Mounier-Kuhn syndrome (MKS) is a rare disease of unknown etiology characterized by abnormal pathologic dilatation of tracheabronchial tree. The diagnosis of MKS is normally made on CT scan of chest on the basis of enlarged diameters of trachea and main stem bronchi. We are presenting histologically confirmed case of MKS, where the diameter of right main bronchus is below minimum diameter (mean+3 SD) required for the diagnosis. Contralateral tension pneumothorax during 1-lung ventilation is rare but life threatening. We report the case of a patient who developed tension pneumothorax of the dependent lung during 1-lung ventilation while the surgeon was anastomosing the bronchi after sleeve lobectomy. Ventilation was not possible in either the dependent or nondependent lung, leading to severe desaturation and cardiac arrest. Upper respiratory tract involvement occurs in 1 to 6% of all patients with sarcoidosis. Dry cough, hoarseness, dyspnea and dysphagia are the main symptoms. The diagnosis is established by biopsy demonstrating granulomatous lesions. We report the case of a 22-year-old woman who presented with erythema nodosum, arthralgias and granulomatous lesions of the trachea, larynx and bronchi. One hundred and sixty-one patients with pulmonary metastases were studied with CT. Six of them proved to have endobronchial (intraluminal) metastatic lesions by bronchoscopy. Five cases of actinomycosis of the main bronchi or trachea which were suggestive clinically of bronchogenic carcinoma are described. In four patients the correct diagnosis was made by a bronchial biopsy or wash, or both. Three of them recovered following antibiotic treatment, and one died a few days after bronchoscopy. An original method of treatment for main bronchus fistula occurring after pneumonectomy was described in 1961. The operation is performed via a median sternotomy and the carinal region exposed. The surgical management of tumors of the left main bronchus with involvement of the lower trachea is one of the most difficult problems of tracheobronchial surgery. Two men developed renal adenocarcinoma in association with situs ambiguous off with polysplenia (SAP) (also known as the polysplenia syndrome). Aplasia of the trachea associated with multiple congenital anomalies is described in a stillborn male foetus with single umbilical artery.
