Variability in biological monitoring of solvent exposure. I. Development of a population physiological model.

Biological indicators of exposure to solvents are often characterised by a high variability that may be due either to fluctuations in exposure or individual differences in the workers. To describe and understand this variability better a physiological model for differing workers under variable industrial environments has been developed. Standard statistical distributions are used to simulate variability in exposure concentration, physical workload, body build, liver function, and renal clearance. For groups of workers exposed daily, the model calculates air monitoring indicators and biological

[Hemorrheologic findings in severe gestosis].

On the basis of the hemorrheologic and hemodynamic data of 30 severe gestoses the importance of rheology in the diagnosis and treatment of these conditions is discussed. It was found that the gestoses can occur with a variety of clinical phenomena (HELLP syndrome, hypervolemic gestosis). The classic picture comprises hemoconcentration, increased erythrocyte aggregation, low central venous pressure and low-normal cardiac output with increased blood viscosity. Depending on the condition of the microcirculation, laboratory values can fluctuate considerably. The mean antithrombin-III value was not

Autonomic control of the cardiovascular system in the cat during hypoxemia.

This study aimed to determine the roles played by the autonomic interoreceptors, the carotid bodies (cbs) and the aortic bodies (abs) in anesthetized, paralyzed, artificially ventilated cats' response to systemic hypoxemia. Four 15min challenges stimulated each of 15 animals: (1) hypoxic hypoxia (10%O₂ in N₂; HH) in the intact (int) cat where both abs and cbs sent neural traffic to the nucleus tractus solitarius (NTS); (2) carbon monoxide hypoxia (30%O₂ in N₂ with the addition of CO; COH) in the intact cat where only the abs sent neural traffic to the NTS; (3) HH in the cat after transection o

Endovascular treatment of an iatrogenic large vessel arteriovenous fistula presenting as high output heart failure: a case report.

Formation of an iatrogenic chronic large vessel arteriovenous (AV) fistula is an uncommon, early or late complication of spine surgery and associated with serious consequences. Its diagnosis is often delayed for months or years after the occurrence and rarely only after the patient develops heart failure secondary to a mature AV fistula. We present the case of a 43-year-old man with high-output heart failure due to an iatrogenic large vessel AV fistula after lumbar disc surgery successfully treated with the endovascular technique.

Paradoxical Sinus Bradycardia in the Setting of Acute Cardiac Tamponade: A Case Report.

Cardiac tamponade is a life-threatening condition classically associated with compensatory sinus tachycardia due to impaired diastolic filling and reduced cardiac output; however, paradoxical sinus bradycardia is rare and may delay recognition. We report a case of a 43-year-old woman with obesity status post gastric sleeve, asthma, and hyperlipidemia who presented with two weeks of progressive dyspnea, pleuritic chest pain, and chest tightness following a suspected viral syndrome, with symptoms worsening supine and improving when leaning forward. Initial evaluation demonstrated muffled heart s

Inhaled nitric oxide and postoperative outcomes in cardiac surgery with cardiopulmonary bypass: A systematic review and meta-analysis.

Cardiac surgeries under cardiopulmonary bypass (CPB) are complex procedures with high incidence of complications, morbidity and mortality. The inhaled nitric oxide (iNO) has been frequently used as an important composite of perioperative management during cardiac surgery under CPB. We conducted a meta-analysis of published randomized clinical trials (RCTs) to assess the effects of iNO on reducing postoperative complications, including the duration of postoperative mechanical ventilation, length of intensive care unit (ICU) stay, length of hospital stay, mortality, hemodynamic improvement (the

Normothermic Crystalloid Polarizing Cardioplegia Improves Systolic and Diastolic Function in a Porcine Model of Cardiopulmonary Bypass.

Previously, we showed that blood-based polarizing cardioplegia exerted beneficial cardioprotection during hypothermic ischemia; however, these positive effects of blood-based polarizing cardioplegia were reduced during normothermic ischemia compared to blood-based hyperkalemic (depolarizing) cardioplegia. This study compares crystalloid polarizing cardioplegia to crystalloid depolarizing cardioplegia in a normothermic porcine model of cardiopulmonary bypass; Methods: Twelve pigs were randomized to receive either normothermic polarizing (n = 7) or depolarizing (n = 5) crystalloid cardioplegia.

High-output heart failure due to a giant left circumflex coronary aneurysm with a left atrial fistula and atrial septal defect: a case report.

BACKGROUND: A coronary artery aneurysm is a rare cardiac anomaly that may be incidentally detected on echocardiography. When associated with a coronary cameral fistula, an aneurysm can become symptomatic. We present a unique case of a giant left circumflex coronary aneurysm with a fistula to the left atrium and a large atrial septal defect causing acute heart failure in a young woman during the peripartum period. CASE SUMMARY: A 32 year-old woman who presented with hypoxia after the delivery of her fourth child was found to have heart failure with severe mitral regurgitation and multiple abno

Echocardiographic indices of left ventricular function and filling pressure are not related to blood pool activity on pyrophosphate scintigraphy.

