrospective cohort study was performed for oncology patients who underwent thoracotomy from January 1, 2017 to May 31, 2021. Outcomes included postoperative opioid use measured in morphine milligram equivalents per kilogram (MME/kg), pain scores (scale 0-10), and opioid prescription at …

The efficacy of thoracoscopic surgery for descending necrotizing mediastinitis.

OBJECTIVES: Thoracotomy is a reliable approach for descending necrotizing mediastinitis (DNM), and the use of video-assisted thoracic surgery (VATS), a minimally invasive procedure, has been increasing. However, which approach is more effective for DNM treatment is controversial. METHODS: We analysed patients who underwent mediastinal drainage via VATS or thoracotomy, using a database with DNM from 2012 to 2016 in Japan, which was constructed by the Japanese Association for Chest Surgery and the Japan Broncho-esophagological Society. The primary outcome was 90-day mortality, and the adjusted …

[The usefulness of mediastinoscopy for primary lung cancer].

The usefulness of mediastinoscopy for lung cancer was retrospectively evaluated for the aspect of prognosis. The subjects were 421 patients with primary lung cancer except exploratory thoracotomy. In the MD(+) group, there was no difference in prognosis between right and left sided thoracotomies, but in the MD(-) group there was a tendency for poorer prognosis in the left-sided thoracotomy (p < 0.1). In the right-sided thoracotomy, there was no difference of prognosis between the MD(+) group and MD(-) group. On the contrary, in the left-sided thoracotomy there was a statistically better progno…

The value of selective mediastinoscopy in predicting resectability of patients with bronchogenic carcinoma.

From 1980 to 1990, 1505 patients underwent thoracotomy as definitive treatment for non small cell lung cancer. Computed tomography (CT) of the chest has been used routinely since 1984 for assessment of mediastinal lymph node involvement. A total of 235 cervical mediastinoscopies and 71 anterior parasternal mediastinotomies were performed on the same patient population as preoperative staging when CT scan demonstrated mediastinal lymph nodes larger than 1 cm. Radical resections showed a constant increase in number from 70.1% in the period 1980-84 to 82.7% in the period 1985-90. Exploratory thor…

The frequency, significance, and management of a right aortic arch in association with esophageal atresia.

An unrecognised right aortic arch (RAA) found at thoracotomy may complicate the repair of oesophageal atresia (OA) and tracheo-oesophageal fistula (TOF). This paper analyses the patient characteristics, peri-operative management, and outcome of 16 infants with a RAA, and proposes management guidelines. Between 1948 and 1996, 709 patients with OA/TOF were admitted to the Royal Children's Hospital, of whom 13 had a RAA. Three additional cases from two other paediatric surgical units were included. All 16 case records were reviewed retrospectively. The overall incidence of RAA in OA was 1.8%. Nei…

Mediastinoscopy as a predictor of resectability in patients with bronchogenic carcinoma.

Eighty consecutive mediastinoscopies, performed for assessment of patients with bronchogenic carcinoma, were reviewed with regard to accuracy and complications. Thirty patients had mediastinal lymph node metastases: 26 were considered inoperable and thus saved non-therapeutic thoracotomy, 4 were considered operable of whom 3 had a curative resection, and one was inoperable at thoracotomy. Of 50 patients with negative mediastinal nodes 43 had thoracotomy; 42 were resectable and one was unresectable. Seven did not have thoracotomy because of other contraindications. In total of 47 patients under…

Access to the thorax by incision.

BACKGROUND: For years the standard access to the thorax has been by posterolateral or other muscle cutting thoracotomy incisions. These are accompanied by significant discomfort and often limitation of shoulder girdle movement. To obviate these distressing features, the less traumatic median sternotomy was recommended. In the last ten years, several types of mini or axillary thoracotomies have been described. We believe that the vertical axillary thoracotomy is the best of these incisions as no major muscles are divided, it can be created rapidly, and exposure is excellent. STUDY DESIGN: We c…

Completely thoracoscopic lobectomy for the surgical management of bronchiectasis.

BACKGROUND: The feasibility of completing a lobectomy by completely video-assisted thoracoscopic surgery (cVATS) in the management of bronchiectasis is unclear. By retrospectively comparing the outcomes from the lobectomies that used thoracotomy vs. cVATS, we determined the appropriateness of the minimally invasive cVATS approach in the management of bronchiectasis. METHODS: Between June 2001 and October 2010, 60 patients with bronchiectasis underwent surgery, of which 56 lobectomies were performed. All lobectomies were carried out by either thoracotomy or cVATS approach. Pulmonary vessels an…

A right-sided aortic arch with Kommerell's diverticulum of the aberrant left subclavian artery presenting with syncope.

A right-sided aortic arch with an aneurysm of the aberrant subclavian artery is a rare disease. We report a case of Kommerell's diverticulum of an aberrant left subclavian artery in a patient with a right-sided aortic arch. Fewer than 50 cases have been reported in the literature. A number of operative strategies are described. Right thoracotomy provides good exposure and avoids the morbidity associated with bilateral thoracotomy or sternotomy and thoracotomy. In our patient with symptoms of dysphagia, syncope, and left subclavian steal syndrome, a left thoracotomy was used. The repair was acc…

The need for thoracic surgery in adult patients receiving extracorporeal membrane oxygenation: a 16-year experience.

OBJECTIVES: Patients on extracorporeal membrane oxygenation (ECMO) are at risk from thoracic complications such as bleeding or pneumothorax, which may subsequently necessitate thoracic surgical intervention. We aimed to: 1) analyse the indication and nature of thoracic surgical intervention in these patients and 2) analyse the effect of a change in the ECMO circuit from roller pump to centrifugal pump on transfusion requirements pre and post thoracotomy. METHODS: We retrospectively reviewed a prospectively collected database of 569 adults put on ECMO between 1995 and 2011. Patients undergoing…

Survival outcomes and surgical morbidity based on surgical approach to pulmonary metastasectomy in pediatric, adolescent and young adult patients with osteosarcoma.

BACKGROUND: Thoracotomy is considered the standard surgical approach for the management of pulmonary metastases in osteosarcoma (OST). Several studies have identified the advantages of a thoracoscopic approach, however, the clinical significance of thoracotomy compared to thoracoscopy is yet to be evaluated in a randomized trial. AIMS: The primary aim was to determine the survival outcomes in OST patients based on surgical approach for pulmonary metastasectomy (PM) and secondary aim was to assess the post-operative morbidities of OST PM through various surgical approaches. Video-assisted thoracic surgery for bronchiectasis.

BACKGROUND: Bronchiectasis is one of the common diseases diagnosed in the world. No major improvement for the treatment approaches and limited efficacy promote a big challenge for management of this disease. Video-assisted thoracoscopic surgery (VATS) offers a new choice for the treatment of bronchiectasis. The purpose of this study was to present our experience of VATS for bronchiectasis and to compare this with thoracotomy in our institution. METHODS: We reviewed the medical records of patients who underwent VATS lobectomy and general lobectomy for bronchiectasis between January 2005 and De…
