The long-term growth rate of residual acoustic neurinomas.

The growth rate of 19 residual acoustic neurinomas was examined in a long-term follow-up study (median, 10 years; range, 5 to 17 years) following intracapsular removal. Of these, 10 (53%) had regrowth, three (16%) showed regression, and six (32%) were unchanged. The 10 acoustic neurinomas showing regrowth were divided into two categories, either solid or cystic, according to computed tomographic findings. Five acoustic neurinomas with cyst formation showed rapid regrowth, with the tumour doubling time ranging from 0.15 to 5.0 years (median, 4.5 years), and required re-operation. Five solid tum

Cerebrospinal fluid leak following acoustic neuroma removal.

Cerebrospinal fluid (CSF) leak has been a constant and unresolved complication of acoustic tumor surgery. This study retrospectively reviews 381 primary acoustic tumor surgeries performed by a single, senior, neurotologist and neurosurgeon team from 1979 through 1991. There were 68 cerebrospinal fluid leaks in 66 patients (66/381; 17%). There was no significant difference in the incidence of CSF leak between the translabyrinthine group (21%) and the retrosigmoid transmeatal group (16%). Translabyrinthine leaks were evenly divided between rhinorrhea and the postauricular wound while retrosigmoi

Linear accelerator-based stereotactic radiosurgery for acoustic schwannomas.

PURPOSE: Stereotactic radiosurgery (SRS) is currently being investigated for treatment of acoustic schwannomas in patients who are not good surgical candidates. The vast majority of the available data is based on gamma knife-treated patients. We present the largest series of patients treated with linear accelerator-based SRS. METHODS AND MATERIALS: Thirty-two patients with acoustic schwannomas were treated with SRS between July 1988 and February 1993; follow-up ranged from 4-59 months. Age ranged from 34-88 years (mean, 62 years). The primary presenting symptom was hearing loss in 30 patients

[Auditory results after transtemporal removal of acoustic neurinoma].

A retrospective study was conducted to assess results of hearing in patients who underwent resections of acoustic neuromas via the middle fossa approach. A statistical correlation of results with preoperative clinical and audiological data determined if any prognostic indicators could be associated with successful preservation of hearing. Of 93 patients included in the study, useful hearing was preserved in 54 (58%), while hearing was preserved within 10 dB of preoperative levels in 42 patients (45%). The possibility for hearing preservation appeared to be inversely related to the size of the

Determinants and impact of headache after acoustic neuroma surgery.

Headache after acoustic neuroma surgery is known to occur clinically, but has not been studied systematically until recently. In the present study, 155 patients were surveyed regarding their experience of headache and associated symptoms following resection of an acoustic neuroma: 73 percent (n = 98) of patients undergoing suboccipital resection of an acoustic neuroma and 53 percent (n = 8) of patients undergoing translabyrinthine resection of acoustic neuroma complained of headache following surgery. The average pain intensity was greater for the suboccipital approach. Only 9 percent (n = 14)

Comprehensive management of bilateral acoustic neuromas. Current perspectives.

Bilateral acoustic neuromas are pathognomonic for neurofibromatosis 2. Patients with neurofibromatosis 2 present complex and challenging management problems, because growth or surgical removal of the acoustic neuroma may result in total hearing loss. Early diagnosis with gadolinium-enhanced magnetic resonance imaging and refinements in hearing preservation surgery have improved our ability to prevent total hearing loss. For patients with larger tumors or no useful hearing, the auditory brain-stem implant allows restoration of some auditory function when the tumor is removed. We describe our ma

Recurrent acoustic neuroma presenting as central alveolar hypoventilation.

Centrally mediated hypoventilation causes respiratory failure without respiratory distress. We present a case of recurrent acoustic neuroma at the cerebellopontine angle causing acute and chronic respiratory failure. Tumor resection eliminated recurrence of respiratory failure.

Acoustic neuroma in an adolescent without neurofibromatosis: case study.

When acoustic neuromas are found in younger age groups, they are typically bilateral and associated with neurofibromatosis (NF). A unilateral acoustic neuroma in a child or adolescent without NF is rare. We report audiometric, auditory brainstem response (ABR) and magnetic resonance imaging (MRI) data for a 15-year-old male with an acoustic neuroma but without associated NF. The patient was seen for audiologic assessment on three separate occasions over a period of 5 years. The first two assessments produced unremarkable audiometric and immittance data. The third assessment, subsequent to a fa

Cell division in the gerbil cochlea after acoustic trauma.

The recent discovery of hair cell regeneration in the avian inner ear raises the possibility that hair cell regeneration might occur in the mammalian cochlea as well. The authors used 3H-thymidine labeling to detect mitotic activity in the cochleas of normal 3-week old gerbils exposed to acoustic trauma. Following an acoustic insult that caused progressively more severe damage in an apical to basal progression, 3H-thymidine was injected for 5 days. Control animals were not exposed to the acoustic insult. The gerbils' cochleas were sectioned and processed for autoradiography. In the control coc

[The sub-occipital approach in functional surgery of acoustic neuroma].

