Journey of B Cell Dysregulation in Rheumatoid Arthritis: Insights from Immunogenomics.

BACKGROUND: Rheumatoid arthritis (RA) is a chronic autoimmune manifestation characterized by persistent chronic inflammation and joint damage. B cells play pivotal roles in the immune system, recognize antigens, and facilitate subsequent immune responses. Recent advancements in single-cell resolution techniques and immunogenomics have provided unprecedented insights into deciphering the complex mechanisms underlying B-cell dysregulation in RA. METHODS AND RESULTS: This review article aims to articulate the current knowledge on how aberrant B cell activation and BCR signalling contribute to th

TRP channels: Role in neurodegenerative diseases and therapeutic targets.

TRP (Transient receptor potential) channels are integral membrane proteins consisting of a superfamily of cation channels that allow permeability of both monovalent and divalent cations. TRP channels are subdivided into six subfamilies: TRPC, TRPV, TRPM, TRPP, TRPML, and TRPA, and are expressed in almost every cell and tissue. TRPs play an instrumental role in the regulation of various physiological processes. TRP channels are extensively represented in brain tissues and are present in both prokaryotes and eukaryotes, exhibiting responses to several mechanisms, including physical, chemical, an

CNN-Based Identification of Parkinson's Disease from Continuous Speech in Noisy Environments.

Parkinson's disease is a progressive neurodegenerative disorder caused by dopaminergic neuron degeneration. Parkinsonian speech impairment is one of the earliest presentations of the disease and, along with tremor, is suitable for pre-diagnosis. It is defined by hypokinetic dysarthria and accounts for respiratory, phonatory, articulatory, and prosodic manifestations. The topic of this article targets artificial-intelligence-based identification of Parkinson's disease from continuous speech recorded in a noisy environment. The novelty of this work is twofold. First, the proposed assessment work

Speech-language evaluation and rehabilitation treatment in Floating-Harbor syndrome: a case study.

UNLABELLED: Floating-Harbor syndrome is a rare congenital disorder characterized by specific facial features, short stature associated with significantly delayed bone age and language impairment. Although language delay is a cardinal manifestation of this syndrome, few reports describe the specific language difficulties of these patients, particularly the development of language abilities in the long run. This paper reports on an Italian boy with Floating-Harbor syndrome and discusses his language evaluation at presentation (age 48 months) and development and progress of his language abilities

Atlantoaxial lateral mass osteoarthritis. A frequently overlooked cause of severe occipitocervical pain.

Localized C1-C2 lateral mass osteoarthritis is a degenerative disorder of the upper cervical spine that has a natural history markedly different from that of degenerative afflictions of the lower cervical spine. Atlantoaxial lateral mass arthritis is a distinct cause of occasionally severe occipitocervical pain in elderly persons. In this series, the diagnosis was suggested by the medical history of nine elderly patients who presented with severe occipitocervical pain (frequently diagnosed as occipital neuralgia). Physical examination demonstrated marked restriction of rotation of the cervical

First person account: maintaining mental health in a turbulent world.

The article and the poems that follow are part of the Schizophrenia Bulletin's ongoing First Person Accounts series. We hope that mental health professionals--the Bulletin's primary audience--will take this opportunity to learn about the issues and difficulties confronted by consumers of mental health care. In addition, we hope that these accounts will give patients and families a better sense of not being alone in confronting the problems that can be anticipated by persons with serious emotional difficulties. Although there are a number of books and articles describing the personal experience

Treating cocaine and opioid use disorder with transcranial magnetic stimulation: A path forward.

Developing new, effective treatments for substance use disorders (SUDs), especially cocaine and opioid use disorders (CUD and OUD), are of immense importance. These are chronic, relapsing brain diseases characterized by dysregulated circuits manifesting from neuroplastic change brought on by repeated exposure to substances of abuse. A potential treatment is therapeutically inducing neuroplastic change in targeted dysregulated circuits. One such intervention, repetitive transcranial magnetic stimulation (rTMS) has gained traction over the past two decades as a method of noninvasively stimulatin

Speech-based concussion detection in athletes using Mel-spectrograms and convolutional neural networks.

Neurological disorders can alter motor control, cognition, and speech production in subtle ways that are difficult to detect using standard diagnostic tools. Because conventional imaging techniques such as CT and MRI often fail to reveal functional abnormalities, there is growing interest in non-invasive digital health approaches. This study investigates the potential of speech-based deep learning for detecting concussion-related dysfunction using structured speech tasks. Audio recordings from 225 high school and college athletes (approximately 200 retained after quality filtering) performing

Mental well-being, emotional-cognitive processing, and attention skills in individuals with cluttering.

