Partnering with High-Risk Communities to Successfully Reduce Pediatric Injury Over Time.

PURPOSE: Injury is the leading cause of childhood morbidity and mortality. Injury prevention (IP) initiatives are often created in isolation from the communities most affected. We hypothesized that the use of a comprehensive approach to injury prevention through community partnerships will result in a measurable reduction in pediatric injuries. METHODS: The IP program at our free-standing level 1 pediatric trauma center developed partnerships within eight targeted high-risk communities. IP coordinators and community partners implemented programs driven by community-specific injury data and co

Treatment of penetrating cardiac wounds for the general surgeon on call.

"Scoop and run" approaches for severely injured patients have been adopted by emergency medical services over the past 40 years. This has resulted in more patients with severe injuries including penetrating cardiac wounds arriving at trauma centers and other acute care hospitals. General surgery trauma teams and general surgeons taking trauma call are the first responders for diagnosis, resuscitation, and operative management of injured patients. By natural selection, 96% to 98% of patients with signs of life on arrival to the trauma center after sustaining a penetrating cardiac wound have inj…

Injury patterns, management and outcomes of retroperitoneal haemorrhage caused by lower intercostal arterial bleeding at a level-1 trauma centre: A 10-year retrospective review.

OBJECTIVE: Haemorrhagic shock is a life-threatening complication of trauma, but remains a preventable cause of death. Early recognition of retroperitoneal haemorrhage (RPH) is crucial in preventing deleterious outcomes including mortality. Injury to the 9-11th intercostal arteries (i.e. arteries of the lower thoracic region) are complicit in RPH. However, the associated injuries, implications and management of such bleeds remain poorly characterised. METHODS: We performed a retrospective review of the medical records of patients diagnosed with RPH who presented to our level-1 trauma centre (2

Medial tibial plateau fractures: a new classification system.

BACKGROUND: : Fractures of the medial plateau are associated with significant soft tissue injuries. A predictive measure of which medial plateau fractures are at higher risk of associated injuries has not been described. The authors propose a simple classification that is both easy to remember, recognize, and predictive of associated injuries. A type A fracture is where the fracture line is medial to the intercondylar spines, a type B is where the fracture line is within the intercondylar spines, and a type C fracture is where the fracture line is lateral to the intercondylar spines. METHODS:

Unwarranted national variation in the use of prophylactic inferior vena cava filters after trauma: an analysis of the National Trauma Databank.

BACKGROUND: The proportion of patients undergoing placement of prophylactic inferior vena cava (IVC) filters after trauma has increased dramatically in the past decade. Guidelines for prophylactic IVC filters vary from never recommended (American College of Chest Physicians/CHEST) to the more liberal Eastern Association for the Surgery of Trauma guidelines that recommend prophylactic IVC filters for patients who have both contraindications to anticoagulation and high-risk injury patterns. We hypothesized that U.S. trauma centers would practice according to Eastern Association for the Surgery o…

Trauma room requirements.

Effective shock (or trauma) room management requires thorough preparation, staff competence, dedicated infrastructure and organised protocols. Shock rooms must be sufficiently equipped according to the hospital's designated level of care. This chapter outlines essential aspects, including equipment and facilities, staff qualifications and composition, and communication practices. Key areas including initial assessment, diagnostic procedures, emergency interventions, and future aspects in the care of severely injured patients are addressed, along with emerging innovations in trauma care.

Trauma recidivism is reduced with engagement in psychosocial programming following orthopaedic trauma.

INTRODUCTION: Recidivism is common following injury. Interventions to enhance patient engagement may reduce trauma recidivism. Education, counseling, peer mentorship, and other resources are known as Trauma Recovery Services (TRS). The authors hypothesized that TRS use would reduce trauma recidivism. METHODS: Over five years at a level 1 trauma center, 954 adults treated operatively for pelvic, spine, and femoral fractures were reviewed. Recidivism was defined as return to trauma center for new injury within 30-months. All patients were offered TRS. Multivariate logistic regression statistica

The use of routine laboratory testing in acute trauma care: A retrospective analysis.

BACKGROUND: In more than 60 countries worldwide, laboratory testing plays a challenging and expensive role in trauma resus-citation. In 1995, the literature already suggested that routine laboratory testing may not be useful for most trauma patients. Our study hypothesized that still the need for some laboratory tests perhaps should be reconsidered. Therefore, the aim of this study was to create more insight in the distribution between normal and abnormal parameters for routine laboratory testing in trauma patient management. METHODS: This retrospective analysis was performed at Amsterdam UMC

Optimizing aeromedical evacuation in combat: Balancing trauma system efficiency and patient outcomes.

BACKGROUND: The Israel Defense Forces Medical Corps (IDF-MC) operates an echelon-based system, evacuating combat casualties to civilian trauma centers. During multiarena conflicts, aeromedical evacuation of urgent casualties is directed to frontline-adjacent medical centers (FLAMCs) or to default medical centers (DMCs; Level I trauma centers) to prevent FLAMCs overwhelming. This study aims to assess the impact of casualty distribution on mortality outcomes. METHODS: This is a retrospective cohort study of IDF combat casualties evacuated by helicopter during the "Swords of Iron" war, between O…

Predictors of initial management failure in traumatic hemothorax: A prospective multicenter cohort analysis.

