The cost and yield of photoscreening: impact of photoscreening on overall pediatric ophthalmic costs.

BACKGROUND: Approximately 5% of preschool-age children suffer from amblyopia. Many of them have high or unequal hyperopia. Amblyogenic risk factors frequently can be detected by photoscreening. METHODS: Free photoscreening was offered to Alaskan children ages 1 to 5 from urban and rural screening hubs. Screened images were mailed to the Alaska Blind Child Discovery coordinating center for physician photoscreen interpretation, specifically seeking latent or anisometropic hyperopia. Parents and screeners then were mailed results and information about amblyopia. Follow-up examination data were t…

Grating visual acuity with Teller cards compared with Snellen visual acuity in literate patients.

OBJECTIVE: To determine the usefulness of Teller Acuity Cards for detecting three levels of vision deficit, the cutoff for amblyopia (20/40 or poorer), vision impairment (20/70), or legal blindness (20/200). DESIGN: We compared grating visual acuity with the Teller cards with Snellen visual acuity (our gold standard) in 69 literate patients with amblyopia or other cause of vision loss in a prospective masked study. RESULTS: Teller card visual acuity and distance Snellen visual acuity correlated significantly (r = .508, P < .001); however, Teller card visual acuity explained only 26% of the v…

Pediatric Cataract: Disparately Blinding Globally and in the United States.

Childhood cataract is a complex condition requiring longitudinal care, including early diagnosis, timely referral to a specialist, early surgical intervention, and dedicated postoperative care. Adherence to refractive correction and amblyopia therapy are critical for visual rehabilitation, even months to years after the cataract is removed. We review the impact of the social determinants of health on each step in the visual rehabilitation pathway for children with congenital and infantile cataracts. Children from socioeconomically marginalized backgrounds are more likely to experience delays i…

Vision photoscreening of infants and young children in a primary care pediatric office: can it identify asymptomatic treatable amblyopic risk factors?

OBJECTIVE: The purpose of this study was to assess the feasibility and accuracy of vision photoscreening a large cohort of asymptomatic chil … mologist, 25 required intervention at their initial visit (26.6%). Ten children were diagnosed with amblyopia, representing 0.5% of all children screened. CONCLUSION: Vision photoscreening of 1976 young children identified 10 with previously undiagnosed amblyopia and an additional 15 with treatable pre-amblyopic risk factors. It is unlikely that any of the …

Environmental enrichment promotes plasticity and visual acuity recovery in adult monocular amblyopic rats.

Loss of visual acuity caused by abnormal visual experience during development (amblyopia) is an untreatable pathology in adults. In some occasions, amblyopic patients loose vision in their better eye owing to accidents or illnesses. While this condition is relevant both for its clinical importance and because it represents a case in which binocular interactions in the visual cortex are suppressed, it has scarcely been studied in animal models. We investigated whether exposure to environmental enrichment (EE) is effective in triggering recovery of vision in adult amblyopic rats rendered monocul…

[Some principles in surgical treatment of strabismus].

Taking into consideration the age of the patient, the strabismus surgery targets different things. Thus, for infants, the goal of the surgical intervention for congenital esotropia, is to prevent the occurrence of amblyopia and binocular vision dysfunctions (ARC, suppression). In preschool children, we operate aiming the recovery of the binocular vision, while in children over ten years old, the surgery is done only for esthetic reasons. On the other hand, in adults, the strabismus surgery has two aspects: for esthetic reasons in monocular strabismus with amblyopia, or for diplopia treatment i…

A comparative trial of Gobin's method versus conventional surgery for refractive/accommodative esotropia uncorrected by non-surgical methods.

INTRODUCTION AND PURPOSE: Gobin's theory of strabismus emphasizes the importance of diagnosis and correction of oblique muscle dysfunction w … vision, 81% versus 44% for the classical approach. They also had a reduced incidence of functional amblyopia after completion of treatment, 11% versus 39%. CONCLUSION: Gobin's theories and techniques deserve careful attention from all surgeons involved in treating infants with strabismus and amblyopia.

A longitudinal study of children with a family history of strabismus: factors determining the incidence of strabismus.

A longitudinal study of ocular refraction, position, and fixation was performed in children with a family history of strabismus. The children were examined at regular intervals between 3 months and 4 years of age, and the results are discussed in terms of changes in refraction between different ages and correlations between refraction and development of strabismus and amblyopia. Six of 34 children (17.6%) developed constant or intermittent esotropia. The strabismus was first noted between 18 and 30 months of age except in one case. All esotropic children were 4 dioptres hypermetropic or more a…

Abnormal visual experience during development alters the early stages of visual-tactile integration.

Visual experience during the critical periods in early postnatal life is necessary for the normal development of the visual system. Disruption of visual input during this period results in amblyopia, which is associated with reduced activation of the striate and extrastriate cortices. It is well known that visual input converges with other sensory signals and exerts a significant influence on cortical processing in multiple association areas. Recent work in healthy adults has also shown that task-relevant visual input can modulate neural excitability at very early stages of information process…

Surgical management of non-traumatic pediatric ectopia lentis: A case series and review of the literature.

PURPOSE: To report a small series of pediatric patients with ectopia lentis that underwent limbal-approach lensectomy and vitrectomy and scl … (p < 0.001). No complications were encountered in our series. A review of the literature found that amblyopia was the biggest vision-limiting factor. In general, the literature suggested that a higher percentage of eyes that were left aphakic achieved better vision than those implanted with a scleral-fixated intraocular lens. However, there may be selection …

Evaluating the perceptual and pathophysiological consequences of auditory deprivation in early postnatal life: a comparison of basic and clinical studies.

Decades of clinical and basic research in visual system development have shown that degraded or imbalanced visual inputs can induce a long-lasting visual impairment called amblyopia. In the auditory domain, it is well established that inducing a conductive hearing loss (CHL) in young laboratory animals is associated with a panoply of central auditory system irregularities, ranging from cellular morphology to behavior. Human auditory deprivation, in the form of otitis media (OM), is tremendously common in young children, yet the evidence linking a history of OM to long-lasting auditory processi…
