Supporting rural families during interhospital patient transfers for critical illness events: An exploration of an acceptable communication process.

UNLABELLED: Critically ill patients in rural areas at times require an interhospital transfer from their local hospital to an urban tertiary care centre for advanced critical care services not available locally. Family members have described this transfer window as a communication blackout and one of the most stressful times of their relative's critical illness event. OBJECTIVE: To explore what communication process would be most acceptable between family members and transfer team members (consisting of critical care nurses, paramedics, and physicians) during interhospital transfers of critic

[Training modules on climate change, heat, and health for medical assistants and nurses in outpatient care].

The number of heatwaves is going to increase due to climate change and will pose a high health risk especially for elderly people. Additional risk factors like immobility, the need for care, chronic and acute diseases (worsening of symptoms), and the intake of medications lead to an increased sensitivity to heat in this particular age group. Nursing staff and medical assistants working in general practices are two important professions to reach the risk group during heatwaves and provide preventive and curative care. The "Klinikum der Universität München" has developed an interprofessional blen

Point prevalence of wounds in a sample of acute hospitals in Canada.

To provide new information on wound prevalence and the potential resource impact of non healing wounds in the acute sector by summarising results from wound audits carried out at 13 acute hospitals in Canada in 2006 and 2007. Audits were carried out in each hospital by the same independent team of advanced practice nurses using standard data-collection forms. The results reported here were derived from the summary reports for each hospital. A total of 3099 patients were surveyed (median 259 patients per hospital). In the sample hospitals, the mean prevalence of patients with wounds was 41.2%.

A Qualitative Study of the Systems-Based Practice Competency in United States Graduate Medical Education.

Background Systems-based practice (SBP) is a core competency in U.S. graduate medical education (GME), but it is not well implemented; we examined current factors that need to be addressed to advance SBP. Methods  Between March and July 2021, 24 physicians, nurses, educators, and leaders in the field of SBP were interviewed using a semi-structured guide. Individuals were selected based on their influence on the origin or development of SBP. The authors iteratively collected and analyzed data using real-time analytic memos, regular adjudication sessions, and thematic analysis. Themes were agree

Progress in Pharmacologic Management of Neuropsychiatric Syndromes in Neurodegenerative Disorders: A Review.

IMPORTANCE: Neuropsychiatric syndromes (NPSs) are common in neurodegenerative disorders (NDDs); compromise the quality of life of patients and their care partners; and are associated with faster disease progression, earlier need for nursing home care, and poorer quality of life. Advances in translational pharmacology, clinical trial design and conduct, and understanding of the pathobiology of NDDs are bringing new therapies to clinical care. OBSERVATIONS: Consensus definitions have evolved for psychosis, agitation, apathy, depression, and disinhibition in NDDs. Psychosocial interventions may

MediaPipe-Based Activity Analysis by Healthcare Professionals: Method Development and Technology Acceptance-A Pilot Study.

Integrating digital assessment technologies into clinical practice remains a challenge because of usability issues, time constraints, and difficulties in data interpretation. This study aimed to examine the feasibility of a clinically practical MediaPipe-based motion analysis method and to evaluate its technological acceptance by healthcare professionals. We tested an analysis method using single-camera frontal-plane videos of gait, eating, and table-slide tasks to quantify movement under both normal and simulated impairment conditions. To evaluate the clinical utility, a questionnaire based o

Teamwork Entrustable Professional Activities in Interprofessional Teams Involving Trainees and Advanced Practice Providers in the Intensive Care Unit.

BACKGROUND: Most academic medical centers rely on both trainees and advanced practice providers (APPs) (nurse practitioners and physician assistants) to provide patient care; however, trainees and APPs are rarely guided on how to work effectively together in teams. In our medical intensive care unit (MICU), planned changes to include trainees and APPs on blended teams presented a unique opportunity to design and evaluate interprofessional teamwork. AIM: We developed entrustable professional activities (EPAs) related to effective teamwork and psychological safety to both promote an inclusive c

A Review of Interprofessional Variation in Education: Challenges and Considerations in the Growth of Advanced Practice Providers in Emergency Medicine.

