Management of chronic pain associated with temporomandibular disorders: a clinical practice guideline.

CLINICAL QUESTION: What is the comparative effectiveness of available therapies for chronic pain associated with temporomandibular disorders (TMD)? CURRENT PRACTICE: TMD are the second most common musculoskeletal chronic pain disorder after low back pain, affecting 6-9% of adults globally. TMD are associated with pain affecting the jaw and associated structures and may present with headaches, earache, clicking, popping, or crackling sounds in the temporomandibular joint, and impaired mandibular function. Current clinical practice guidelines are largely consensus-based and provide inconsistent

Community engagement in outbreak response: lessons from the 2014-2016 Ebola outbreak in Sierra Leone.

Documentation of structured community engagement initiatives and real-time monitoring of community engagement activities during large-scale epidemics is limited. To inform such initiatives, this paper analyses the Community Led Ebola Action (CLEA) approach implemented through the Social Mobilization Action Consortium (SMAC) during the 2014-2016 Ebola epidemic in Sierra Leone. The SMAC initiative consisted of a network of 2466 community mobilisers, >6000 religious leaders and 42 local radio stations across all 14 districts of Sierra Leone. Community mobilisers were active in nearly 70% of all c

Correlation of magnesium deficiency with C-reactive protein in elective cardiac surgery with cardiopulmonary bypass for ischemic heart disease.

BACKGROUND AND OBJECTIVE: Cardiac surgery is associated with systemic inflammatory response, which is triggered by cardiopulmonary bypass (CPB) and possibly with underlying magnesium deficiency. Animal studies have shown that magnesium deficiency intensifies oxidative stress and inflammatory processes. We aimed to find a link between serum, erythrocyte, cardiac tissue magnesium concentration and C-reactive protein (CRP) as an inflammatory marker in patients undergoing elective cardiac surgery with CPB. MATERIALS AND METHODS: The data of 27 patients undergoing elective cardiac surgery with CPB

A Case of Intercurrent Dengue and Probable Relapsing Plasmodium vivax Malaria in a Returned Traveler to India: Case Report and Literature Review.

Dengue and malaria are common vector-borne tropical diseases and are associated with high morbidity and mortality. Co-infection of dengue and malaria is underestimated due to parsimonious diagnostic approaches once the diagnosis of either is made, particularly using point-of-care assays, such as rapid diagnostic tests (RDTs). We present a case of dengue and Plasmodium vivax co-infection in a returned traveler from an endemic region, in whom the epidemiology and clinical course are highly suggestive of dengue triggering a P. vivax relapse. The literature on the co-occurrence of dengue and malar

Schistosomiasis in the United States.

Canine schistosomiasis is a well-established cause of a granulomatous enteropathy and hepatopathy in dogs. In a small subset of patients, infection triggers significant hypercalcemia. Clinical signs and clinicopathologic findings are fairly nonspecific but ultrasonographic evidence of heterogenous small intestinal wall layering and pin-point hyperechoic foci in bowel, nodes, and liver is highly suggestive of infection. A sensitive, commercially available, fecal polymerase chain reaction test can be used to establish the diagnosis. Treatment protocols rely on praziquantel with fenbendazole. Mos

[Therapeutic hypothermia-induced QT prolongation and “torsade de pointes” ventricular tachycardia].

The authors describe the course of disease in a 44-year-old female patient with autoimmune disease. After successful resuscitation following unexpected hospital ventricular fibrillation, the patient remained in a comatose state and therefore received therapeutic hypothermia. Hypothermic treatment resulted in significant QT prolongation and "torsade de pointes" ventricular tachycardia. The probable cause of arrhythmia is the QT prolongation caused by the hypothermia and the consequential early afterdepolarization and triggered activity. The authors draw attention to the fact that - to the best

[Chronic pelvic pain syndrome : Treatment options using osteopathy].

Urogenital problems are dramatically increasing; especially chronic pelvic pain syndrome (CPPS) poses a major challenge for physicians and therapists. Few forms of therapy have been able to promise relief because the cause of CPPS remains unclear. Functional complaints are increasingly discussed as triggers. Osteopathic treatment has been reported to help many patients in recent years. In this article, the approach and osteopathic point of view of functional complaints are presented. In addition, possible causes for the development of CPPS and important anatomical structures that are directly

Topical review: temporomandibular disorders in an integral otic symptom model.

The literature has closely observed otic symptoms (and other craniofacial complaints) in temporomandibular disorders; however, there is little evidence for an association between the two. This review tries to provide an integrated biological basis for otic symptoms in temporomandibular disorders from both anatomical and physiological points of view; it also attempts to enlarge the view of one of the ranges of central and peripheral mechanisms involved. The pathophysiology of common symptoms is integrated within different health specialties through basic science. This review is not based on a s

[Head injury and disturbed pulmonary function (author's transl)].

Results compiled from the literature confirm the occurrence of a neurogenic initiated disturbance of pulmonary function. Neurogenic pulmonary oedema is a rare but grave form of neurogenic triggered changes of pulmonary and haemodynamic parameters. Increase of intracranial pressure or cerebral hypoxia, respectively, is the starting point of mechanisms, mediated primarily by the sympathetic nervous system and leading to hypoxaemia accompanying head injury. Pulmonary parameters involved are decreased pulmonary compliance, increased pulmonary right-to-left-shunt as well as disturbances of mechanis

Prescription Drug Monitoring Program Reminder Emails, Program Use, and Prescribing: A Randomized Clinical Trial.

