OBJECTIVE: How far the results of randomized controlled studies apply to everyday care cannot be judged without regular measurements of outc … achieving at least one remission during the 3-year follow-up period was 96.5% for PD and 95.9% for Agoraphobia. Relapses after a period of at least 2 months of complete remission were also common, and the probability of presenting at least one relapse during the 3-years follow-up period was 67.1% for PD and 39% for Agoraphobia. The longest period of remissio …

Long-term disability in anxiety disorders.

BACKGROUND: This longitudinal study aims to investigate differences in long-term disability between social anxiety disorder (SAD), panic disorder with agoraphobia (PDA), panic disorder without agoraphobia (PD), generalized anxiety disorder (GAD) and multiple anxiety disorders (multiple AD), focusing on the effects of different course trajectories (remission, recurrence and chronic course) and specific symptom dimensions (anxiety arousal and avoidance behaviour). METHODS: Data were used from participants with no psychiatric diagnosis (healthy controls, n = 647) or with a current anxiety disord…

OBJECTIVE: Panic disorder (PD) is an anxiety disorder characterized by sudden attacks of intense fear. Biochemical studies suggest that oxid … nalyzed with real-time polymerase chain reaction. Severity of PD symptoms was assessed by Panic and Agoraphobia Scale (PAS). RESULTS: No significant relationship was found in genotype distributions of OGG1 Ser326Cys and GPX1 Pro198Leu polymorphisms between PD and control groups (p > 0.05). There was no significant relationship between OGG1 or GPX1 polymorphi …

Lifetime and six-month prevalence of mental disorders in the Munich Follow-Up Study.

The Lifetime and 6 month DSM-III prevalence rates of mental disorders from an adult general population sample of former West Germany are reported. The most frequent mental disorders (lifetime) from the Munich Follow-up Study were anxiety disorders (13.87%), followed by substance (13.51%) and affective (12.90%) disorders. Within anxiety disorders, simple and social phobia (8.01%) were the most common, followed by agoraphobia (5.47%) and panic disorder (2.39%). Females had about twice the rates of males for affective (18.68% versus 6.42%), anxiety (18.13% versus 9.07%), and somatization disorder…

Higher postdexamethasone serum cortisol levels in agoraphobic than in nonagoraphobic panic disorder patients.

The dexamethasone suppression test (DST) was performed in panic disorder (PD) patients with (n = 32) or without (n = 31) agoraphobia and in normal controls (n = 49). Postdexamethasone serum cortisol levels were significantly higher in agoraphobic PD patients (105.3 +/- 19.3 nmol/L) both when compared to PD patients without agoraphobia (47.3 +/- 7.7 nmol/L; p less than 0.01) and when compared to healthy controls (51.7 +/- 8.3 nmol/L; p less than 0.01). The rate of nonsuppressors (i.e., subjects displaying postdexamethasone cortisol levels greater than 138 nmol/L) was 28% and 3% in agoraphobic a…

Exploring the relationship between asthma, its severity and anxiety symptoms in pediatric patients: a case-control study.

BACKGROUND: Asthma is a global health concern, especially among children, and is associated with various underlying mechanisms. Childhood ex … ysis revealed elevated scores in separation anxiety (SA; p = 0.025), social phobia (SP; p < 0.001), agoraphobia (p = 0.004), and fears of physical injury (p < 0.001) in children with asthma. Furthermore, increased need for SABA, frequency of nocturnal symptoms, and asthma severity were associated with higher anxiety levels in pediatric asthma patients. CONCL …

An open-label study of tiagabine in panic disorder.

γ-aminobutyric acid (GABA) has been implicated in the pathophysiology of anxiety disorders, including panic. Tiagabine, a selective GABA reuptake inhibitor (SGRI), has been shown to reduce symptoms of anxiety. This pilot study evaluated the efficacy and safety of tiagabine in patients with panic disorder. Male and female outpatients aged 18-64 years with a DSM-IV diagnosis of severe to moderately severe panic disorder (with or without agoraphobia) received open-label tiagabine 2-20 mg/day for 10 weeks. Outcome assessments included the Sheehan Panic Disorder Scale (SPS), Panic Disorder Severity…

Cocaine-associated panic attacks in methadone-maintained patients.

The incidence of panic attacks methadone-maintained patients has increased over a 10-year period from 1 to 6-13%. Cocaine use appears to be associated with this increase, although other environmental and constitutional factors may be contributory. Patients with cocaine-associated panic differ from other panic patients in rates of psychiatric hospitalization and medical illness, but not in depression, other drug use, or agoraphobia.

Primary and secondary social phobia: psychopathologic and familial differentiations.

We compared "primary" social phobic patients (n = 25) with patients whose social phobia developing in the setting of panic disorder ("secondary" social phobics, n = 26) and panic-agoraphobic patients (n = 82). Familial, demographic, and symptomatologic data suggest a categorical distinction between the two types of social phobia. Secondary social phobics were essentially similar in all parameters to panic disorder with agoraphobia. These data support DSM-III conventions.

Menstrual fluctuation in the symptoms of panic anxiety.

Ten women with DSM-III-defined panic attacks (five with and five without agoraphobia) had symptom severity rated daily, weekly, and retrospectively through one full menstrual cycle. Substantial fluctuations in retrospective ratings of severity were observed, with the premenstrual week being rated as most severe. Daily and weekly ratings showed much smaller fluctuations in the predicted direction. Possible reasons for this outcome are considered.

Psychosocial treatment of phobia and panic disorders.

Intervention packages combining cognitive restructuring techniques and exposure exercises have become the psychosocial treatments of choice for panic, agoraphobia, and social phobia. Exposure and related desensitization techniques are the treatment of choice for all variants of simple phobia. Combining psychotropic medication and cognitive-behavior therapy suggests strategies that offer new opportunities for increasing the number of individuals with anxiety disorders who can be effectively treated with a biobehavioral approach.

UCS inflation in the aetiology of a variety of anxiety disorders: some case histories.

This paper reports a number of case histories which illustrate the involvement of UCS inflation processes in the aetiology of a variety of anxiety disorders including agoraphobia, simple phobia, PTSD, obsessive-compulsive disorder and panic disorder. These cases are discussed in the context of: (i) renewed interest in cognitive features of conditioning models of anxiety disorders; and (ii) the implications that such models might have for an understanding of the aetiology and treatment of anxiety-based disorders.

Further evidence for an association between affective disorders and anxiety disorders: review and case reports.

The relationship between anxiety and affective disorders continues to be debated in the literature. The authors report a case of identical twins concordant for attention deficit disorder and somnambulism but discordant for major depression and agoraphobia with panic attacks. The authors discuss the implications of this "finding in nature." Results from neuroendocrinological, family, and twin studies are also reviewed that either lend support or question the existence of a relationship between anxiety disorders and affective disorders.

Social support and married agoraphobic women.

This study was conducted to determine if a relationship exists between social support and agoraphobia. Married agoraphobic women from a support group for phobics were compared with married, non-agoraphobic women from Family Practice clinics, and social support factors were examined. The agoraphobic women were found to perceive their husbands as less supportive. As well, there were important reductions in network size and network support in these agoraphobic women.
