Near-complete obstruction of her supraglottis from a lymphangioma necessitated tracheotomy to secure the airway. The diagnostic work-up and management of this patient and her post-operative course are discussed. Beckwith-Wiedemann syndrome is a congenital disorder manifested by organomegaly, omphalocele, hypoglycemia, and macroglossia. We have found a significant number of these children to be at risk for upper airway obstruction during infancy or childhood. In this review of 13 children, 2 required tracheotomy during infancy for cor pulmonale caused by macroglossia. Seven of nine children older than 1 year required tonsillectomy and adenoidectomy to relieve upper airway obstruction. Although macroglossia can be a cause of airway obstruction in infants with Beckwith-Wiedemann syndrome, we have found t…

Laryngeal cysts, particularly epiglottic cysts, are generally benign lesions which cause mild dysphagia or hoarseness. We report a case of an epiglottic cyst that caused almost complete airway obstruction. A 43-year-old man presented with progressive dysphagia, hoarseness, and airway obstruction secondary to a large cystic mass involving the entire epiglottis and filling the hypopharynx. He required emergency tracheotomy to secure the airway, and the cyst was incised and drained. Following a subsequent recurrence, laser excision of the cyst was performed and the problem resolved. The cystic ma…

We report on a 74-year-old man who presented with recurring episodes of asthma. He was admitted in a severe asthmatic state which could not be resolved with the usual bronchodilatatory therapy. An attempt at oro-tracheal intubation failed due to an obstruction in the larynx and the patient died before an emergency tracheotomy could be done. At autopsy a chondroma of the larynx measuring 2.5 x 2.5 x 3 cm was found. It originated from the posterior cricoid cartilage and virtually filled the whole infraglottic space. Chondromas of the larynx are very rare. Dyspnea and hoarseness are the most freq…

A case of severe dyspnoea is reported which occurred in a 21-years-old woman with Willebrand-Jürgens disease four days after intubation during a dental operation. A 34 Charrière catheter was introduced into the glottis after a 22 Charrière catheter had failed to pass the obstruction. The patient was then artificially ventilated by means of a Spiromat and was subsequently tracheotomized after her cardio-pulmonary condition had become stabilized. The cause of the dyspnoea was a fibrin clot of 5-6 cm length. Three weeks after the tracheotomy and one week after extubation the patient was discharge…

INTRODUCTION: Atypical pleomorphic adenoma (PA) is an uncommon tumor, more frequent in submandibular and parotid glands. PA is classified as atypical when it presents hypercellularity, necrosis or hyalinization, dysplasia, capsular violation or distant metastases.
CASE REPORT: We described a case of a 39-year-old female presented with a slowly growing mass involving the soft palate. A life-threatening bleeding from PA with hemorrhagic shock occurred and required ligation of the external carotid artery with tracheotomy. A transoral en-bloc excision of the mass (70 x 50 x 40 mm) was performed. …

A 75-year-old woman suffering from respiratory difficulty was diagnosed with severe tracheal stenosis due o malignant thyroid cancer. She was scheduled for an mergent tracheotomy, but preoperative computed omography revealed severe tracheal stenosis below he glottis due to thyroid cancer invasion. Anticipating lifficult tracheal intubation and risk of obstruction from hemorrhage, intubation preserving spontaneous ventiation was performed to avoid a 'can't ventilate, can't intubate' situation. A size 3.5 air-Q intubating laryngeal airway was inserted using a bronchofiberscope to perform trachea…

BACKGROUND: Neurolymphomatosis (NL) is an extremely rare disease and tracheal asphyxia due to NL has not been previously reported.
CASE PRESENTATION: A 54-year-old Chinese woman with a history of diffuse large B-cell lymphoma in her first complete remission developed peripheral neuropathy and tracheal asphyxia. Neurolymphomatosis involving the right brachial plexus and the right vagus nerve was demonstrated by PET/CT, but not by MRI. She underwent urgent tracheotomy and impact chemotherapy using rituximab combined with high dose methotrexate and involved field radiotherapy. She achieved a sec…

A group of 253 patients with laryngeal carcinoma was treated by endoscopic laser-CO2 surgery, 50 of them presented a supraglottic localizati … a, less complications, shorter stay in hospital and, consequently, lower health costs. In addition, tracheotomy was almost never necessary. Recurrencies can be also treated endoscopically, but, when total laryngectomy is needed following recurrency, this is considered to be a bad prognostic factor.

