Evaluation of a novel osteosynthesis plate system for mandibular defects.

Continuity defects of the jaw can be reconstructed with titanium plates or microvascular bone flaps; osteosynthesis plates are necessary for both. In this study we performed a retrospective review of patients treated with Medartis MODUS® Mandible Trauma/Reco 2.0-2.5, TriLock bridging plates, mandibulectomy and soft tissue free flap or reconstruction with a bony free flap and TriLock mandibular plates from the same system from January 2015 to August 2019. The variables recorded were sex, age, diagnosis, radiotherapy, date of implantation, date of explantation or death of patient, size of mandib

Exploring the micromorphological characteristics of adult lower cervical vertebrae based on micro-computed tomography.

We will use micro-computed tomography to scan 31 sets of the adult lower cervical vertebrae (155 vertebrae) to observe the morphological characteristics and direction of trabeculae in the lower cervical vertebrae by outlining and reconstructing the regions of interest and to calculate the variation laws of the microstructure in the regions of interest to reveal their structural characteristics and weak areas. As a result, the images showed that the trabeculae in the lower cervical pedicle near the medial and lateral cortices were relatively dense, and their bone plates were lamellar. There wer…

Three dimensional structure of the distal condyles of the third metacarpal bone of the horse.

This study examined the three-dimensional (3D) microarchitecture of regions of the equine third metacarpal bone (McIII) commonly involved in distal condylar fractures. Limbs were obtained from Thoroughbred horses (neonates to age 24 years) destroyed for inoperable fractures and a variety of other conditions. Beams, blocks and sections were cut in the principal axes, some embedded in PMMA and others examined unembedded. Several methods were used to study the 3D structure, including conventional and confocal optical microscopy, scanning electron microscopy (SEM) and radiography. The mineralised

Spinal fixation. Part 1. Principles, basic hardware, and fixation techniques for the cervical spine.

Spinal fixation devices provide stability and restore anatomic alignment in the treatment of fractures, degenerative disease, infection, and tumors and correct congenital deformities such as those seen in scoliosis. The devices provide immediate stability but are not strong enough to withstand prolonged stress and eventually fail, in most cases, if bone fusion does not occur. Bone graft material is often used to promote fusion and to replace bone after resection. Internal fixation is used to maintain position and alignment and to prevent motion as the spine fuses. Plates and rods are attached

[Treatment of cervical spine injuries by an anterior stable spondylodesis with an H-plate (author's transl)].

A stable spondylodesis of the cervical spine was obtained by an osteosynthesis with metal plates. The H-plate of the A-O in connection with autologous bone-chips from the iliac crest or Kieler bone and bone cement was used in five cases and resulted in a solid fusion of the affected cervical segments. The development of this technique and the advantages over other methods are demonstrated. The results of our cases reflect the superiority of this procedure.

The novel Kambin Torpedo full-endoscopic lumbar interbody fusion technique: a case series.

PURPOSE: Full-endoscopic lumbar interbody fusion (FELIF) is a new-generation treatment for spondylolisthesis. However, owing to their unique characteristics, the two main endoscopic fusion trajectories, the trans-Kambin and posterolateral approaches, have important limitations. Herein, we aimed to introduce a new technique called Kambin Torpedo FELIF (KT-FELIF). METHODS: The KT-FELIF technique is based on the trans-Kambin approach. It additionally completes ipsilateral total facetectomy and contralateral direct decompression. Thus, this novel technique combines the advantages of the trans-Kam

Transcriptional, epigenetic and microRNA regulation of growth plate.

Endochondral ossification is a critical event in bone formation, particularly in long shaft bones. Many cellular differentiation processes work in concert to facilitate the generation of cartilage primordium to formation of trabecular structures, all of which occur within the growth plate. Previous studies have revealed that the growth plate is tightly regulated by various transcription factors, epigenetic systems, and microRNAs. Hence, understanding these mechanisms that regulate the growth plate is crucial to furthering the current understanding on skeletal diseases, and in formulating effec

The role of the PTHrP/Ihh feedback loop in the unusual growth plate location in mammalian metatarsals and pisiforms.

BACKGROUND: Longitudinal skeletal growth takes place in the cartilaginous growth plates. While growth plates are found at either end of conventional long bones, they occur at a variety of locations in the mammalian skeleton. For example, the metacarpals and metatarsals (MT) in the hands and feet form only a single growth plate at one end, and the pisiform in the wrist is the only carpal bone to contain a growth plate. We take advantage of this natural anatomical variation to test which components of the PTHrP/Ihh feedback loop, a fundamental regulator of chondrocyte differentiation, are specif

Case report: Corpectomy and iliac crest bone autograft as treatment for a vertebral plasma cell tumor in a dachshund dog.

A 7-year-old, 7.5 kg, female spayed dachshund dog presented to a veterinary teaching hospital after being referred for lameness and the inability to urinate. The dog presented as weakly ambulatory paraparetic with intact pain perception. Computed tomographic (CT) imaging showed ventral bony lysis and periosteal proliferation of the L3 vertebra, consistent with a plasma cell tumor. A corpectomy of the L3 vertebra was performed and subsequently stabilized with autogenous cortico-cancellous iliac crest bone staves, autogenous cancellous bone, and bilaterally placed locking plates [String of Pearl

Feasibility of Implant Strain Measurement for Assessing Mandible Bone Regeneration.