BACKGROUND: Cluttering is a fluency disorder marked by a rapid and irregular speech rate and articulation difficulties such as "slurred" speech and sound omissions. These speech disruptions reduce intelligibility and negatively impact communication. Treatment for individuals with cluttering typically focuses on improving their speech clarity and fluency. In contrast, while addressing speech disruptions, treatment for stuttering, a related fluency disorder, extends to include psychological and emotional aspects. This broader approach is based on research linking stuttering to mental health indi

Vocal Behavior of Teachers Reading with Raised Voice in a Noisy Environment.

(1) Objective: Teaching is a particularly voice-demanding occupation. Voice training provided during teachers' education is often insufficient and thus teachers are at risk of developing voice disorders. Vocal demands during teaching are not only characterized by speaking for long durations but also by speaking in noisy environments. This provokes the so-called Lombard effect, which intuitively leads to an increase in voice intensity, pitch and phonation time in laboratory studies. However, this effect has not been thoroughly investigated in realistic teaching scenarios. (2) Methods: This stud

The contribution(s) of the insula to speech production: a review of the clinical and functional imaging literature.

Skilled spoken language production requires fast and accurate coordination of up to 100 muscles. A long-standing concept--tracing ultimately back to Paul Broca--assumes posterior parts of the inferior frontal gyrus to support the orchestration of the respective movement sequences prior to innervation of the vocal tract. At variance with this tradition, the insula has more recently been declared the relevant "region for coordinating speech articulation", based upon clinico-neuroradiological correlation studies. However, these findings have been criticized on methodological grounds. A survey of

Cross-Hemispheric Spatial-Temporal Attention Network for Decoding Silent Speech From EEG.

OBJECTIVE: Speech, as the core of advanced human cognition, is fundamental to social interaction and daily life. Electroencephalogram (EEG)-based speech brain-computer interface (BCI) offers a novel communication pathway for patients with speech disorders, where deep learning has demonstrated significant advantages. Given the established dominance of the left hemisphere in speech processing, exploring methods to extract speech-related neural features fully is crucial for enhancing decoding performance. APPROACH: In this study, EEG signals were recorded during a silent speech task involving th

Singing-Based Interventions for Treating Hypokinetic Dysarthria in Parkinson's Disease: A Systematic Review.

Parkinson's disease (PD) progressively affects motor functions, often resulting in hypokinetic dysarthria, characterized by reduced speech articulation and vocal intensity. Over the last few decades, singing-based interventions have demonstrated potential therapeutic benefits for treating neurological speech disorders. To review the effectiveness of singing-based interventions in treating hypokinetic dysarthria, articles were identified based on a search across MEDLINE, PubMed, Scopus, ScienceDirect, and Web of Science. After being independently screened by the authors, the information was rev

Speaker-independent dysarthria severity classification using self-supervised transformers and multi-task learning.

Dysarthria, characterised by slurred speech, is a hallmark of many neurological disorders and brain trauma. Clinical assessment requires an audio-visual investigation by a trained healthcare expert, who evaluates criteria such as respiration, phonation, articulation, resonance, and prosody during speech. Quantitative assessment of dysarthria is challenging due to its complexity, variability, and the subjective nature of human-observation-based scoring methods. We present a novel machine-learning framework using transformers for stratifying and monitoring patient speech. Our framework integrate

Acoustic Vowel Metrics as Correlates of Dysphagia and Dysarthria in Brainstem Neurodegenerative Diseases.

Background/Objectives: Swallowing and speech rely on shared brainstem circuits coordinating oropharyngeal motor functions. In neurodegenerative diseases affecting the brainstem-such as progressive supranuclear palsy (PSP), amyotrophic lateral sclerosis (ALS), and multiple system atrophy (MSA)-bulbar dysfunction often impairs tongue propulsion and motility, affecting both swallowing (dysphagia) and phonation (dysarthria). This study aimed to investigate whether vowel-based acoustic features are associated with swallowing severity in brainstem-related disorders and to explore their potential as

Impaired motor-to-sensory transformation mediates auditory hallucinations.

Distinguishing reality from hallucinations requires efficient monitoring of agency. It has been hypothesized that a copy of motor signals, termed efference copy (EC) or corollary discharge (CD), suppresses sensory responses to yield a sense of agency; impairment of the inhibitory function leads to hallucinations. However, how can the sole absence of inhibition yield positive symptoms of hallucinations? We hypothesize that selective impairments in functionally distinct signals of CD and EC during motor-to-sensory transformation cause the positive symptoms of hallucinations. In an electroencepha