BACKGROUND: Pyrophosphate (PYP) imaging has a high diagnostic accuracy for transthyretin cardiac amyloidosis (ATTR-CA). Indeterminate findings are often reported due to persistent blood pool activity, presumed to be from low cardiac output. We evaluated the relationship between blood pool activity on PYP imaging and echocardiographic indices of cardiac function. METHODS: Clinical and imaging data of 189 patients referred for PYP scintigraphy were evaluated. All patients underwent planar imaging and SPECT (diagnostic standard). Among those with a negative PYP SPECT, persistent left ventricular

[Cardiac surgery in the aged].

One hundred and seven patients aged greater than or equal to 60 years with cardiac surgery were reviewed. These patients underwent open heart surgery at University Hospital of Tokyo and JR Tokyo General Hospital during 1981 to 1987. Prevalence of the elderly with 60 years or older in all patients with cardiac surgery increased 4.1% to 20% during these seven years. Surgery for ischemic heart disease has become more common. There were less number of cases with valvular heart disease referred for surgery, but, not a few cases with calcified aortic valve or floppy mitral valve had valve replacemen

Bupivacaine-induced cardiac arrhythmias in sheep.

Controversy persists about the cardiac toxicity of bupivacaine if accidentally administered intravenously during regional anesthesia. Using awake, unanesthetized sheep, we evaluated the cardiac effects of low and high equivalent doses of lidocaine and bupivacaine given intravenously over 10 s. All animals convulsed within 30 s of injections. Although both drugs significantly increased heart rate and systemic and pulmonary arterial blood pressure for up to 10 min, cardiac output was affected variably. The magnitude of hemodynamic changes that each drug produced did not differ significantly from

Pediatric cardiac emergencies.

Successful management of pediatric cardiac emergencies requires an accurate diagnosis to institute an appropriate plan of therapy. The diagnosis, however, is not always straightforward, as evidenced by the nonspecific clinical picture that can be presented by congenital heart defects. Entertaining the possibility of a cardiac problem in neonates with pulmonary symptoms unresponsive to standard therapies is crucial for successful management of patients with congenital heart disease. In addition to ventilatory support, prostaglandin E1 infusions or emergency interventional cardiac catheterizatio

Segmental Spinal Anesthesia With Fascial Plane Block for Cesarean Delivery in Severe Mitral Stenosis: A Case Report.

Severe mitral stenosis (MS) during pregnancy represents a fixed cardiac output state and poses major anesthetic challenges during elective cesarean delivery (CD). Conventional single-shot spinal anesthesia may precipitate profound hypotension in these patients. We report two high-risk parturients with severe MS, classified as New York Heart Association (NYHA) class III, who underwent elective CD. Both patients were managed using low-dose segmental spinal anesthesia supplemented with ultrasound-guided erector spinae plane block (ESPB) and invasive hemodynamic monitoring. In both cases, the proc

Post-resuscitation strategies to avoid ongoing injury following intrapartum hypoxia-ischemia.

The interruption of placental blood flow during labor with redistribution of cardiac output resulting in increased flow to brain, heart, and adrenal glands at the expense of flow to kidney, gut, and skin can result in systemic organ as well as cerebral injury. Thus, post-resuscitation strategies should focus on both the management of potential systemic organ dysfunction and on methods of preventing ongoing brain injury in high-risk infants. General management strategies should include ventilator management to maintain pCO(2) values in the normal range, close attention to blood pressure to avoi

Spontaneous positive end-expiratory pressure (sPEEP).

Spontaneous positive end-expiratory pressure (sPEEP) is a simple, low-cost method of providing low to moderate levels of PEEP as an alternative to continuous positive airway pressure (CPAP), whether used alone or with continuous positive pressure ventilation (CPPV) in intermittent mandatory ventilation (IMV). The therapeutic effects of sPEEP therapy are demonstrated in 7 adult respiratory distress syndrome patients. All patients, initially hypoxemic on a high F1O2, had an acceptable PaO2 (with F1O2 less than or equal to 0.4) within 12 hours of initiation of sPEEP therapy. sPEEP, unlike CPAP an

Dose-related differences in the distribution of cocaine in the maternal-fetoplacental compartments in rats.

Our goals were to examine whether a high dose of cocaine to causing CNS toxic manifestations in the pregnant rats influences the delivery of cocaine to the fetus, and whether the non-placental compartments have a significant role in the distribution of cocaine to the fetal tissues. Either a low or high dose of cocaine was infused intravenously to near-term pregnant rats. Arterial blood pressure and heart rate were monitored. Cardiac output and uterine and placental blood flows were measured by using radiolabeled microspheres. Plasma and tissue samples were obtained from the mother, placenta, a

Circular Shunt: A Loop Not to Be Ignored.

A circular shunt, initially described by Shone et al. in 1962, refers to abnormal blood recirculation through complete intracardiac or intra- and extracardiac communications, bypassing the capillary beds. This pathophysiological condition is most commonly associated with complex congenital heart defects, such as Ebstein's malformation, pulmonary atresia, Gerbode defect, and so on. Circular shunts significantly disrupt cardiac output and systemic perfusion, leading to heart failure, hypotension, and organ dysfunction. Clinical presentation varies from severe neonatal distress to progressive hea
