The interproximal height of bone: a guidepost to predictable aesthetic strategies and soft tissue contours in anterior tooth replacement.

Enhanced aesthetic objectives can be achieved with precision and predictability due to recent advances in restorative materials and procedures. Although these developments have expanded the therapeutic options available to practitioners and their patients, anterior hard and soft tissue deformities in the aesthetic zone continue to represent a significant technical challenge to the reconstructive team. The objective of this article is to present diagnostic and prognostic criteria that emphasize the osseous-gingival relationship as a means to achieve predictable aesthetic results in the anterior

A Proposed Parameter-Based Planning Framework for Inferior Alveolar Nerve Repositioning.

Background: Rehabilitation of the severely atrophic posterior mandible remains challenging when the residual bone height above the inferior alveolar nerve (IAN) is insufficient for conventional implant placement. In such cases, IAN lateralization or transposition may facilitate placement of longer implants and support fixed prosthetic rehabilitation. However, these procedures require careful case selection because of their technical complexity and the risk of neurosensory complications. Objective: This narrative review presents a literature-informed, expert-derived, parameter-based clinical de

Analog emergence profile conditioning after simultaneous hard- and soft-tissue augmentation: A 5-year clinical report.

A biologically guided, time-efficient analog workflow for anterior implant rehabilitation is presented. Two implants were placed in a horizontally deficient anterior maxilla with simultaneous guided bone regeneration and a de-epithelialized connective tissue graft to restore ridge contour and increase mucosal thickness. After healing, the transmucosal contour was shaped extraorally on a silicone index and transferred intraorally using custom interim restorations with controlled soft-tissue refinement. The emergence profile was captured with a custom impression coping, and multilayer zirconia c

The use of short dental implants in clinical practice: literature review.

When anatomic structures and ridge resorption limit the placement of a standard implant, the clinician can apply augmentation techniques or use short implants. A literature review was carried out to evaluate the differences in survival rate and the rational use of short implants. Electronic search (MEDLINE) and manual search have been performed to select papers from 2000 to 2008. Of all the inclusion criteria the most relevant were: 1) studies with data on short implants; 2) studies on humans; 3) prospective, longitudinal, retrospective and multicenter studies; 4) no restrictions were applied

Sinus augmentation analysis revised: the gradient of graft consolidation.

OBJECTIVE: Graft consolidation follows a gradient that reflects the properties of bone substitutes at sites of sinus augmentation. Here we present an analytical method to investigate the process of graft consolidation taking the distance from the maxillary host bone into account. MATERIAL AND METHODS: We therefore evaluated histological specimens, 6 and 12 weeks after the sinus of minipigs was augmented with Bio-Oss((R)), a deproteinized bovine bone mineral, and Ostim((R)), an aqueous paste of synthetic nanoparticular hydroxyapatite. A curve was drawn that represents the changes in histomorph

Patient self-evaluation of intra-oral bone grafting treatment to the maxillary frontal region.

Bone grafting to the anterior region of the maxilla is a treatment method often used for reconstruction after dental trauma when teeth have been lost. The purpose of this study was to interview patients who underwent bone grafting from the chin or lateral mandible to assess the patients' experiences, especially regarding morbidity of the donor site and their overall assessment of treatment and care. Twenty-six patients who had undergone bone grafting from the chin and lateral mandible regions before implant treatment were interviewed about their experiences of treatment and present status 3-5

Distraction osteogenesis with subperiosteal devices in edentulous mandibles.

Nine patients with severely atrophic edentulous mandibles were treated by distraction osteogenesis with subperiosteal distractors for vertical augmentation of the anterior alveolar bone before insertion of implants. All the patients had severe complications and we conclude that the use of subperiosteal devices for vertical augmentation of edentulous mandibles is hazardous and offers no advantage over other surgical methods.

Hard and soft tissue augmentation in implant therapy using acellular dermal matrix.