IMPORTANCE: Prescription Drug Monitoring Programs (PDMPs) can contribute to safer prescribing of opioids and other controlled substances but are often underutilized by prescribers. Evidence on the feasibility and effects of email campaigns to increase clinicians' engagement with PDMP is limited. OBJECTIVE: To investigate whether email communications can increase PDMP use and, in turn, reduce guideline-discordant prescribing among clinicians. DESIGN, SETTING, AND PARTICIPANTS: This randomized clinical trial included clinicians who prescribed controlled substances but lacked PDMP accounts, had

Sensorless control of switched reluctance motor based on inductance characteristic point under magnetic saturation.

This paper presents a method of rotor position estimation for switched reluctance motors suitable for saturation. The effects of saturation as well as voltage changes are taken into account at the same time. It is based on the inductance in the unsaturated region. When the phase inductance is equal to the threshold, it is defined as a characteristic point. Meanwhile the characteristic pulse signal is triggered. Different inductance intersection thresholds are determined when the phase current and bus voltage change. The rotor position is estimated by interval speed. Compared with the tradition

Takotsubo syndrome and its linkage to neuroendocrinological disorders: a case report.

Takotsubo syndrome (TTS) is an increasingly diagnosed entity, however the underlying pathophysiological mechanisms and their clinical implications are incompletely understood. An 82-year-old female, diagnosed with pituitary apoplexy, presented with ECG abnormalities and hsTnI levels consistent with an acute coronary syndrome and therefore underwent urgent coronary angiography that showed no significant stenosis and apical ballooning at left ventricle angiogram and therefore a TTS diagnosis was made. Moreover, during catheterization a 20 s torsade de pointes was registered. TTS is an entity tha

Myxoedema Coma Masquerading as Acute Stroke.

UNLABELLED: This report describes the management of a patient with myxoedema coma masquerading as an acute stroke (with or without 'cold sepsis'). Myxoedema coma is an endocrine emergency occurring when physiological adaptations to untreated hypothyroidism are overwhelmed by an acute precipitant. Even promptly treated, it has an associated mortality of up to 50%. LEARNING POINTS: Early recognition of myxoedema coma is essential, however the chance of misdiagnosis remains high.Key management consists of rapid thyroid hormone replacement (intravenous T4 at 300-500 μg over 24 hours, followed by

Anterior Cutaneous Nerve Entrapment Syndrome Possibly Triggered by Oral Contraceptives.

We herein report a teenage girl who had been taking oral contraceptive pills for three months and complained of left lower abdominal pain that had continued for two months. A physical examination indicated anterior cutaneous nerve entrapment syndrome (ACNES), although no abnormality was found in various biochemical and imaging examinations. The pain was only transiently ameliorated by trigger-point injection, and neurectomy surgery was eventually effective. Sex steroids can be involved in the progress of local tissue edema causing ACNES. ACNES should be considered in cases of abdominal pain in

TORSADES DE POINTES ASSOCIATED WITH TAKOTSUBO CARDIOMYOPATHY IN AN ANOREXIA NERVOSA PATIENT DURING EMERGENCE FROM GENERAL ANESTHESIA.

Takotsubo cardiomyopathy, also known as stress-induced cardiomyopathy, is a disease in which the patient exhibits transient, reversible left ventricular dysfunction that is triggered by physical or emotional stress. Prolongation of QT interval, a risk factor for arrhythmia and sudden death, has been reported to be prevalent among patients with Takotsubo cardiomyopathy and is also observed in those with severe anorexia nervosa. In this report, we describe the rare case of a 30-year-old female patient with anorexia nervosa who developed Torsades de Pointes associated with Takotsubo cardiomyopath

Acquired Pulmonary Vein Isolation in a Patient with Friedreich Ataxia.

The electrophysiologic nature of atrial fibrillation (AF) and related atrial arrhythmias in Friedreich ataxia has not previously been characterized. In the presented case, dense atrial scar had progressed to the point of acquired pulmonary vein (PV) isolation before the delivery of a single radiofrequency lesion. AF was induced, and ultimately organized spontaneously into a microreentrant atrial tachycardia. Other atrial tachycardias were also identified near scar border zones; these potentially served as triggers for AF in this patient, independent of the PVs. This case emphasizes the need to

A case of a short-coupled variant of torsades de Pointes with electrical storm.

This case report describes a short-coupled variant of Torsades de Pointes with a characteristic ECG pattern consisting of a prominent J wave in leads V3-V6, in which an electrical storm was evoked with autonomic receptor stimulation and a blockade test. The patient's frequent VF attacks were triggered by short-coupled premature ventricular contractions with a right bundle branch block morphology and left-axis deviation, and were suppressed by deep sedation followed by a combination therapy using verapamil and mexiletine. Interestingly, with the use of those drugs, the prominent J wave diminish

Myofascial pain syndrome induced by malpositioning during surgery--a case report.

It is a real challenge to the anesthesiologists to differentiate brachial plexus injury (BPI) from myofascial pain syndrome (MPS). The possibility of MPS should be suspected in a patient with complaints of pain and dysfunction of the upper arm immediately after surgery. Here we report a case of gallstone with cervical ankylosing spondylitis who sustained myofascial pain syndrome (MPS) immediately after open cholecystectomy. We utilized dry needle stimulation to deactivate the trigger point of the pectoris minor muscle and stretching the muscle to relieve the muscle pain after the diagnosis was

Medial superior cluneal nerve entrapment neuropathy in teenagers: a report of two cases.

Medial superior cluneal nerve entrapment neuropathy causes pain radiating from the low back down to the posterior thigh. It tends to be misdiagnosed as a lumbar spine disorder. Patients in previous reports were in the middle or old age at the onset. Proposing simultaneous full flexion of the ipsilateral hip and knee joints as a provocation test, we present two cases of teenager females who spent long before diagnosis of their condition. Both of them had engaged in vigorous sports activities and completely recovered from the disability following local anaesthetic and corticosteroid injection at
