Plasmapheresis for recurrent posttransplant focal segmental glomerulosclerosis.

The high recurrence rate of focal segmental glomerulosclerosis (FSGS) in kidney transplant recipients suggests that such patients have a circulating factor that alters glomerular capillary permeability. Serum from patients with FSGS increases glomerular permeability to albumin. This permeability factor has been partially identified as a protein. The removal of this protein by plasmapheresis (PP) decreases proteinuria. In this study we report data on the therapeutic effects of PP in FSGS children with recurrence in the transplanted kidney. Three hundred pediatric (age <19 years) renal transplan…

[Circulatory and respiratory problems in preeclampsia].

The hemodynamic and cardiovascular changes seen during PE vary according to the natural history of the disease, its severity and eventual th … l pregnancy. The cardiac function is however usually preserved during PE. PE induces an exaggerated capillary permeability. This results in the worsening of the airway edema which may render the intubation very difficult. The increased capillary permeability contributes, among other factors, to the heightened risk of acute pulmonary edema. It is not justified to adminis …

Histological efficacy of high-dose intravenous vitamin C infusion in severely burned patients.

Severe burn injuries cause increased endothelial permeability leading to systemic capillary leak and massive fluid shifts into the extravascular space. Excessive fluid resuscitation to ensure sufficient end-organ perfusion results in critical tissue oedema. The present study assessed the underlying microscopic data regarding the integrity of thin skin dermal capillary endothelial barrier and angiogenesis after high-dose vitamin C (HDVC) administration in severely burned patients at days 1, 3 and 7 postburn. Twenty severely burned patients [20% to 25% total body surface area of second deep derm

Spinster homolog 2 (spns2) deficiency causes early onset progressive hearing loss.

Spinster homolog 2 (Spns2) acts as a Sphingosine-1-phosphate (S1P) transporter in zebrafish and mice, regulating heart development and lymphocyte trafficking respectively. S1P is a biologically active lysophospholipid with multiple roles in signalling. The mechanism of action of Spns2 is still elusive in mammals. Here, we report that Spns2-deficient mice rapidly lost auditory sensitivity and endocochlear potential (EP) from 2 to 3 weeks old. We found progressive degeneration of sensory hair cells in the organ of Corti, but the earliest defect was a decline in the EP, suggesting that dysfunctio

Endothelial colony-forming cells reduced the lung injury induced by cardiopulmonary bypass in rats.

BACKGROUND: Cardiopulmonary bypass (CPB) results in severe lung injury via inflammation and endothelial injury. The aim of this study was to evaluate the effect of endothelial colony-forming cells (ECFCs) on lung injury in rats subjected to CPB. METHODS: Thirty-two rats were randomized into the sham, CPB, CPB/ECFC and CPB/ECFC/L-NIO groups. The rats in the sham group received anaesthesia, and the rats in the other groups received CPB. The rats also received PBS, ECFCs and L-NIO-pre-treated ECFCs. After 24 h of CPB, pulmonary capillary permeability, including the PaO2/FiO2 ratio, protein level…

Murine lupus nephritis. A structure-function study.

This report examines the correlation between glomerular injury and glomerular dysfunction in murine lupus nephritis. Glomerular filtration rate was measured by the clearance of inulin in conscious NZB/W female mice and shown to vary 12-fold in the animals tested. Detailed morphometric measurements were made on the perfused fixed kidneys. As disease progressed the surface density (Sv) of the open capillary loops decreased by 73%. This drop in Sv correlated with a 4-fold increase in mean glomerular volume (MGV, r = -0.79, p less than 0.0001). The "compensating" increase in MGV maintained or incr

Oxidized Low-Density Lipoprotein Drives Dysfunction of the Liver Lymphatic System.

BACKGROUND AND AIMS: As the incidence of nonalcoholic steatohepatitis (NASH) continues to rise, understanding how normal liver functions are affected during disease is required before developing novel therapeutics which could reduce morbidity and mortality. However, very little is understood about how the transport of proteins and cells from the liver by the lymphatic vasculature is affected by inflammatory mediators or during disease. METHODS: To answer these questions, we utilized a well-validated mouse model of NASH and exposure to highly oxidized low density lipoprotein (oxLDL). In additi

An In Vivo Blood-brain Barrier Permeability Assay in Mice Using Fluorescently Labeled Tracers.