In this study the Authors analyze 380 acoustic neuroma removals carried out from 1972 to 1992 focusing their attention on 90 attempts to save hearing by employing a suboccipital approach. In this series the facial nerve was preserved in 99% of the cases with completely normal function in 78%. The cochlear nerve was anatomically preserved in 96% of the subjects. According to the Shelton-Brackmann classification applied to evaluate hearing results, good hearing (Class A = PTA < or = 30 dB; SDS > or = 70%) was obtained in 12% of the cases, serviceable hearing (Class B = PTA < or = 50 dB; SDS > or

The changing clinical presentation of acoustic tumors in the MRI era.

The application of magnetic resonance imaging (MRI) scanning in the diagnosis of acoustic neuroma (AN) has increased the relative incidence of smaller tumors and has impacted on the typical clinical presentation of AN patients. The charts of 126 patients treated at the University of California, San Francisco for newly diagnosed AN from 1986 to 1990 were reviewed. Twenty-four percent of tumors fell into the smallest size category (< 1 cm); this was a substantial improvement over earlier series. However, 16% of tumors remained undiagnosed until they achieved large size (> 3 cm). The incidence of

Acoustical detection of coronary occlusions using neural networks.

A nonlinear neural network classifier was applied to noninvasive acoustic detection of coronary artery disease; the classifier included a feature vector, derived from diastolic heart sounds, and a multi-layered network trained by the backpropagation. The feature vector is based on the linear prediction coefficients of the autoregressive method after an adaptive line enhancement method was used as the input pattern to the neural network. One hundred and twelve recordings (70 abnormal, 42 normal) were studied and the network was trained on a randomly chosen set of six abnormal and six normal pat

Acoustic and perceptual characteristics of voicing in fricatives and fricative clusters.

Several types of measurements were made to determine the acoustic characteristics that distinguish between voiced and voiceless fricatives in various phonetic environments. The selection of measurements was based on a theoretical analysis that indicated the acoustic and aerodynamic attributes at the boundaries between fricatives and vowels. As expected, glottal vibration extended over a longer time in the obstruent interval for voiced fricatives than for voiceless fricatives, and there were more extensive transitions of the first formant adjacent to voiced fricatives than for the voiceless cog

Clinical observations on the on-off effect of the acoustic stapedius reflex.

The on-off effect of the acoustic stapedius reflex was found in association with several kinds of ear disease: otosclerotic stapes fixation, incudostapedial dislocation and sensorineural hearing loss. One fifth (8/40) of the patient group with an on-off effect and sensorineural hearing loss showed the typical symptoms of Ménière's syndrome.

[Acoustic trauma from firearms: study of auditory function].

This article deals with the shift in audiometric tracings brought about by the discharge of firearms (acute acoustic trauma). The study included 60 subjects with normal hearing, between 18 and 20 years of age. Each individual shot 25 bullets in about 5 minutes, at an intensity level calculated at 163 dB. None of them related antecedents of sensorineural deafness. The audiometric examinations were performed as follows: the first one prior to the experiment, the second immediately after the series of shots, and others 8 and 40 hours later. These are the conclusions drawn out by the AA.: the most

[Massive hemorrhage within an acoustic neurinoma. Case report].

A 39-year-old male experienced unilateral right hearing loss and tinnitus for 7 years and was hospitalized after he suddenly developed severe headache, vertigo, and right facial paralysis. Computed tomography (CT) showed a round, high-density are in the right cerebellopontine angle. Magnetic resonance (MR) imaging demonstrated a crescent-shaped region of high signal intensity, representing hemorrhage, in the superior aspect of the tumor, surrounded by edema. The right internal auditory canal was enlarged. Four-vessel angiography disclosed neither an aneurysm nor an arteriovenous malformation.

Facial nerve recovery following acoustic neuroma surgery.

A series of 76 consecutive patients undergoing unilateral acoustic neuroma excision is reported. Preoperative facial weaknesses and a tumour larger than 2.5 cm were predictive factors of poor facial nerve recovery, re-emphasising the need to detect and remove these tumours at an early stage. Electroneuronography performed around one week after surgery appears to be a good indicator of the likelihood of recovery in facial nerves which are anatomically intact, but which show no function at that stage. Multiple rehabilitative procedures are often required when inadequate function or poor cosmetic

Surgery for acoustic neurinoma. An analysis of 100 translabyrinthine operations.

A consecutive series of 100 translabyrinthine operations for removal of acoustic neurinoma is reported. Complete tumor removal was achieved in 97% of cases. There were three perioperative deaths, and the postoperative morbidity rate was low. The facial nerve was preserved anatomically in 82% of patients. Preoperative facial weakness and tumor size larger than 2.5 cm were predictive of poor facial recovery. In patients with an intact nerve but complete facial palsy 1 week after surgery, electroneuronography was a good predictor of the final facial outcome and may influence the timing of rehabil
