Burden of surgical congenital anomalies in Kenya: a population-based study.

BACKGROUND: Congenital malformations are a significant component of the global burden of disease among children, accounting for 25 million disability-adjusted life years (DALYs) worldwide. Unfortunately, efforts to estimate the burden of pediatric surgical disease in Africa are limited by the absence of population-based data. The objective of this study was to estimate both the prevalence and the disease burden of several common congenital surgical malformations among children in Kenya. METHODS: Community volunteers randomly surveyed households at sites across Kenya. Caretakers were asked to

VACTERL association with hydrocephalus in a fetus conceived by in vitro fertilization and embryo transfer.

OBJECTIVE: We present a case of VACTERL association with hydrocephalus (VACTERL-H) in a fetus conceived by in vitro fertilization (IVF) and embryo transfer (ET) and review the literature. CASE REPORT: A 35-year-old woman presented with multiple fetal anomalies at 22 weeks of gestation. She and her husband were non-consanguineous and there was no family history of congenital malformations. This was her second pregnancy conceived via IVF-ET. Two embryos had been implanted and only one survived. She underwent chorionic villus sampling at 17 weeks of gestation because of oligohydramnios and advan

Complete colonic duplication in children.

BACKGROUND: Complete colonic duplication is a very rare congenital anomaly that may have different presentations according to its location and size. Complete colonic duplication can occur in 15% of gastrointestinal duplication. We report two cases of complete colonic duplications, and their characteristics. CASE PRESENTATION: We present two patients with complete colonic duplication with different types and presentations. Case 1: A 2- year old boy presented to the clinic with abdominal protrusion, difficulty to defecate, chronic constipation and mucosal prolaps covered bulging (rectocele) sin

The genetic architecture of Down syndrome phenotypes revealed by high-resolution analysis of human segmental trisomies.

Down syndrome (DS), or trisomy 21, is a common disorder associated with several complex clinical phenotypes. Although several hypotheses have been put forward, it is unclear as to whether particular gene loci on chromosome 21 (HSA21) are sufficient to cause DS and its associated features. Here we present a high-resolution genetic map of DS phenotypes based on an analysis of 30 subjects carrying rare segmental trisomies of various regions of HSA21. By using state-of-the-art genomics technologies we mapped segmental trisomies at exon-level resolution and identified discrete regions of 1.8-16.3 M

The new Malone antegrade continence enema automatic instillation device allows independence and decreases flush times.

PURPOSE: The Malone antegrade continence enema procedure has changed the lives of many school-aged children who would otherwise be in diapers. Unfortunately, our current procedures to flush the Malone antegrade continence enema are difficult to accomplish independently and require an extensive time commitment. In this study we confirm that a new device, the ACE Malone automatic instillation device (ACE-MAID) is safe and effective, will decrease the time it takes to perform the flush and will increase independence. MATERIALS AND METHODS: Nine patients were prospectively enrolled for a 1-year s

Voiding cystourethrography in boys. Does the presence of the catheter during voiding alter the evaluation of the urethra?

PURPOSE: We determined whether the presence of the catheter during the voiding phase of voiding cystourethrography alters the evaluation of the urethra concerning the normal structures as well as pathological findings, especially posterior urethral valves. MATERIALS AND METHODS: A total of 123 males 3 days to 16 years old (median age 2.6 months) underwent voiding cystourethrography. Urethral catheterization was performed in all cases. Four views were taken during the voiding phase with and without the catheter in place. Only 80 patients had available results. These examinations were studied w

Bowel vaginoplasty in children: a retrospective review.

BACKGROUND/PURPOSE: There are numerous congenital genitourinary conditions that result in vaginal agenesis or atresia. Reconstruction presents a challenge to the surgeon who wishes long-term functional and cosmetic results with low morbidity. Historically, reconstruction has involved the use of skin grafts and nonoperative methods with less than ideal results. The preference of the authors is to use intestinal segments for creating the neovagina and herein review their experience with this procedure. METHODS: The authors identified 10 children who underwent vaginal construction with intestina

Gonadal mosaicism in severe Pallister-Hall syndrome.

Pallister-Hall syndrome (PHS, MIM #146510) is characterized by central and postaxial polydactyly, hypothalamic hamartoma (HH), bifid epiglottis, imperforate anus, renal abnormalities, and pulmonary segmentation anomalies. It is inherited in an autosomal dominant pattern. Here, we describe a family with two affected children manifesting severe PHS with mental retardation, behavioral problems, and intractable seizures. Both parents are healthy, with normal intelligence, and have no malformations on physical, laryngoscopic, and cranial MRI exam. The atypical presentation of these children and the

Toward evidence-based best practices in neonatal surgical care-I: The Canadian NICU Network.

