From childhood to adulthood: health care use in individuals with cerebral palsy.

AIM: To analyse the health care usage of individuals with cerebral palsy (CP) as a function of age and ambulatory status. METHOD: In total, 970 self-administered questionnaires relating to health care usage were sent, via a clinical network of professionals and institutions, to children and adults with CP in Brittany, France. Frequency of use of different aspects of health care were analysed as a function of age and ambulatory status. Multivariate logistic regression evaluated differences in the frequency of each health care type with age; the transition from childhood to adulthood was specif

Why do infection preventionists leave a job? A qualitative evaluation of infection preventionist attrition in health care.

BACKGROUND: Infection preventionists' (IPs) importance has increased over the years for various reasons. IP attrition results in risks to both patients and staff. The goal of this project was to determine the drivers of IP attrition and retention through focus groups. METHODS: Focus groups constituted IPs who have been in the same role for > 5years (Tenured IPs), IPs who have left one IP role for another IP role in the last 3 years (Transitional IPs), IPs who have left the field of infection prevention (Former IPs), and IP leaders discussed drivers of attrition and retention. A thematic analy

Transition from pediatric and adolescent gynecology to complex benign gynecology - who, what, and when?

PURPOSE OF REVIEW: The purpose of this article is to provide a comprehensive review of recent literature addressing diagnoses and conditions that intersect pediatric and adolescent gynecology (PAG) and complex benign gynecology (CBG) to inform which patients should make the transition from PAG to CBG care, as well as when and how that transition should occur. RECENT FINDINGS: Recent literature lacks data on formalized transition processes for pediatric gynecology patients who require ongoing management of benign conditions, including, but not limited to, disorders of sexual development, endom

Enhancing care coordination for neurofibromatosis type 1 in primary care: insights and applications for rare diseases.

Patients with rare diseases often encounter significant challenges, including poor coordination of healthcare services. The UK Rare Disease Framework emphasizes key priorities such as faster diagnoses, greater awareness among healthcare professionals, improved care coordination, and better access to specialist care. This National Health Service (NHS) project, based in the North East and Yorkshire Genomic Medicine Service (GMS), aimed to improve care coordination for patients with rare genetic diseases in primary care. The project focused on developing a generic clinical pathway to improve care

Challenges in transitioning from adolescent to Adult Mental Health Services for young adults with ADHD in Italy: an observational study.

AIMS: Ensuring a successful transition to Adult Mental Health Services (AMHS) is fundamental for attention deficit/hyperactivity disorder (ADHD) patients to prevent adverse scenarios in adults (e.g., psychiatric disorders, substance or alcohol abuse). Yet, most European nations do not have appropriate transition guidelines and still fail to adequately support transition processes. This study aims to enquire about the current transition paths in Italy and the perceived experiences of the patients and their clinicians. METHODS: The present observational study collected 36 interviews with young

Moving on Up: Shifting Sickle Cell Disease Management During Adolescence in Preparation for Adulthood.

INTRODUCTION: Sickle cell disease (SCD) is a progressive, life-limiting disease that begins in childhood. Effective SCD management is critical during the transition from pediatric to adult care when morbidity and mortality risks increase. Yet, this transition often coincides with disease-management responsibilities shifting from parents to adolescents, raising questions about adolescents' preparedness. The current qualitative study examines adolescents' (ages 13-17) and their parents' experiences with shifting disease-management responsibilities in preparation for transitioning to adult care.

Healthcare transition in pediatric neurosurgery: lessons learned from a pilot program for patients with hydrocephalus and spina bifida.

OBJECTIVE: The pediatric neurosurgical community has increasingly recognized the importance of healthcare transition, the process of moving a patient from a pediatric to an adult model of care. However, surveys of pediatric neurosurgeons have revealed that few institutions have formal transition programs. Here, the authors share their preliminary experience with the development of a formal transition pilot program for patients with spina bifida and/or hydrocephalus. METHODS: Patients 18 years of age or older with a diagnosis of spina bifida and/or hydrocephalus who were followed by a pediatri

A transitional opioid program to engage hospitalized drug users.

BACKGROUND: Many opioid-dependent patients do not receive care for addiction issues when hospitalized for other medical problems. Based on 3 years of clinical practice, we report the Transitional Opioid Program (TOP) experience using hospitalization as a "reachable moment" to identify and link opioid-dependent persons to addiction treatment from medical care. METHODS: A program nurse identified, assessed, and enrolled hospitalized, out-of-treatment opioid-dependent drug users based on their receipt of methadone during hospitalization. At discharge, patients transitioned to an outpatient inter

Improving Electronic Health Record Access for People Incarcerated in the United States.

Around the United States, carceral medical records are often sequestered from incarcerated patients and their external physicians, hampering the provision of both acute and transitional care. Despite laws meant to ensure better ownership and portability of electronic health records (EHRs), many carceral institutions have not implemented interoperable systems, nor do incarcerated patients have easy access to their medical information. These issues can compromise care for a patient population that experiences higher rates of many diseases, hospitalization, and death compared with the general pub…

Current management of transition of young people affected by rare renal conditions in the ERKNet.

Transition in medical care is a high-risk period in adolescence and young adulthood. To date, data on transition policy, its application in practice, and transition procedures in patients with rare, hereditary kidney diseases in Europe is scarce. An online survey was developed and was distributed within the paediatric centres of the European Reference Network for Rare Kidney Diseases (ERKNet) aiming to assess the transition-relevant structures from the providers' perspectives. Its items were based on the consensus statement on transition published by the International Society of Nephrology (IS

"I think I could have coped if I was sleeping better": Sleep across the trajectory of caring for a family member with dementia.

Dementia-related sleep changes can lead to disruptions among families living with dementia which can jeopardise carers' wellbeing and ability to provide support. This research explores and represents the sleep of family caregivers across the trajectory of caring, before, during, and after the key period of their care recipient moving into residential care. The focus of this paper is viewing dementia caregiving as a trajectory, characterised by care needs which change over time. Semi-structured interviews were conducted with 20 carers whose family member with dementia had transitioned into resi
