Hospital-acquired urinary tract infection.

From 16,534 admissions, 60 patients, 4 days to 15 years of age, with one or more hospital-acquired urinary tract infections were identified during a 5-year period by a prospective surveillance system. The patient charts were subsequently reviewed to characterize the population at risk for such infections and to describe the course and consequences of these infections. Infections in individual patients ranged from one to greater than 50. The hospital-acquired urinary tract infection rate for the study period was 14.2 infections per 1,000 admissions. In the patients in whom all urinary tract inf

Prospective Observational Study to Evaluate Post-Operative Urinary Retention With Pediatric Epidural Analgesia.

BACKGROUND AND OBJECTIVES: Epidural analgesia is an effective analgesic method for children throughout the postoperative phase. The post-operative urinary retention affects the post-operative recovery immensely. Many studies look at post-operative urinary retention in adults who receive epidural analgesia, while there is a paucity of data in children. This is a study that addresses the question of post-operative urinary retention in children in whom epidural analgesia is employed. METHODOLOGY: This is a prospective study undertaken in the pediatric surgery department of Christian Medical Coll

Health Care Utilization and Complications Rates among Users Of Hydrophilic-Coated Catheters.

This article presents results from an electronic survey completed by intermittent catheterization users who were registered members of a global web-based database. The results provide data on complication rates, health care utilization, and benefits from using hydrophilic-coated urinary catheters isotonic to urine.

Rupture of ileal neobladder due to urethral obstruction by mucous plug.

We report a case of ileal neobladder rupture after radical cystectomy due to mucus obstruction of the bladder neck. Since mucus production in bowel neobladders cannot be sufficiently influenced pharmacologically, patients with a continent urinary diversion connected to the urethra should learn self-catheterization.

Iatrogenic bladder and ureteral injuries following gynecological and obstetric surgery.

KEY MESSAGE: Ureteral injuries are often associated with complications and risk of fistula especially in case of malignancy. Length of catheterization should be reconsidered according to the injury. PURPOSE: Iatrogenic urinary tract injuries are potential complications of gynecologic and obstetric surgery. Our aim was to describe suture type and size, length of urethral catheterization, length of hospitalization, reoperation rate, follow-up, and impact on quality of life following iatrogenic bladder and ureteral injury. METHODS: In total, 81 women met inclusion criteria. Bladder injuries inc

[Management of postoperative transurethral catheterization with two technics for bladder drainage in surgery for pelvic floor relaxation].

Fifty patients with diagnoses of pelvic floor relaxation or stress urinary incontinence were randomly allocated to either transurethral closed, or transurethral open bladder drainage after vaginal surgery for pelvic floor relaxation or stress urinary incontinence. Postoperative normal bladder function resumed more rapidly when transurethral closed drainage was used (P < 0.05). Postoperative use transurethral closed bladder drainage significantly reduced urinary tract infection (P < 0.05), so most of the patients didn't need bladder catheterization at time of discharge, as opposed to more than

No drains in thoracic surgery with ERAS program.

Enhanced recovery after lobectomy surgery (ERAS) concept has been greatly developed between clinical implementation and minimally invasive surgery. In addition to the minimally invasive surgery, the management of the perioperative catheter has also attracted everyone's attention. Tubeless minimally invasive treatment includes no urinary catheter placement during the operation and no chest tube after the operation. Here, we summarized all the reports on no urinary catheterization and no chest tube in patients with thoracic surgery and the impact of postoperative length of stay (LOS) and postope…

Ileovesicostomy: update.

Currently, we have different technical options for treating clinically complex scenarios such as neurogenic bladder, difficult to manage lower urinary tract obstruction and many disasters sometimes seen after prostate cancer treatment. During the seventies, clean intermittent bladder catheterization was established as the best treatment option for these patients. As a method, suprapubic urinary diversion has undergone a major evolution over the past fifty years trying to solve the most refractory and poor prognosis cases. Continent ileovesicostomy is an easy emptying and low-pressure non-cathe

Asymptomatic ureteral rupture secondary to chronic urinary retention from massive prostatic enlargement.

Non-traumatic ureteral rupture has been reported more frequently, resulting from increased intraluminal pressures from distal urinary tract … sence of specific symptoms, the patient had successful conservative management with antibiotics and urinary catheterization for his acute episode, although declined further surgical intervention.

Urinary reconstruction in vertebral, anorectal, cardiac, trachea-esophageal, renal abnormalities and limb defects association with chronic renal failure and penile duplication.

Various urological complications in VATER association require careful management. A 15-year-old boy with VATER association, including a hypoplastic lower urinary tract and diphallia, presented with chronic kidney disease and incontinence after a right loop ureterostomy. In order to acquire urinary continence without renal function impairment, an ileocecal reservoir with umbilical catheterizable stoma was created as a urinary reconstruction. The ectopic posterior penis was resected for cosmetic reasons, and the stump of the hypoplastic urethra was opened at the perineal skin. Clean intermittent

Meningitis-retention syndrome as a presentation of west nile virus meningitis.

A 26-year-old previously healthy man presented with fever, urinary retention, nuchal rigidity, and hyperreflexia but with a clear sensorium. His initial spinal fluid results were consistent with aseptic meningitis from West Nile virus infection, and this was confirmed by serological studies on blood and cerebrospinal fluid. Computed tomography and magnetic resonance imaging studies were unremarkable. He received supportive care and urinary catheterization to prevent bladder injury from overdistension. He was discharged home without recurrence of urinary retention after five days of hospitaliza…

The use of urinary sheaths in male incontinence.

Overall prevalence of urinary incontinence in men is 17%, rising to 31% in those men aged 85 years old or older (Anger et al, 2006). Penile sheaths, sometimes referred to as 'condom catheters', provide a reliable form of management for those men with intractable urinary incontinence. A penile sheath is a soft, flexible sleeve that fits over the penis with an anti-reflux bulbous end which culminates as a short tube that attaches to any standard urinary drainage system. Penile sheaths provide clinicians with a useful alternative to long-term urethral catheterization as they are non-invasive and

Use of the appendix for a catheterization stoma in partial quadriplegics.

Continent reconstruction of the lower urinary tract using the appendicovesicostomy (Mitrofanoff principle) is an excellent means of managing many patients with altered bladder function, especially those with good bladder volume and low pressure. Motivated patients unable to perform urethral catheterization because of paresis or other reasons now have a method of urinary diversion free of an appliance, though additional procedures may be necessary if bladder volume is low and/or bladder pressure is high. We no longer routinely perform appendectomy during bladder substitution or bladder augmenta

Use of bowel in undiversion.

Urinary undiversion is becoming an uncommon procedure because fewer patients are being diverted. The lessons learned from undiversion, however, have made possible early primary reconstructions in patients who would previously have been candidates for diversion. Intermittent clean catheterization and the use of bowel to augment or form a compliant storage vessel for urine have permitted reconstruction in patients with abnormal bladder and urethral function. Thus patients with neurogenic bladder or with severe lower urinary tract abnormalities can look forward to healthy kidneys and urinary cont
