Coaching doctors to improve ethical decision-making in adult hospitalized patients potentially receiving excessive treatment. The CODE stepped-wedge cluster randomized controlled trial.

PURPOSE: The aim of this study was to assess whether coaching doctors to enhance ethical decision-making in teams improves (1) goal-oriented care operationalized via written do-not-intubate and do-not attempt cardiopulmonary resuscitation (DNI-DNACPR) orders in adult patients potentially receiving excessive treatment (PET) during their first hospital stay and (2) the quality of the ethical climate. METHODS: We carried out a stepped-wedge cluster randomized controlled trial in the medical intensive care unit (ICU) and 9 referring internal medicine departments of Ghent University Hospital betwe

Innovative decision making tools using discrete mathematics for stress urinary incontinence treatment.

In this study, we applied graph theory to clinical decision-making for Stress Urinary Incontinence (SUI) treatment. Utilizing discrete mathematics, we developed a system to visually understand the shortest path to the desired treatment outcomes by considering various patient variables. Focusing on women aged 35-50, we examined the effectiveness of Tension-free Vaginal Tape (TVT) surgery and Vaginal Erbium Laser (VEL) treatment for over 15 years. The TVT group consisted of 102 patients who underwent surgery using either the Advantage Fit mid-urethral sling system (Boston Scientific Co., MA, USA…

The Impact of Patient Race, Patient Socioeconomic Status, and Cognitive Load of Physician Residents and Fellows on Chronic Pain Care Decisions.

Social stereotypes are more likely to influence decision-making under conditions of high cognitive load (ie, mental workload), such as in medical settings. We examined how patient race, patient socioeconomic status (SES), physician cognitive load, and physician implicit beliefs about race and SES differences in pain tolerance impacted physicians' pain treatment decisions. Physician residents and fellows (N = 120) made treatment decisions for 12 computer-simulated patients with back pain that varied by race (Black/White) and SES (low/high). Half of the physicians were randomized to be interrupt

[Shared decision making even for complex systemic autoimmune diseases such as Systemic Lupus Erythematosus (SLE)?].

OBJECTIVE: The treatment of systemic autoimmune diseases, such as systemic lupus erythematosus (SLE) is an enormous challenge for all involved due to the complexity. This study investigated whether shared decision making (SDM) can advantageously be implemented in such a situation. METHOD: To answer this question qualitative group surveys were carried out with rheumatologists according to the standardized method of K&P Psychodrama®. A total of 41 German rheumatologists who had been invited by GlaxoSmithKline (GSK) Germany participated in 2 meetings over a complete day in 2012 and 2013 as well

Constructing High-stakes Surgical Decisions: It's Better to Die Trying.

OBJECTIVE: To explore high-stakes surgical decision making from the perspective of seniors and surgeons. BACKGROUND: A majority of older chronically ill patients would decline a low-risk procedure if the outcome was severe functional impairment. However, 25% of Medicare beneficiaries have surgery in their last 3 months of life, which may be inconsistent with their preferences. How patients make decisions to have surgery may contribute to this problem of unwanted care. METHODS: We convened 4 focus groups at senior centers and 2 groups of surgeons in Madison and Milwaukee, Wisconsin, where we

Determining clinical decision thresholds for HIV-positive patients suspected of having tuberculosis.

Clinical decision thresholds may aid the evaluation of diagnostic tests but have rarely been determined for tuberculosis (TB). We presented clinicians with six web-based clinical scenarios, describing patients with HIV and possible TB at various sites and with a range of clinical stability. The probability of disease was varied randomly and clinicians asked to make treatment decisions; threshold curves and therapeutic thresholds were calculated. Test and treatment thresholds were calculated using Bayes theorem and the diagnostic accuracy of Xpert MTB/RIF. We received 165 replies to our survey.

Surgical wisdom as phronesis: An evidence-based hypothesis about "understanding… the great gray that exists in the field".

BACKGROUND: Surgeons make vital decisions in the face of uncertainty, yet, the development of surgical wisdom necessary to make these choices prudently remains understudied and undefined. On the basis of previous commentary, we asked whether phronesis, the ability to combine epistemic knowledge and technical skill and choose the best action from many possible right actions, provides a useful conceptual framework to explicate the notion of surgical wisdom. METHODS: We conducted a thematic analysis. One-hour in-depth semistructured interviews were conducted with a convenience sample of faculty

Clinical Relevance of Large Language Models in Endodontics: Diagnostic Appropriateness Based on 50 Simulated Case Scenarios.

Large language models (LLMs) are increasingly used in healthcare, but their performance in endodontic decision-making remains unclear. This study aimed to compare six LLMs in terms of diagnostic appropriateness for endodontic treatment planning. Fifty clinical scenarios were developed and entered into six LLMs (ChatGPT-4o, ChatGPT-3.5, Claude 4, Copilot, DeepSeek-V3, Gemini 2.5). Two specialists scored responses as appropriate or inappropriate. Repeated measures ANOVA and chi-square tests were used for analysis. Claude showed the highest accuracy (76%), followed by DeepSeek and Gemini. ChatGPT

Mapping cognitive biases in multidisciplinary team (MDT) decision-making for cancer care in Scotland: a cognitive ethnography study protocol.

INTRODUCTION: The efficiency of multidisciplinary teams (MDTs) in cancer care hinges on facilitating clinicians' cognitive processes as they navigate complex and uncertain judgements during treatment planning. When systems and workflows are not designed to adequately support human judgement and decision-making, even experts are prone to fallible reasoning due to cognitive biases. Incomplete integration of information or biased interpretations of patient data can lead to clinical errors and delays in the implementation of treatment recommendations. Though their impact is intuitively recognised,

How Should Physicians Manage Neuroprognosis with ECPR?

Rapidly advancing technologies in the field of extracorporeal cardiopulmonary resuscitation (ECPR) have presented a new challenge in accurate neuroprognostication following cardiac arrest. Determination of brain state informs the prognostic picture and allows providers to begin effective communication regarding likelihood of meaningful neurological recovery as defined by patients or family members. The evolving role of sedation during ECPR and its impacts on ethical tension in decision-making is reviewed. Work surrounding the advancing field of neuroprognostication after cardiac arrest and hyp

Let's Code! How Can Programming Logic Make a Vascular Neurologist Even Better?

BACKGROUND: As digital health and artificial intelligence (AI) become integral to medicine, there is a growing need for physicians to develop computational thinking skills. In vascular neurology, a specialty reliant on algorithmic decision-making and complex data interpretation, programming logic (PL) offers a powerful cognitive framework. This review argues that PL can enhance diagnostic precision, clinical efficiency, and data-driven reasoning. By aligning core programming structures - such as conditional statements, loops, and data abstraction - with clinical workflows, neurologists can imp
