Six Southwestern American Indian patients are reported to have primary myocardial disease or congestive failure of unknown cause. Two are Navajo, three are Laguna, and one is Isleta. Clinical and laboratory features are discussed. This disease appears in culturally traditional Indians and is not associated with contact with American civilization. It occurs in probably adequately nourished Indians in the majority of whom excessive alcohol consumption is not found. No familial cases were noted. This syndrome is probably not uncommon in the American Indian.

BACKGROUND: Research comparing the survival of children with familial dilated cardiomyopathy (FDCM) to that of children with idiopathic dilated cardiomyopathy (IDCM) has produced conflicting results.
METHODS AND RESULTS: We analyzed data from children with FDCM or IDCM using the National Heart, Lung, and Blood Institute-funded Pediatric Cardiomyopathy Registry. Compared to children with IDCM (n=647), children with FDCM (n=223) were older (mean 6.2 versus 4.5 years, P<0.001), less often had heart failure (64% versus 78%, P<0.001), had less-depressed mean left ventricular fractional shortening

Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disorder, yet remains substantially underdiagnosed in clinical practice. Although current guidelines define HCM primarily based on left ventricular wall thickness, this approach alone does not capture the full spectrum of disease expression. Comprehensive phenotyping using multimodality imaging, combined with genetic testing and systematic family screening, allows for more accurate diagnosis and earlier detection, although its integration into routine clinical algorithms remains incomplete. Clinical outcomes and therapeutic

BACKGROUND AND AIMS: Atrial cardiomyopathy (AtCM) is increasingly recognized as an important substrate for atrial fibrillation (AF). This study aimed to examine potential markers and risk factors of AtCM, and associations with incident AF, heart failure (HF), and stroke.
METHODS: Individuals from the UK Biobank with cardiac magnetic resonance imaging and electrocardiographic information were included. Atrial cardiomyopathy markers included left atrial dilation, left atrial mechanical dysfunction, P-wave prolongation, and abnormal P-wave terminal force. Risk factors for AtCM were assessed usin

Arrhythmogenic cardiomyopathy (ACM) is a form of inheritable cardiomyopathy characterized by ventricular scarring and/or fibro-fatty replacement. ACM is associated with a significant risk of sudden cardiac death due to ventricular arrhythmias, especially in young patients and endurance athletes. Aim of this review is to summarize the changing in understanding and framing of this multifaceted disease, in particular highlighting the current transition from a phenotype-first to a genotype-first framework for diagnosis and management.

Hypertrophic cardiomyopathy (HCM) is a genetically determined myocardial disease in which symptoms are driven not simply by ventricular hypertrophy, but by a combination of dynamic left ventricular outflow tract obstruction (LVOTO), impaired diastolic filling and other factors. The therapy for HCM differs fundamentally from routine heart failure management and must be mechanism based. In obstructive HCM, the pharmacologic objective is to blunt hypercontractility, lengthen diastole, preserve loading conditions, and reduce the LVOT gradient. In non-obstructive HCM, management is focused on sympto

Stress-induced cardiomyopathy (siCM), also known as Takotsubo syndrome, is an acute, transient, and typically reversible form of myocardial dysfunction. In the intensive care unit (ICU), siCM remains underrecognized because its clinical presentation, electrocardiographic abnormalities, and biomarker profile often overlap with acute coronary syndromes and other critical illness-associated cardiomyopathies. Unlike the emotional triggers commonly described in ambulatory populations, ICU-associated siCM is more frequently precipitated by severe physiologic stressors, including sepsis, neurologic i…

Peripartum cardiomyopathy (PPCM) is a rare, but potentially life-threatening condition that typically presents during the final month of pregnancy, or within the first few months postpartum. PPCM is characterized by a decrease in left ventricular function that leads to heart failure in pregnant women with no previous history of cardiovascular disease, and for which the etiology of heart failure cannot be identified. Despite advances in understanding its molecular and clinical underpinnings, PPCM remains a diagnosis of exclusion. This review provides an overview of PPCM, including discussions o

Cardiomyopathy is a condition in which the myocardium is diseased, resulting in structural and functional abnormalities. This review focuses on nonischemic causes for cardiomyopathies. The overall presentation can vary in severity and age at symptom onset depending on the underlying cause. Symptoms depend on the type of cardiomyopathy but often include dyspnea, fatigue, arrhythmia, and syncope. Cardiomyopathies can be classified as primary (related to conditions isolated to the heart) or secondary (related to systemic disease such as amyloidosis). The most common primary cardiomyopathy is hype…

CASE SUMMARY: A 50-year-old man and a 33-year-old woman with obstructive hypertrophic cardiomyopathy underwent auscultation and Doppler echocardiography at our specialty center. Both had a systolic ejection murmur that increased with Valsalva strain and on standing. During prompt squat, the murmur disappeared in the first patient and became grade I in the second. This is to our knowledge the first documentation of confirmation of the hemodynamic changes associated with prompt squat.
TAKE-HOME MESSAGES: Mastery of bedside maneuvers such as squatting enhances clinical assessment in obstructive

Dilated cardiomyopathy (DCM) is a leading cause of nonischemic heart failure. Genetic causes are identifiable in 25% to 30% of patients. This review describes the pathophysiology underlying genetic DCM, the genes most strongly associated with DCM, and both standard and novel therapies for treating genetic DCM.
