Heart rate variability as a non-invasive biomarker of autonomic dysfunction in amyotrophic lateral sclerosis: A systematic review and meta-analysis.

OBJECTIVE: This study assessed heart rate variability (HRV) alterations in amyotrophic lateral sclerosis (ALS) patients compared to healthy control groups using both frequency-domain and time-domain HRV parameters. METHODS: A systematic review and meta-analysis were conducted using studies retrieved from PubMed, Embase, Web of Science, and Cochrane Library databases up to November 13, 2024. Fourteen studies were included in the qualitative synthesis and eight in the quantitative analysis. RESULTS: ALS patients exhibited significantly reduced Low Frequency (LF) and High Frequency (HF) HRV par

Autonomic dysfunction in patients with lower urinary tract disorders.

The autonomic nervous system (ANS) plays a key role in the control of micturition, ensuring an alternation between storage and voiding phases. Any lesion in the neural pathways may be responsible for lower urinary tract (LUT) dysfunction, usually associated with bowel and sexual disorders. In individuals with isolated LUT symptoms, dysregulation of the ANS may play a role in the origin of these disorders. This review summarizes the current evidence regarding the association between LUT disorders and ANS dysfunction, explored through a panel of autonomic tests (autonomic cardiovascular testing,…

Assessment of cardiac autonomic function: from bench to bedside.

PURPOSE OF REVIEW: Cardiac autonomic dysfunction plays a crucial role in the pathogenesis of common cardiovascular diseases. The review discusses current and emerging techniques in the evaluation of cardiac autonomic tone. RECENT FINDINGS: Assessment techniques include invasive nerve recordings, neurochemical staining, serum biomarkers, advanced imaging, and autonomic reflex testing. Noninvasive modalities such as heart rate variability and skin sympathetic nerve activity have improved clinical feasibility. Novel radiotracers and serum-based assays are emerging to map neurotransmitter dynamic

Autonomic dysfunction in polycystic ovary syndrome.

Polycystic ovary syndrome (PCOS) is the most common endocrinopathy affecting premenopausal females. Alongside the endocrine, reproductive and psychological consequences of the condition, PCOS has now been linked to increased risk of cardiovascular and metabolic disease e.g., insulin resistance. The mechanisms behind this excess risk are not fully understood and multiple characteristics of PCOS (e.g., hyperandrogenism, obesity and vascular dysfunction) likely contribute to individual risk. Autonomic dysfunction may also drive cardiovascular risk in PCOS, via its effects on both blood pressure c

Maternal autonomic responsiveness is attenuated in healthy pregnancy: a phase rectified signal averaging analysis.

Autonomic regulation is essential in enabling a healthy pregnancy. In fact, several pregnancy complications are associated with autonomic dysfunction. Better understanding of the maternal autonomic state during healthy pregnancy may aid in the early detection of such complications. One aspect of autonomic regulation is autonomic responsiveness, which can by assessed by phase rectified signal averaging (PRSA). While other areas of research have found blunted physiological responses in pregnancy, this paper presents the first investigation of maternal autonomic responsiveness as assessed by PRSA

Cardiac autonomic neuropathy in fibromyalgia: Revisited.

BACKGROUND: Fibromyalgia (FM) is associated with widespread autonomic dysfunction where sympathetic predominance explains associated complaints such as widespread pain, sleep disorders and anxiety. Recent studies indicate a possible neurovascular autonomic interaction in the pathogenesis of FM. OBJECTIVE: Our study paradigm included a modified Ewing's battery of autonomic function tests to find the cardiac autonomic neuropathy (CAN) in FM patients. The battery comprises some tests such as the Valsalva maneuver, which are effort-dependent, so we also aimed to identify a potential simplified te

Clinical Assessment of Autonomic Function in Fibromyalgia by the Refined and Abbreviated Composite Autonomic Symptom Score (COMPASS 31): A Case-Controlled Study.

BACKGROUND: It has been shown that autonomic dysfunction in fibromyalgia can be assessed by the Composite Autonomic Symptom Score (COMPASS) questionnaire. More recently, a refined and much abbreviated 31-item version of the questionnaire has been developed, the COMPASS 31. OBJECTIVES: First, to determine whether the COMPASS 31 can assess changes in autonomic function in fibromyalgia. Second, to assess whether the COMPASS 31 values in fibromyalgia patients are positively correlated with scores on the Revised Fibromyalgia Impact Questionnaire (FIQR). METHODS: A cross-sectional, case-controlled

[Efficiency of neuromultivit in complex therapy of patients with rheumatoid arthritis associated with diabetes mellitus].

AIM: To examine the dysregulation of the autonomic nervous system and manifestations of pain syndrome in patients with rheumatoid arthritis combined with diabetes mellitus and the possibility of their correction in patients who receive complex therapy including neurotropic vitamins (neuromultivitis). MATERIAL AND METHODS: One hundred and forty-six patients with rheumatoid disease were examined including 59 patients with diabetes mellitus. RESULTS AND CONCLUSION: When testing for the presence of neuropathic pain syndrome using the diagnostic questionnaire of neuropathic pain (DN4), a greater …

Proper performance of autonomic function testing.

INTRODUCTION: The aim of this position statement is to provide a recommendation from experts at the American Association of Neuromuscular & Electrodiagnostic Medicine (AANEM) on the proper performance of autonomic testing. METHODS: The AANEM Professional Practice Committee and selected AANEM members reached a consensus based on expert opinion on the training required to perform autonomic testing and the appropriate performance of such tests. RESULTS: Individuals without sufficient medical education in autonomic disorders are unqualified to interpret the data generated or to coordinate the fi

Heart rate variability--current diagnosis of the cardiac autonomic neuropathy. A review.

BACKROUND: The autonomic nervous system is one of the major homeostatic regulatory systems of the body. Detection of cardiac autonomic neuropathy (CAN), reflected by reduced heart rate variability (HRV), is an independent adverse prognostic factor: sympathovagal balance failure with a sympathetic dominance is the main trigger for lethal arrhythmias and sudden death. METHODS AND RESULTS: PubMed database based on original articles from 1983 to 2013 and the author´s clinical experience. This review covers the current status of the methodology and the clinical uses of HRV, especially in the field…

[Key-Gaskell syndrome in a cat in Switzerland].

As far as the authors know this is the first case of Key-Gaskell-syndrome being described in Switzerland. The clinical signs of megaoesophagus, anorexia, constipation, dryness of all mucous membranes, reduced tear production, protrusion of the membrana nictitans, mydriasis, regurgitation and bradycardia are pathognomonic and can't be mistaken by any other disease. The subject of the Key-Gaskell-syndrome is a dysfunction of the autonomic nervous system. Histopathological changes are exclusively related to the autonomic nervous system and to neurons of some nuclei in the cranial nerves. Less sev…