The employment and utilization of advanced practice providers (APPs) in the emergency department has been steadily increasing. Physicians, physician assistants (PAs), and nurse practitioners (NPs) have vastly different requirements for admission to graduate programs, clinical exposure, and postgraduate training. It is important that as supervisory physicians, patients, hospital administrators, and lawmakers, we understand the differences to best create a collaborative, supportive, and educational framework within which PAs/NPs can work effectively as part of a care team. This paper reviews the

A cross-sectional study of transplant clinicians' moral distress and grit.

BackgroundWhen transplant clinicians encounter ethical challenges and are constrained by their institution from acting in a manner within their professional ethical obligations, they may experience moral distress. Moral distress has been associated with negative effects such as burnout and turnover. Conversely, high grit has been positively correlated with job satisfaction, job performance, and organizational commitment.Research questionsWhat is the level of grit and moral distress among transplant clinicians? What correlations between demographics, grit, moral distress, and intent to leave ex

The effect of work characteristics on advanced practice provider burnout: a secondary cross-sectional analysis.

BACKGROUND: There are few comprehensive studies looking at burnout and well-being among advanced practice providers (APPs). Most of the studies focus on physicians and nurses. The prevalence of burnout in APPs is limited largely to single center studies or certain APPs specialty. Determining drivers of burnout amongst a heterogenous APPs workforce may strengthen the ability to customize support and interventions. In a multicenter randomized control trial studying the feasibility and impact of online group coaching on APPs, we analyzed baseline data to identify work characteristics associated w

Factors Contributing to CPOE Opiate Allergy Alert Overrides.

CONTEXT: Increasing regulatory incentives to computerize provider order entry (CPOE) and connect stores of unvalidated allergy information with the electronic health record (EHR) has created a perfect storm to overwhelm clinicians with high volumes of low or no value drug allergy alerts. Data sources include the patient and family, non-clinical staff, nurses, physicians and medical record sources. There has been little written on how to collect hypersensitivity information suited for drug allergy alerting. Opiates in particular are a frequently ordered class of drugs that have one of the highe

Exploring the use of observational tools for advancing patient safety learning among preregistration healthcare students: a scoping review using the 3P model of teaching and learning.

BACKGROUND: Patient safety is a global health priority and a fundamental part of healthcare curricula worldwide. While teaching medical students on theoretical components is straightforward, students may fail to prioritise patient safety due to an incomplete understanding and experience of the pressures of front-line care. Using an observational tool for patient safety learning in real clinical environments appears to help students engage with the significance of safe practice. This review seeks to identify learning approaches that use observational safety tools in practice. METHODS: This scop

Develop a diagnostic tool for dementia using machine learning and non-imaging features.

BACKGROUND: Early identification of Alzheimer's disease or mild cognitive impairment can help guide direct prevention and supportive treatments, improve outcomes, and reduce medical costs. Existing advanced diagnostic tools are mostly based on neuroimaging and suffer from certain problems in cost, reliability, repeatability, accessibility, ease of use, and clinical integration. To address these problems, we developed, evaluated, and implemented an early diagnostic tool using machine learning and non-imaging factors. METHODS AND RESULTS: A total of 654 participants aged 65 or older from the Nu

Usability and acceptability of mixed reality to support emergency clinicians during community outreaches: A quality improvement project (2500-5000 words).

BACKGROUND AND PROBLEM STATEMENT: The global population is ageing, and Australia's older population continues to experience the highest growth in emergency department (ED) presentations. Older adults have a higher level of vulnerability and are at greater risk of hospital acquired complications. LOCAL PROBLEM: The Older Persons Emergency Network Acute Outreach Service (OPEN AOS) provides ED substitution and ambulance co-response to older persons in their home to avoid unnecessary transfers to ED. Since establishment, service demand has grown and cost-effective initiatives to increase service