The use of acellular dermal matrix to correct soft and hard tissue defects involving implants is described through three case reports. Correction of a ridge deformity caused by a root fracture, submerging of an existing implant and correction of recession defects around adjacent teeth, and ridge preservation for implant placement are presented. The use of acellular dermal matrix prevented the need for a second surgical site for donor material and the possible attendant postoperative complications. It also enhanced patient comfort and satisfaction with the procedure. All three cases demonstrate

Bone Ring Grafts Versus Non-Resorbable Membrane GBR for Vertical Ridge Augmentation: A Prospective Cohort Study.

PURPOSE: Vertical alveolar bone defects often require advanced augmentation techniques to enable successful implant placement. The aim of this study was to compare short-term vertical bone gain following two approaches: the bone-ring graft technique and guided bone regeneration (GBR) using a non-resorbable membrane in patients with vertical alveolar deficiencies. MATERIALS AND METHODS: A prospective cohort study was conducted involving 30 systemically healthy adult patients with posterior mandibular vertical bone defects measuring ≥4 mm. Participants were assigned to one of two treatment grou

Assessment of two treatment protocols using prebent titanium mesh and customized PEEK mesh for predominantly horizontal maxillary ridge augmentation with volumetric evaluation: a randomized clinical trial.

BACKGROUND: Reconstruction of the deficient maxillary ridge in three dimensions is crucial for the successful placement of implants. Prebent titanium meshes are an established modality for GBR, while patient-specific PEEK meshes have recently emerged with some possible advantages over the former. This study compared the effectiveness of two treatment protocols involving prebent titanium mesh and customized PEEK mesh, each combined with different graft compositions for bone augmentation. MATERIALS AND METHODS: 14 patients with 28 augmented sites in the maxillary ridges of primarily horizontal

Staged ridge splitting technique for horizontal expansion in mandible: a case report.

Ridge split procedures are commonly performed for horizontal augmentation of narrow ridges which would otherwise preclude implant placement. A 47-year-old patient with bilaterally edentulous posterior mandibular ridges was treated in the Department of Periodontology and Implantology, M.A. Rangoonwala College of Dental Sciences and Research Centre, employing a "staged ridge split" technique. In this technique, a simple corticotomy at the crestal, apical, and lateral aspects of the buccal cortical plate of the mandible was performed. Following this, a month later, the ridge was expanded and bone

The use of human allogenic graft (HBA) for maxillary bone regeneration: review of literature and case reports.

The use of graft materials is developed from the strong demand to support the complete bone regeneration of the empty socket and to increase the bone volume in treating the atrophies of sites already consolidated and with adverse alveolar bone conditions. A number of graft materials with different origin and mechanism of bone regeneration are available. Autologous graft materials, coming from the same patient, are defined as the gold-standard. The need of a second surgical site and the risk of morbidity and complications may make their use difficult. Human bone allografts (HBA) have been recen

Transcrestal sinus floor elevation with osteotomes: simplified technique and management of various scenarios.

In order to place implants subantrally when there is a reduced amount of bone, some form of bone augmentation is required. A transcrestal sinus floor elevation using osteotomes can increase bone levels and is a highly successful procedure. However, malleting osteotomes can be disconcerting for a patient. Therefore, a simplified technique is presented that minimizes the use of a mallet when executing a sinus floor elevation in medium dense bone. In addition, issues related to performing a transcrestal sinus floor elevation are addressed.

Management of a perplexing sinus lift complication.

BACKGROUND: This case report describes a dilemma that occurred after a maxillary right lateral window sinus lift was performed. The patient developed a gnawing pain in the sinus region but manifested no signs of sinusitis, and the subjacent teeth were asymptomatic. Differentiating between a pain that could be originating in the maxillary sinus or adjacent premolars was problematic. METHODS: After several weeks, a CT scan revealed no signs of sinusitis, despite the patient's belief that her discomfort was caused by her sinus lift. By the process of elimination, the decision was made to extract
