Endocarditis due to group B streptococci occurred in two men associated with meningitis in one and septic arthritis in the other. Therapy with penicillin failed in the first patient necessitating aortic valve replacement. Clindamycin therapy was not curative in the second and the patient responded to a four-week course of vancomycin therapy.

A consecutive series of 18 patients with clostridial myonecrosis has been reviewed. The two deaths were attributed to other causes. A treatment regimen including repeated debridement and excision of all non-viable tissue, and therapeutic antibiotics is outlined. Hyperbaric oxygenation has been omitted from our treatment program.

We evaluated antibiotic resistance selection in Streptococcus pneumoniae isolates from children participating in an individually randomized trial of single-dose azithromycin versus placebo. After 14 days, the prevalence of resistance to erythromycin, oxacillin, and clindamycin was elevated in the azithromycin versus placebo group. There was no difference at 6 months.

We report the case of a young adult with a history of an allergic reaction to a sulfonamide antibiotic who developed toxoplasmosis after his second orthotopic heart transplant. As a result of this drug allergy, the patient did not receive prophylaxis with trimethoprim and sulfamethoxazole. He was successfully treated with clindamycin, pyrimethamine, and folic acid.

The species composition of aerobic and anaerobic microflora of paratonsillar abscesses was investigated in 20 patients. It was found that obligate nonspore-forming anaerobic bacteria predominated in the pus of the abscess cavity. Anaerobic infection was diagnosed rapidly by gas-liquid chromatography. Anaerobic bacteria were shown to be highly sensitive to clindamycin and metronidazol.

The clinical success rate in the diagnosis of acute pelvic inflammatory disease (PID) is unacceptably low. There is an invaluable role for laparoscopy in the diagnosis, investigation and treatment of patients with suspected pelvic infection. PID at Ngwelezana Hospital is predominantly gonococcal and a combination of penicillin G and clindamycin has proved highly effective in the management of these patients.
