The authors present and comment on 18 cases of Giant-cell arteritis observed in an Internal Medicine Department between 1984 and 1991 with emphasis on clinical aspects and diagnostic considerations, on the occult presentation forms and, finally, on two cases with peripheral neuropathy. The clinical manifestations were: headache (78%), general non-specific symptoms (78%), polymyalgia rheumatica (61%), sudden blindness (33%), local temporal signs (28%), jaw pain (24%), articular complaints (17%) and peripheral neuropathy (12%).

A 10 year-old boy presented with an acute transitory right hemiparesis with aphasia. On CT-scan there was a left temporal low density. The cerebral angiography showed a total obstruction of the left terminal internal carotid with a "beading" of the posterior cerebral artery on the same side. Blood serologies and the presence of specific IgM in the cerebrospinal fluid gave evidence for recent rubella. An other arteriogram after a follow-up of 3 months showed a persistent obstruction of the internal carotid but a disappearance of the cerebral posterior artery abnormalities.

The perioperative nurse must understand the temporal arteritis disease process and surgical procedure so that he or she may function as an integral member of the surgical team. Preoperative, intraoperative, and postoperative nursing interventions are important and necessary to provide holistic patient care.

Two patients with clinical features consistent with giant cell arteritis were found to have systemic cholesterol microembolization syndromes. Diagnostic confirmation was established by a muscle biopsy in one patient and by a kidney biopsy in the other. Systemic cholesterol embolization can masquerade as a variety of disorders, including vasculitis, and should be considered in a patient with suspected giant cell arteritis who has a negative temporal artery biopsy.

Liver involvement in polymyalgia rheumatica/giant cell arteritis (PMR/GCA) before treatment and during follow-up of up to 3 1/2 years was assessed in 74 patients clinically, with liver function tests, isotope scans and blood flow studies. Twenty-seven patients had elevated alkaline phosphatase levels which fell to normal after 2.6 weeks treatment. Both PMR and GCA patients were affected, the latter more commonly. Isotope scans were abnormal in 7 of 29 patients and remained abnormal on follow-up. The arterial fraction of hepatic flow was significantly reduced in GCA patients in comparison with …

Paraffin sections and more than 7,000 plastic cross-sections of temporal arteries from 27 patients with a clinical diagnosis of polymyalgia rheumatica (PMR), 16 patients with a clinical diagnosis of temporal arteritis (TA) and 18 age- and sex-matched postmortem controls were studied using light microscopy. A new method was developed to permit a morphometric comparison between biopsies and autopsy specimens. Two stages of inflammation were discerned in TA. In atrophic arterial segments there was a focal, foreign-body, giant-cell reaction to the calcified internal elastic membrane (IEM) with a s…

Giant cell (temporal) arteritis G.C.(T) A. is common disorder and affects medium sized and large arteries in people over the age of fifty. Many series show that up to 50% of people with the clinical syndrome of polymyalgia rheumatica (PMR) go on to develop manifestations of G.C.(T) A. within a year; others may do so later. A critical review is offered of aspects of the subject which despite much study and research remain controversial or neglected. This includes epidemiology: statistics from routine autopsy suggests that the disease affects more people than are diagnosed clinically. The need t…

We have studied 40 patients with non-specific aorto-arteritis (Takayasu's disease) using intravenous digital subtraction angiography (DSA). The aorta, its major branches and (in 18 patients) pulmonary arteries were evaluated to determine the degree and extent of involvement. No complications related to the procedure were encountered. Good quality diagnostic images were obtained in 39 out of 43 instances. The unsuccessful examinations were in patients with congestive cardiac failure, and in one patient who would not co-operate. Aortic wall thickness and mild involvement of the descending aorta …

Neurologic findings were studied in 166 consecutive patients with biopsy-proven giant cell (temporal) arteritis. Neurologic problems occurred in 51 patients (31%): neuropathies (23), TIA/strokes (12), neuro-otologic syndromes (11), tremor (6), neuropsychiatric syndromes (5), tongue numbness (3), and myelopathy (1). Neuro-ophthalmologic problems occurred in 35 patients (21%): amaurosis fugax (AF) (17), permanent vision loss (PVL) (14), scintillating scotoma (8), and diplopia (3). Abnormalities in large arteries in 52 patients (31%) included bruits and diminished pulses. The carotid artery was i…

Cutaneous involvement in giant cell arteritis is quite uncommon. A patient is described who presented with pretibial skin lesions clinically indistinguishable from erythema nodosum which, on biopsy, showed subcutaneous pannicular giant cell vasculitis. Cutaneous manifestations of giant cell arteritis are subsequently reviewed.

Thirty-five Caucasian patients with giant cell (temporal) arteritis were typed for HLA class I and II antigens. A significant increase was found for A31, B40, Cw3, and DR4. HLA-Cw3 was the most frequent antigen observed (57%) and had the highest relative risk (5.65), suggesting that Cw3 may be the primary HLA risk factor for this disease. The increased occurrence of A31, B40, and DR4 is probably secondary to their association with Cw3.

Six biopsy proven cases of giant-cell arteritis in women beyond the age of 70 years are presented to illustrate the wide range of clinical manifestations of this disease. Besides polymyalgia rheumatica or cranial arteritis the illness may afflict various organs including eye, ear, skin, heart, cerebrum and liver or may mimic a chronic infectious or malignant disease. Diagnosis should be considered in any such cases, especially when laboratory data suggest a chronic infectious disease due to high sedimentation rate, anemia with low serum iron and normal total iron binding capacity and an increa…

Giant cell arteritis is a disease of the elderly, affecting the temporal or other cranial arteries, often in patients with polymyalgia rheumatica. A case of giant cell arteritis with necrosis of the tongue in a 77-year-old woman is reported. It is important to be aware of this condition because of the serious complications. As the lesion is focally distributed, it is necessary to obtain a large biopsy specimen from the superficial temporal artery in order to make a correct diagnosis. Prompt treatment with corticosteroid in large doses followed by a lower maintenance dose is important to preven…