BACKGROUND/PURPOSE: The Canadian Neonatal Network collects clinical data prospectively on every admission to each of its 17 tertiary level units in Canada. The purpose of this study was to interrogate the network database to determine case volumes, outcomes, and resource utilization for several neonatal surgical conditions. METHODS: The network database was used to measure a population-based, profile of incidence, outcome and resource utilization over a 22-month period for the following International Classification of Diseases, Ninth Revision (ICD-9) diagnostic codes: 741, spina bifida; 748.4

Malrotation of the intestines in children: the effect of age on presentation and therapy.

Because of the devastating consequences of midgut volvulus as a result of malrotation, we reviewed the charts of 70 consecutive children to define the spectrum of presentation. Although 27 patients (39%) had presenting symptoms within the first ten days of life, 35 (50%) were older than 2 months of age. In general, the older children had a longer course of vague, antecedent symptoms such as intermittent, nonbilious vomiting and chronic abdominal pain. Associated congenital anomalies were common, with 32 patients (46%) presenting with 56 anomalies, the most prevalent of which were intestinal at

Three additional cases of the congenital hypothalamic "hamartoblastoma" (Pallister-Hall) syndrome.

The cause of this syndrome remains unknown. Epidemiologic studies should be done by experts. The first two cases above gave a "muddy" history for possible teratogens--a "recall bias"--one of a vaginal foam spermicide, one of exposure over several days to a heavy residue of aviation diesel fuel exhaust. We do not think these are pertinent but they do point to the problem of getting a meaningful history. In addressing this problem (in a letter of 5/27/82) R.J. Berry, M.D., medical epidemiologist, Centers for Disease Control, Atlanta, Georgia, wrote "... a designed study providing standardized in

A ten-year experience with the pediatric Kock pouch.

In 1981, a five-year experience with the pediatric Kock pouch was reported. This series has now increased to 20 patients with an overall experience of 10 years. There were 12 females and eight males ranging in age from 13 to 19 years. These pouches were all elective: 18 were converted from standard ileostomies, (six with proctectomy or Hartmann procedure), and two had a total proctocolectomy. Three of these conversions were for failed Swenson, Soave, and Duhamel procedures. Sixteen children had ulcerative colitis, three Hirschsprung's disease (one with colon atresia and imperforate anus with r

Aphallia in a patient with 9q34 duplication syndrome: a case report.

INTRODUCTION: Congenital aphallia is a rare condition with less than 100 documented cases. It may occur isolated or in association with additional anomalies such as gastrointestinal malformations, caudal defects, imperforate anus, and other genitourinary anomalies. All previously reported patients with aphallia had a normal karyotype; here, we report the first case associated with a chromosomal imbalance. CASE PRESENTATION: A newborn was found to have an absence of the penis; a pre-sphincteric urethrorectal fistula was identified and a vesicostomy was performed. Hormonal studies at three mont

Nonfamilial VACTERL-H Syndrome in a Dizygotic Twin: Prenatal Ultrasound and Postnatal 3D CT Findings.

Background: VACTERL association is a widely known congenital malformation that includes vertebral, anal, cardiac, tracheoesophageal, renal, and limb anomalies. Patients with VACTERL and hydrocephalus appear to form a distinct group, both genetically and phenotypically, and their condition has been called VACTERL-H syndrome. Most cases of VACTERL-H have been reported postnatally, as VACTER-H syndrome is difficult to diagnose prenatally. Case Presentation: Here, we report a case of VACTERL-H syndrome in a dichorionic and diamniotic twin diagnosed prenatally by ultrasonography and confirmed postn

Müllerian anomaly in a woman with Hirschsprung's disease.

Müllerian anomalies are usually isolated or found in association with abnormalities of the renal tract. Rarely, they are encountered with coexisting Ggastrointestinal anomalies. Similarly, Hirschsprung's disease may coexist with additional somatic abnormalities, yet only a single case report has reported a müllerian anomaly coexisting with Hirschsprung's. A 35-year-old woman with long-standing dysmenorrhoea and infertility and history of Hirschsprung's disease and some components of VATER syndrome (vertebral anomalies, anal atresia, tracheoesophageal fistula, renal anomalies) underwent operati
