Anti-ulcer therapy. Past to present.

Until 1950 the clinical treatment of peptic ulcer disease relied on dieting and antacids. However, recent controlled studies suggest that the natural course of peptic ulcer disease is not affected by diet. Antacids are primarily used to relieve distress, although high- and low-dose antacid regimens have been reported to promote duodenal ulcer healing. Initial favorable reports following the introduction of synthetic anticholinergic drugs have not been confirmed. Pirenzepine is an anticholinergic compound with a specific action on the muscarinic receptors of the parietal cells. Although pirenze…

Prostaglandins, gastroduodenal ulcer and hemorrhagic gastritis.

Today, we have effective and potent drugs such as H2-receptor antagonists for the treatment of peptic ulcers. Cimetidine and ranitidine are antisecretory drugs which heal 67-90% of duodenal ulcers in 4 weeks. Certain prostaglandins (PGs) which also heal gastroduodenal ulcers and hemorrhagic gastritis not only diminish gastric acid secretion but also confer unique protective properties on the gastroduodenal mucosa. This phenomenon of 'cytoprotection' is supported by the experimental finding that PGs prevent gastroduodenal mucosal injury caused by absolute ethanol, HCl, NaOH and other irritating…

Corneal ulcer due to Achromobacter xylosoxidans.

We report a case of corneal ulcer caused by the opportunistic organism Achromobacter xylosoxidans which developed during chronic topical steroid treatment of an eye with neovascular glaucoma. A. xylosoxidans has probably been underreported as a cause of ocular infection because of confusion between this organism and other Gram-negative organisms, particularly pseudomonas. A. xylosoxidans is resistant to aminoglycosides and some cephalosporins but not carbenicillin. This difference in antibiotic sensitivity patterns between A. xylosoxidans and pseudomonas makes an accurate differentiation betwe…

Sucralfate, and cimetidine for gastric ulcer.

Eighty consecutive patients with endoscopically proven gastric ulcer were randomly assigned to treatment with sucralfate, 1 g four times daily, or cimetidine, 200 mg three times daily and 400 mg at night. The patients were endoscoped after four and eight weeks by an examiner who was unaware of the treatment in use. Of the 40 patients assigned to each treatment group, 2 in the sucralfate and 4 in the cimetidine group were withdrawn due to the finding of gastric cancer in the biopsies. Two patients in the sucralfate and 3 patients in the cimetidine group interrupted treatment. Complete ulcer hea…

HLA antigens and peptic ulcer disease.

The aim of this study was to investigate the association of HLA antigens with chronic duodenal and gastric ulcer. 104 patients with chronic peptic ulcer, 52 with duodenal ulcer, and 52 with gastric ulcer were studied, and HLA antigens of the A, B and DR series were sought using the microlymphocytotoxicity test. In duodenal-ulcer patients, no significant differences were observed. In the gastric-ulcer group, Bw49 was increased in both sexes, with an adjusted odds ratio of 10.2. However, when corrected for the number of comparisons made, this difference failed to reach statistical significance. …

Pachydermoperiostosis, hypertrophic gastropathy, and peptic ulcer.

Two brothers with pachydermoperiostosis, an autosomal dominant syndrome characterized by digital clubbing, periosteal new bone formation, coarse facial features with thick, furrowed, and oily skin, presented in their twenties with severe complicated duodenal ulcer disease requiring multiple operations. Their father and one paternal uncle also had pachydermoperiostosis and a past history of ulcer dyspepsia. The mother, one sister, two maternal aunts, and one other paternal uncle were healthy. Both brothers had giant hypertrophic gastritis (Ménétrier's disease). Their pentagastrin-stimulated aci…

Septic complications of perforated peptic ulcer.

The septic complications of peptic ulcer perforation have not been adequately described in the literature because the sparse microflora usually present in the upper gastrointestinal tract is generally believed to represent a minimal risk. One hundred and eighty-two peptic ulcer perforations (150 duodenal, 32 gastric), seen over 15 years, are reviewed. The septic complications relating to intestinal perforation included intra-abdominal abscess (22 cases), wound infection (26 cases) and generalized bacterial peritonitis (18 cases). The incidence of postoperative abscesses was significantly (p le…

Pancreatic polypeptide release in gastric ulcer.

The pancreatic polypeptide (PP) response to a protein-rich meal has been studied in similarly aged patients with gastric ulcer, duodenal ulcer, and controls. The response of the gastric ulcer patients was significantly lower at all points than that of the duodenal ulcer patients or controls, which were similar to one another. Vagal stimulation is probably the single most important factor in PP release, and it is possible that diminished PP release in the gastric ulcer patients is a reflection of vagal underactivity.

Biochemistry of antral ulcer in patients.

Separation and measurement of adenosine triphosphate (ATP), adenosine diphosphate (ADP), adenosine monophosphate (AMP), adenine-adenosine, lipid phosphates (lipid-P), RNA and DNA, furthermore, separation of Mg2+-dependent, total (Mg2+-dependent plus Na+--K+-dependent or Mg2+--Na+--K+-dependent) and Na+--K+-dependent ATPases and measured of their activities were carried out in tissue samples obtained from "ulcerated antral mucosa", from "non-ulcerated antral mucosa" and the muscular layer beneath the examined mucosa specimens, of patients with antral ulcer. It has been found that (1) the tissue…
