BACKGROUND: One of the complications of forearm shaft fracture is refracture. Elastic stable intramedullary nailing represents an alternative method for refracture treatment to cast immobilisation for another five to seven weeks. Operative treatment often necessitates an open reduction in most cases due to blocked or narrowed medullary canals. The purpose of this study was to examine the expense of the operative procedure, technique (closed or open intramedullary nailing) and postoperative complications in diaphyseal forearm refractures.
METHODS: We retrospectively examined the expense of ope

INTRODUCTION: Birth injuries are common phenomenon. But commonly encountered birth injuries are clavicle fracture, humerus fractures and uncommonly femur fracture. Incidence of Distal humerus Epiphyseal injuries as birth injury are rare. As the first secondary centre around elbow-capitellum appears at 3-9 months, its radilological assessment of alignment with elbow or the radius shaft is not possible in neonates. It is difficult to differentiate elbow dislocation from epiphyseal injury in neonates.
CASE REPORT: Here we are reporting a case of lower end humerus epiphyseal injury, as birth inju

BACKGROUND: It has been advocated that reduction of nasal bone fractures should be followed by internal packing and/or external splinting. Despite the ample literature concerning the advantages and limitations of various splint types, the necessity and effectiveness of external splinting has not been well documented.
OBJECTIVE: To present the authors' experience and review the literature on treatment of nasal bone fractures, focusing on the indications and effectiveness of external splinting following closed reduction.
STUDY DESIGN: Retrospective analysis and literature review.

There is a specific type of displaced four-part fracture of the proximal humerus which consists of valgus impaction of the head fragment; this deserves special consideration because the rate of avascular necrosis is lower than that of other displaced four-part fractures. Using either closed reduction or limited open reduction and minimal internal fixation, 74% satisfactory results can be achieved in this injury.

In general, all traumatic dislocations of the hip must be treated as surgical emergencies. Multiple attempts at closed reduction are contraindicated, particularly in Type V dislocations. Every effort must be made to recognize the dislocation, particularly in patients with other severe lower extremity trauma. Reduction within 24 hours gives better results than late reductions. Roentgenograms of the pelvis must include both hips after closed or open procedures as a check for a concentric reduction of the hip. Any abnormality, or failure to reduce the avulsed head fragment, demands an immediate h

BACKGROUND: Most of the Intraarticular distal radius fractures are complex and unstable. They are caused by high-energy injuries. The optimal and appropriate treatment remains a topic of controversy. Many different methods of treatment have been advocated such as closed reduction maintained by cast, K-wire fixation with cast, and opened reduction with internal fixation with plate and screws or external fixation with or without K-wire augmentation.
OBJECTIVE: To study the efficacy of the TU Model External Fixator for the treatment of unstable intraarticular fracture of distal radius. This was

PURPOSE: To evaluate a stepwise reduction algorithm including closed, percutaneous, and open reduction techniques followed by percutaneous pin fixation for displaced pediatric phalangeal neck fractures.
METHODS: Sixty-one consecutive children (mean age, 9.4 y; range, 2-18 y) presenting with closed, type II displaced phalangeal neck fractures were treated using the following algorithm. If satisfactory reduction was achieved with closed reduction, percutaneous pinning (CRPP) was performed. If not achieved, then percutaneous reduction and pinning (PRPP) was performed using a temporary intrafocal

Arteriovenous fistula formation after a closed extremity fracture is rare. We present the case of an 11-year-old boy who developed an arteriovenous fistula between the anterior tibial artery and popliteal vein after closed fractures of the proximal tibia and fibula. The fractures were treated by closed reduction and casting. A fistula was diagnosed 12 weeks after the injury. It was treated by embolisation with coils. Subsequent angiography and ultrasonography confirmed patency of the popliteal vein and anterior and posterior tibial and peroneal arteries, with no residual shunting through the f

PURPOSE: The purpose of the present study is to report an intraoperative method of determining which condylar process fractures of the mandible do not require open reduction and internal fixation.
PATIENTS AND METHODS: A total of 332 patients with unilateral extracapsular fractures of the mandibular condylar process were retrospectively studied. After any other mandibular fractures had undergone open reduction and internal fixation, the maxillomandibular fixation was released and the occlusion checked to determine whether deviation of the mandible was present toward the side of the condylar f