Blood-brain barrier (BBB) is a specialized barrier that protects the brain microenvironment from toxins and pathogens in the circulation and maintains brain homeostasis. The principal sites of the barrier are endothelial cells of the brain capillaries whose barrier function results from tight intercellular junctions and efflux transporters expressed on the plasma membrane. This function is regulated by pericytes and astrocytes that together form the neurovascular unit (NVU). Several neurological diseases such as stroke, Alzheimer's disease (AD), brain tumors are associated with an impaired BBB

Heterogeneous Wettability Alters Methane Migration and Leakage in Shallow Aquifers.

Capillary heterogeneity is increasingly recognized as a first-order control of gas plume migration and trapping in aquifers and storage formations. We show that spatial variability in the water-methane contact angle, set by mineralogy and salinity, reshapes capillary entry pressures and, in turn, migration pathways. Using molecular dynamics simulation, we estimate contact angles on quartz and kaolinite under fresh and saline conditions and embed these results in continuum-scale multiphase flow simulations via a contact-angle-informed Leverett J-function, mapping wettability directly onto the f

Evaluation of Microcirculation, Cytokine Profile, and Local Antioxidant Protection Indices in Periodontal Health, and Stage II, Stage III Periodontitis.

Periodontitis, initiated by the subgingival biofilm and modified by the individual's inflammatory/immune response, has been associated with vascular dysfunction. To analyze microcirculation indices in periodontal tissues and determine the activity of the enzymatic component of antioxidant defense and humoral immunity factors, a single-blind non-invasive clinical trial was realized. Forty subjects, aged from 30 to 65 years, with moderate to severe chronic periodontitis (chronic generalized periodontitis, CGP) vs. 40 subjects as periodontally healthy were recruited. Information such as capillary

Atypical Hemorrhages Secondary to Dengue Infection and Cardiopulmonary Resuscitation: Subcutaneous Thrombin Injection as a Salvage Technique for Refractory Femoral Artery Hemorrhage.

Dengue fever is a viral infection transmitted by the bites of female Aedes mosquitoes. Repeat infections with different viral serotypes are possible, with an increased risk of severe dengue. Dengue hemorrhagic fever is one of the most severe presentations of dengue, with thrombocytopenia, increased capillary permeability with resultant rash, and an increased risk of spontaneous bleeding. The management of severe dengue is done through supportive care and symptomatic management only, as there are no specific treatments available. We describe a case of severe dengue hemorrhagic fever presenting …

A permeability barrier to lanthanum and the presence of collagen between odontoblasts in pig molars.

Previous experiments in rat incisors indicate that the odontoblasts form an impermeable barrier which prevents fluid movement between pulp and dentine. The permeability of the odontoblast layer has now been investigated in pig molars which are more analogous to human teeth. The heads and necks of anesthetised piglets were perfused intra-arterially with lanthanum nitrate in Ringer's solution or with Ringer's solution alone. Molar tooth germs were removed, sliced, fixed by immersion and embedded in resin. Ultrathin sections including pulp and dentine were examined by transmission electron micros

Idiopathic systemic capillary leak syndrome and related shock in PICU: an underdiagnosed disease?

BACKGROUND: Idiopathic systemic capillary leak syndrome (SCLS), or Clarkson syndrome, is a rare disorder characterized by increased capillary permeability causing fluid leakage into interstitial spaces. SCLS remains underdiagnosed in both adults and children due to its resemblance to septic shock, with particularly limited pediatric data available. METHODS: We conducted a retrospective analysis of six pediatric patients with seven SCLS episodes diagnosed in two Italian PICUs over a ten-year period. RESULTS: Patients aged 4-10 years presented with hypovolemic shock and the characteristic tria…

HIF-1α: a bridge connecting sepsis and acute respiratory distress syndrome.

Sepsis is a life-threatening condition marked by an abnormal host response to infection that can result in organ dysfunction, making it recognized as one of the primary causes of acute respiratory distress syndrome (ARDS). The pathophysiology of sepsis involves a cascade of events, including heightened pulmonary capillary permeability, dysfunction of alveolar epithelial cells, and the infiltration of inflammatory cells, such as neutrophils, macrophages, monocytes, and lymphocytes. The presence of these inflammatory cells triggers capillary leakages, alveolar epithelial damage, and the accumula…

Advances in imaging analysis of the inner ear in patients with Meniere's disease.

PURPOSE OF REVIEW: Visualization of endolymphatic hydrops using MRI has become a cutting-edge method not only for diagnosing Meniere's disease but also for pathophysiological elucidation of the disease. Here, we review recent advances in imaging analysis of the inner ear in Meniere's disease, which could provide additional information over previous findings. RECENT FINDINGS: In addition to identification of endolymphatic hydrops on MRI, high-intensity signals in the perilymph, which represent alterations in vascular permeability in the blood-perilymph barrier (BPB), might be a key to elucidat