BACKGROUND: Traumatic hemothorax is common, and management failure leads to worse outcomes. We sought to determine predictive factors and understand the role of trauma center performance in hemothorax management failure. METHODS: We prospectively examined initial hemothorax management (observation, pleural drainage, surgery) and failure requiring secondary intervention in 17 trauma centers. We defined hemothorax management failure requiring secondary intervention as thrombolytic administration, tube thoracostomy, image-guided drainage, or surgery after failure of the initial management strate…

Transection of the thoracic aorta: assessment of a trauma system.

A large, heavily populated area regionalized the care of critical trauma in 1980. To evaluate the system, we reviewed patient outcome for thoracic aortic transection due to blunt injury for the first 18 months of trauma system operation. Of the total of 86 patients, 43 were transferred to trauma centers, 8 to nontrauma centers, and 35 were either directly transported to the coroner or dead on arrival at the hospital. Of the eight patients transported to non-trauma centers, seven were in cardiopulmonary arrest during transport and the eighth was pronounced dead shortly after admission to the em…

Missed injuries in trauma care: An analysis of mechanisms and prevention of one of the surgeon's worst nightmares.

BACKGROUND: Missed injuries (MIs) remain a significant and potentially preventable complication in trauma care, often associated with increased morbidity, mortality, prolonged hospitalization, and legal consequences. Despite decades of recognition, MIs continue to challenge trauma teams, particularly in complex, multi-injury scenarios. OBJECTIVE: This study aims to review the literature and identify the most relevant factors contributing to missed injuries in trauma patients, highlighting opportunities for prevention and clinical improvement. METHODS: A systematic review was conducted accord

Operative Management Considerations in a Patient With Multiple Occult Penetrating Cardiac Injuries.

Penetrating cardiac injury remains one of the deadliest traumatic injuries. Early identification and definitive operative management are critical tenets for patient survival; however, variable clinical presentations can obscure the diagnosis. Here, we present the case of a 58-year-old obese man who presented to an urban level 1 trauma center with multiple stab wounds to the epigastrium and lateral left chest in the axillary line with an unknown weapon. The patient was taken emergently to the operating room for exploratory laparotomy, median sternotomy, and attempted repair of multiple full-thi

The effect of circle of willis anatomy and scanning practices on outcomes for blunt cerebrovascular injuries.

BACKGROUND: Limited research has explored the effect of Circle of Willis (CoW) anatomy among blunt cerebrovascular injuries (BCVI) on outcomes. It remains unclear if current BCVI screening and scanning practices are sufficient in identification of concomitant COW anomalies and how they affect outcomes. METHODS: This retrospective cohort study included adult traumatic BCVIs at 17 level I-IV trauma centers (08/01/2017-07/31/2021). The objectives were to compare screening criteria, scanning practices, and outcomes among those with and without COW anomalies. RESULTS: Of 561 BCVIs, 65% were male …

Quantifying the burden of trauma imaging on the CT scan service at a major trauma centre in South Africa.

BACKGROUND: Imaging is an integral part of trauma management and the huge burden of trauma in South Africa places substantial pressures on radiology resources. This study aims to provide a holistic overview of the burden of trauma imaging and the cost of trauma to a busy CT scanning facility at a tertiary hospital in South Africa. METHOD: We set out to describe and quantify the impact of blunt poly-trauma on CT scanning services at Grey's Hospital in Pietermaritzburg. We aimed to provide a holistic assessment in terms of use of equipment and staff, cost to the hospital and overall usage of CT

The Short and the Long of it: Timing of Mortality for Older Adults in a State Trauma System.

BACKGROUND: The impact of injury extends beyond the hospital stay, but trauma center performance metrics typically focus on in-hospital mortality. We compared risk adjusted rates of in-hospital and long-term mortality among Pennsylvania trauma centers. We hypothesized that centers with low rates of in-hospital mortality would also have low rates of long-term mortality. METHODS: We identified injured patients (age ≥ 65) admitted to Pennsylvania trauma centers in 2013 and 2014 using the Pennsylvania Trauma Outcomes Study, a robust, state-wide trauma registry. We matched trauma registry records …

Should Rib Fracture Patients be Treated at High Acuity Trauma Hospitals?

BACKGROUND: It is well known that severely injured trauma patients have better outcomes when treated at centers that routinely treat high acuity trauma. The benefits of specialty treatment for chest trauma have not been shown. We hypothesized that patients with high risk rib fractures treated in centers that care for high acuity trauma would have better outcomes than patients treated in other centers. METHODS: All rib fracture patients were identified via the 2016 National Inpatient Sample using ICD-10 codes; Abbreviated Injury Scales (AIS) and Elixhauser comorbidity scores were also extracte