Sarcoidosis is a chronic granulomatous disease that tends to involve the lungs, hilar and mediastinal lymph nodes, liver, eyes, skin, bones, and nervous system. Sarcoidosis involves the larynx less commonly than it afflicts these other sites. Laryngeal sarcoidosis may occur in isolation or as a component of systemic sarcoidosis and it may produce hoarseness, dysphagia, and dyspnea. Life-threatening airway obstruction can occur. Treatments used for laryngeal sarcoidosis have included tracheotomy, low-dose radiation, surgical excision, systemic steroids, and intralesional steroids. An autoimmune…

One of the most common complications of surgery of the thyroid gland is vocal folds immobility. New advances in its management have been ach … routinely used to obtain vocal rehabilitation. In case of bilateral vocal fold paralysis, to avoid tracheotomy, partial posterior cordectomy using laser surgery restores sufficient laryngeal airflow, with minimal vocal sequelae. Early management of these complications by teams of specialists should allow appropriate and less invasive surgery.

Posterior glottic stenosis in children is not uncommon and must be differentiated from vocal cord paralysis when there is posterior glottic fixation. Procedures aimed at increasing the airway lumen by tissue excision have not been uniformly successful. Chronic aspiration and poor voice results have been reported. Expansion of the posterior glottis yields excellent results. Tracheotomy decannulation without aspiration and return of vocal cord mobility in children who have vocal cord fixation with achievement of a functional voice can be expected from widening the laryngeal framework. Scar incis…

IMPORTANCE: There is controversy over whether a critical period in the development of olfaction exists, as there is in hearing and vision, whereby early stimulation of the olfactory nerve is necessary for normal olfactory performance later in life. Children who undergo tracheotomy early in life are deprived of airflow through the nasal cavity during a critical period of development. Persistent olfactory dysfunction in this patient group after decannulation would provide evidence that postnatal stimulation of the olfactory nerve is critical to normal development.
OBJECTIVE: To determine whethe…

The care and treatment of children and adolescents requiring long-term ventilation pursues several overarching goals and is ideally carried … earance or severe dysphagia with aspiration in spontaneously breathing patients are indications for tracheotomy. For many of these congenital diseases, the disease course and prognosis do not inherently suggest any potential for weaning or decannulation.Consequently, some of the measures and examinations required by the German Federal Joint Committee's (GBA) …

Granular cell tumors (GCTs) are uncommon neoplasms of unknown origin that can manifest in multiple locations throughout the body. Physicians should be aware of this type of tumor presenting in unusual locations such as the larynx, particularly in pediatric patients with stridor and dysphonia. We describe an 11-year-old female with a large laryngeal mass that obstructed the majority of the laryngeal lumen. A tracheotomy was performed to secure the patient's airway, followed by a direct suspension laryngoscopy, during which the mass was excised in its entirety. The biopsied mass was histopatholo…

A 32-year-old man with Down's syndrome was referred to the ear, nose and throat (ENT) department in view of failed attempts at extubation, and subsequently, at decannulation of tracheotomy tube. He had previously required ventilatory support and had history of intubation for 1 week. A flexible fibre-optic laryngoscopy showed a smooth mass covering the laryngeal inlet which moved with respiration. Direct laryngoscopy under general anaesthesia revealed a smooth mucosa covered fleshy mass arising from the left aryepiglottic fold and arytenoid, obstructing the laryngeal inlet. The mass was removed…