Nonunion is one of the most dreaded complications after operative treatment of mandible fractures or after mandible reconstruction using vascularized and non-vascularized bone grafts. Often diagnosis is made at advanced stage of disease when pain or complications occur. Devices that monitor fracture healing and bone regeneration continuously are therefore urgently needed in the craniomaxillofacial area. One promising approach is the strain measurement of plates. An advanced prototype of an implantable strain measurement device was tested after fixation to a locking mandible reconstruction plat

Clinical Outcomes of Surgical Treatment for Paediatric Forearm Fractures: A Retrospective Analysis of 86 Fractures.

Background: Surgical treatment of paediatric forearm fractures is usually based on age and fracture type. Most patients undergo intramedullary and, occasionally, conventional pinning or plate fixation. However, the optimal fixation method remains controversial. We retrospectively examined the clinical and radiographic outcomes and complications associated with pinning (intramedullary and conventional) and plate fixation. Methods: Surgery was performed for 86 diaphyseal fractures of the forearm in 60 patients aged ≤ 15 years. Their mean age and follow-up duration were 8.7 years and 10.4 months,

Dermatology (Beau's Lines) and Forensic Pathology (Harris Lines and Linear Enamel Hypoplasia) Signs of Growth Arrest: A Case Report and Literature Review.

Dermatology encompasses the evaluation and management of localized conditions that affect the nail plate, including trauma, primary dermatoses involving the nail bed and nail matrix, and systemic disorders with manifestations that impact the nail unit. In addition to traumatic events that can alter the nail unit, localized conditions such as periungual dermatitis and infections, as well as generalized diseases, can affect the nail matrix, resulting in temporary growth arrest of the nail plate. This can present as a groove (Beau's line) in the nail plate that progressively grows out as the nail

Biomechanical design optimization of proximal humerus locked plates: A review.

BACKGROUND: Proximal humerus locked plates (PHLPs) are widely used for fracture surgery. Yet, non-union, malunion, infection, avascular necrosis, screw cut-out (i.e., perforation), fixation failure, and re-operation occur. Most biomechanical investigators compare a specific PHLP configuration to other implants like non-locked plates, nails, wires, and arthroplasties. However, it is unknown whether the PHLP configuration is biomechanically optimal according to some well-known biomechanical criteria. Therefore, this is the first review of the systematic optimization of plate and/or screw design

Flexure-Based Locking Plates Can Modulate Interfragmentary Motion in Distal Femur and Diaphyseal Fractures: A Parametric Finite Element Analysis.

Axial interfragmentary motion is known to stimulate fracture healing. A mechanically compliant fracture fixation plate incorporating flexures is proposed to provide controlled axial micromotion to long bone fractures. To explore the concept's feasibility, computational modeling of general diaphyseal and distal femur fractures treated with both rigid and compliant plates is conducted. In Part I of this study, a diaphyseal fracture finite element model for novel compliant plates is validated against experimental data with good agreement. In Part II, a parametric analysis is conducted using the v

[Biomechanical study on wing shaped titanium plate fixation of acetabular anterior column and posterior hemi-transverse fracture under multiple working conditions].

This article aims to compare and analyze the biomechanical differences between wing-shaped titanium plates and traditional titanium plates in fixing acetabular anterior column and posterior hemi-transverse (ACPHT) fracture under multiple working conditions using the finite element method. Firstly, four sets of internal fixation models for acetabular ACPHT fractures were established, and the hip joint stress under standing, sitting, forward extension, and abduction conditions was calculated through analysis software. Then, the stress of screws and titanium plates, as well as the stress and disp

[Recent advances in treatment of aseptic femoral shaft nonunion].

OBJECTIVE: To review the recent advances in treatment of aseptic femoral shaft nonunion. METHODS: The clinical studies about the treatments of aseptic femoral shaft nonunion in recent years were widely reviewed and analyzed. RESULTS: There are several surgical methods for aseptic femoral shaft nonunion. Due to uncertain clinical outcome, dynamization of nail should be carefully selected. The exchange nailing is suitable for the hypertrophic nonunion of the isthmal femoral shaft fracture. The exchange lateral plating is suitable for nonunion with obvious malformation. However, wave plate or d

Effects of plate contouring quality on the biomechanical performance of high tibial osteotomy fixation: A parametric finite element study.

Locking plates have threaded holes, in which threaded-head screws are affixed. Hence, they do not need to be in intimate contact with underlying bone to provide fixation. There are, however, reports that a large distance between the plate and the bone might cause clinical complications such as delayed union or nonunion, screw pull out, and screw and plate breakage. Considering the diversity in the capabilities and costs of different plate customization techniques, the purpose of this study was to investigate the effect of the plate contouring quality on the biomechanical performance of high ti

A novel method for lateral callus distraction and its importance for the mechano-biology of bone formation.

We introduce a novel method of lateral callus distraction for bone formation, which avoids the conventional splitting and weakening of bones. At the medial aspect of the sheep tibia the periosteum was resected and small holes were drilled into the cortex to connect the bone surface with the marrow. A distraction device with a hydroxyapatite-coated titanium plate was fixed over the drilled area. After 10 days latency the plate was distracted perpendicular to the bone's long axis twice a day by 0.27 mm for 10 days. The newly formed tissue was then allowed 50 days of maturation. In a control grou

The effects of mechanical loading on the mRNA expression of growth-plate cells.

Bone growth is a complex process involving proliferation, maturation and hypertrophy of chondrocytes in the growth plates. Mechanical forces applied to growing bones alter their longitudinal growth. However, the mechanisms by which chondrocytes modulate longitudinal bone growth are not well understood. This in vitro study investigated the effects of mechanical loading on the mRNA expression pattern of key molecular components of the growth-plate. Short-term static loading was applied to rat proximal tibial growth-plate explants. Various age groups at specific developmental stages were investig