BACKGROUND: This study provides intermediate-term follow-up data on the vascular status of a cohort of children with a perfused, pulseless supracondylar humeral fracture who were managed with closed reduction, percutaneous pinning, and observation.
METHODS: Pediatric patients who sustained a perfused, pulseless supracondylar humeral fracture in the period from 2007 to 2011 and who had at least six months of clinical follow-up were identified and returned for evaluation. The primary outcome was vascular status as indicated by palpation of the radial pulse, the wrist brachial index, and arteria

OBJECTIVE: To evaluate whether listening to music through binaural headphones contributes to the perception of pain and anxiety in patients undergoing closed nasal bone fracture reductions.
STUDY DESIGN: Randomized controlled trial.
SUBJECTS AND METHODS: We recruited patients from San Juan de Dios Hospital with displaced nasal fractures who required a reduction and assigned them to a control group or a music group. For both groups, a protocolized closed reduction of the nasal fracture with local anesthesia was performed. The music group heard music through headphones during the pre-, intra-,

PURPOSE: Femoral neck fractures are clinically rare and are associated with a high risk of complications in children. Traditional internal fixation implants such as Kirschner wires and partial-thread cannulated screws (PTCS) have complications such as screw withdrawal and internal fixation failure. To address this problem, in this study we investigated the effectiveness of headless cannulated compression screws (HCCS) in the treatment of femoral neck fractures in children patients.
METHODS: Children diagnosed with Delbet-Colonna II or III femoral neck fracture treated by closed reduction and

Background. Supracondylar humeral fractures are the most frequent fractures of the elbow in children. Gartland type III fractures require a surgical treatment. The preferred management is closed reduction and percutaneous pinning with K-wires. Methods. We have treated 15 patients (mean age 6,5 years) with Gartland type III fractures. In 14 patients the percutaneous reduction with K-wires was possible but in 1 case the open reduction was necessary due to the irreducibility of the fracture. In all cases a cycle of rehabilitation was performed. Results. All patients have been evaluated using Flyn

When both a coxarthrosis and a sub- or pertrochanteric fracture of the femur are simultaneously present, it is, as a rule, most difficult to carry out a closed reduction of the fracture on the extension table. In almost all cases, an open reduction has to be performed. When such an open reduction is necessary, this should not give reason to dispense with the advantages offered by the Ender nailing method and select another technique of osteosynthesis. An early mobilisation, which is of particular importance to old patients, can be achieved best by the aforementioned osteosynthesis. On the othe

The posterior sternoclavicular joint dislocation is a rare and potentially life-threatening injury, as massive haemorrhage can occur at the time of trauma, during reduction manoeuvres and drilling. These injuries are rare and a collective experience of managing them is of paramount importance. We present our multidisciplinary experience of managing several of these injuries in our centre, with learning points we have identified. Assessment should include Computerised Tomography Angiography (CTA) to assess the anatomy of the joint including the proximity to the underlying innominate vein and to

Isolated volar dislocation of the distal radioulnar joint (DRUJ) without associated fracture is a rare wrist injury that may be overlooked because clinical and radiographic findings can be subtle. We report the case of a 19-year-old male rodeo rider who presented with severe left wrist pain, swelling, and inability to rotate the forearm following a torsional injury. Radiographs demonstrated an isolated volar DRUJ dislocation without associated fracture. Initial closed reduction was unsuccessful. Under anesthesia, reduction was achieved but was unstable, requiring percutaneous radioulnar transf

Facial fractures in children are rare. Lack of pneumatization, fat pockets, mixed dentition, contribute to the elasticity and bone stability. When mandibular fractures occur in children, most often involve the condyle by indirect trauma. Such fractures are the center of discussion on the form of treatment if this should be performed conservatively, or held the reduction and fixation of the fracture with surgical exposure of the fragments. In condylar fractures in children, in most patients, the proposed treatment is closed reduction. Treatment with open reduction and fixation has its specific

BACKGROUND: Patellar dislocations are a commonly treated injury in the Emergency Department (ED), with a majority of cases involving lateral subluxation of the patella outside of the joint space. Intra-condylar dislocations of the patella are rare. Of the two types of axis rotation, vertical and horizontal, the vertical occurs five times less frequently. These injuries most often undergo open reduction or, at best, closed reduction under general anesthesia.
OBJECTIVES: To remind Emergency Physicians to consider this injury in any patient with severe knee pain and limited mobility, even with a
