Vector-control synergies, between 'roll back malaria' and the Global Programme to Eliminate Lymphatic Filariasis, in the African region.

The perspectives and opportunities for controlling the mosquito vectors of Wuchereria bancrofti in tropical Africa are summarized and discussed. The countries covered by the World Health Organization's African Region have about one third of the world's burden of lymphatic filariasis (LF) as well as large shares of the planet's malaria and of many other vector-borne diseases. African LF is entirely caused by nocturnally periodic W. bancrofti, filarial nematode that is transmitted in urban East Africa by Culex quinquefasciatus, and in rural areas across tropical Africa by the same anopheline spe…

Bancroftian filariasis in the Kassena Nankana District of the upper east region of Ghana: a preliminary study.

A filariasis prevalence survey was conducted in April 1992 in the Southern sector of the Kassena Nankana District which was the site of the … howed an overall microfilaraemia rate of 41.1% (95% CI 36.9-45.3%). The only species identified was Wuchereria bancrofti. The most important clinical manifestation was hydrocele, 30.8% of males (95% CI 25-36.6%); followed by elephantiasis of the leg, 3.6% of the study population (95% CI 2.0-5.2%). The mean haemoglobin level of the population was 12.4 g dl-1. …

Comparison of immunoblot and immunoprecipitation methods for analyzing cross-reactive antibodies to filarial antigens.

Qualitative analysis of antibody responses in helminth infections is essential not only for developing better immunodiagnostic antigens but also for understanding immune recognition and its relevance to immunopathogenesis and protective immunity. In this study 2 qualitative analytic methods (immunoprecipitation and immunoblotting) were compared for the ability to define the extent of cross-reactivity in the serum antibodies from patients with various forms of filariasis (caused by Brugia malayi, Wuchereria bancrofti, Loa loa and Tetrapetalonema perstans) or other non filarial helminth infectio…

IgE antibodies are more species-specific than IgG antibodies in human onchocerciasis and lymphatic filariasis.

To explore the relative species specificities of the IgE and IgG antibody responses to helminth infections in man, we studied four pools of sera from patients infected with Wuchereria bancrofti, Brugia malayi, Onchocerca volvulus or Ascaris lumbricoides and ten individual sera from patients with onchocerciasis. IgE antibodies were detected by radioallergosorbent test (RAST) analysis and IgG antibodies by a Staphylococcus protein A radioimmunoassay (Staph A-RIA). Analysis of the binding curves with four different immunosorbents (prepared from antigens of B. malayi, O. volvulus, Dipetalonema vit…

IgG4 antibodies to the recombinant filarial antigen Wb-Bhp-1 decrease dramatically following treatment of lymphatic filariasis.

BACKGROUND: Lymphatic filariasis (LF) is a neglected tropical disease and a major cause of chronic disability. Improved diagnostic tests are needed because of long-term persistence of anti-filarial antibodies or circulating filarial antigenemia after treatments that clear microfilaremia. Here, we assess changes in levels of antibodies to the recombinant filarial antigens Wb-Bhp-1, Wb123, and Bm14 after anti-filarial treatment. METHODOLOGY/PRINCIPAL FINDINGS: IgG4 antibodies to recombinant filarial antigens were assessed by ELISA. We tested serial plasma samples from a clinical trial in Papua

Lymphatic filariasis in Brazil: epidemiological situation and outlook for elimination.

Since the World Health Assembly's (Resolution WHA 50.29, 1997) call for the elimination of lymphatic filariasis by the year 2020, most of the endemic countries identified have established programmes to meet this objective. In 1997, a National Lymphatic Filariasis Elimination Plan was drawn up by the Ministry of Health of Brazil, creating local programs for the elimination of Bancroftian filariasis in areas with active transmission. Based on a comprehensive bibliographic search for available studies and reports of filariasis epidemiology in Brazil, current status of this parasitic infection and

An occult filarial infection presenting as chyluria with proteinuria: a case report and review of literature.

Chyluria is the passage of chyle into urine, and develops as a result of communication between the lymphatic system and the urinary system. It is an unusual manifestation of lymphatic filariasis reported mainly from South Asian countries. We report the case of a 38-year-old man from an endemic area who presented with passage of milky urine. Physical examination did not reveal any lymphadenopathy or lymph oedema. Urine tests revealed nephrotic range proteinuria. A 99m technetium sulphur colloid lymphoscintigraphy confirmed connection between lymphatic vessels and the urinary tract. Predominant

Activation and regulation of toll-like receptors (TLRs) by helminth parasites.

Helminth (worm) infections are major public health problems that have important socioeconomic consequences for the more than 2 billion infected individuals. Chronicity (their hallmark) can lead to anemia (in hookworm infection), river blindness (onchcerciasis), cirrhosis (schistosomiasis), and elephantiasis (lymphatic filariasis). Although there have been many studies examining innate immune responses (including TLR expression and function) in response to intracellular pathogens, fewer have examined the interaction of the multicellular helminth parasites and the innate immune system. This revi

Global eradication of lymphatic filariasis: the value of chronic disease control in parasite elimination programmes.

The ultimate goal of the global programme against lymphatic filariasis is eradication through irrevocable cessation of transmission using 4 to 6 years of annual single dose mass drug administration. The costs of eradication, managerial impediments to executing national control programmes, and scientific uncertainty about transmission endpoints, are challenges to the success of this effort, especially in areas of high endemicity where financial resources are limited. We used a combined analysis of empirical community data describing the association between infection and chronic disease prevalen

Evidence for the interruption of transmission of lymphatic filariasis among schoolchildren in Trinidad and Tobago.

This study was carried out to provide some evidence for the interruption of transmission of lymphatic filariasis (LF) among schoolchildren in Trinidad and Tobago. A cross-sectional survey for LF antigenaemia was performed among 63 (13.2%) of the 479 primary schools located in eight administrative (and geographical) regions of Trinidad and Tobago. From these communities, 2597 schoolchildren aged 6-12 years were sequentially selected for a survey of bancroftian antigenaemia. From each child, 100 microl (finger-prick) whole blood sample was applied to a Binax immunochromatographic card test (ICT)

Ultrasound diagnosis of filarial funiculoepididymitis.

Genital presentation of filarial disease is not uncommon in endemic areas of the world. Acute, febrile illness involving the epididymis and spermatic cord (funiculoepididymitis) is one of many such presentations. With an internationally mobile society, physicians today, even in nonendemic areas, may encounter patients with filarial infestations. We report the first case of presumptive diagnosis of this disease using scrotal ultrasound.

The association between microfilaraemia and disease in lymphatic filariasis.

A standard tenet in the epidemiology of lymphatic filariasis is that patent infection is negatively related to chronic disease. We examine the empirical evidence for this relation by using published data from field studies carried out in a variety of bancroftian filariasis endemic areas. Meta-analysis of the individual study results for each disease category of hydrocele in males only, lymphoedema only, and both conditions combined (total chronic disease) indicate, contrary to expectation, no evidence for a negative association between infection and disease. Indeed, the trend of the empirical
