[Gut hormone profiles after various types of gastrointestinal surgery].

Gastrointestinal and pancreatic hormone responses to test meal or oral glucose were studied in totally gastrectomized patients (TG), subtotally gastrectomized patients with BI and BII, patients with pylorus preserving gastrectomy (PPG), a case of massive (4m 15 cm) bowel resection (MBR) and patients with pancreatoduodenectomy (PD), and compared to the pattern of healthy control subjects (HC). Plasma IRI, GIP and GLI responses to meal in TG and BII were significantly higher, and those in BI appeared to be higher than those in HC. In contrast, those hormonal patterns in PPG were similar to HC. T

Gastroduodenostomy after gastric resection.

Gastroduodenostomy after gastric resection is a procedure which can be readily performed if the short gastric vessels are first divided. It makes for a more physiological restoration of the gastrointestinal tract than is accomplished with gastrojejunostomy and permits proper admixture of the food with the bile and pancreatic enzymes. It avoids several potential dangers such as leakage from the duodenal stump, the possibility of a gastrocolic fistula, and malfunction of the anastomosis due to distortion of a jejunal loop. Apparently the "dumping syndrome" does not occur after gastroduodenostomy

Five Lessons in Feeding Following Abdominal Surgery in Children: Can We Discontinue Unnecessary Fasting?

Fasting children after gastrointestinal surgery is traditionally thought to reduce complications. However, over the last two decades, there has been increasing interest in feed commencement within 24 h of operation, termed 'early enteral nutrition'. Evidence is now demonstrating that this approach to feeding aligns more closely with value-based healthcare principles than traditional postoperative fasting. This is reflected in the fact that early feeding can be safely offered and is associated with earlier return of bowel function, with shorter time to stool, quicker progression to full feeds a

Brazilian consensus- and evidence-based recommendations in the diagnosis and treatment of pancreatic exocrine insufficiency in patients after digestive surgeries. Position paper of six brazilian medical societies of surgery.

BACKGROUND: Exocrine pancreatic insufficiency (EPI) is a condition characterized by reduced exocrine secretion, leading to decreased food digestion, and digestive tract surgeries can be a cause. Postoperative "de novo" EPI is defined as the onset of digestive symptoms following surgeries, which show significant improvement after the initiation of pancreatic enzyme replacement therapy (PERT). The diagnosis of postoperative EPI may be delayed due to mild or nonspecific symptoms, both in pancreatic surgeries and in upper abdominal surgeries. AIMS: The aim of this study was to conduct a systemati

The Right Colon Patch Pull-through for Total Colonic Aganglionosis: A Systematic Review.

BACKGROUND: Total colonic aganglionosis (TCA) is a rare form of Hirschsprung Disease with many surgical techniques. There is limited good quality research to recommend one technique over another. The right colon patch (RCP) pull-through may have physiological benefits over other techniques but it is infrequently reported. This systematic review aims to describe this technique and report its outcomes. METHODS: A systematic review was conducted as per PRISMA guidelines using EMBASE and PubMed databases. Papers were included if patients underwent a right colon patch pull-through and had outcomes

Incidence and risk factors for postoperative gastrointestinal dysfunction occurrence after gastrointestinal procedures in US patients.

BACKGROUND: Incidence of, and potential risk factors for, postoperative gastrointestinal dysfunction (POGD) after gastrointestinal procedures performed in US hospitals were examined. METHODS: This retrospective study used hospital discharge data of inpatients who underwent ≥1 gastrointestinal procedures from 1-Jan-2016 to 30-Apr-2019. POGD incidence was calculated based on all hospitalizations for MDC-06 procedures. Predictors of POGD were assessed using multivariable logistic regression. RESULTS: POGD incidence was 5.8% among 638 611 inpatient hospitalizations. Major bowel procedures, perit

A simple and effective technique for laparoscopic gastrorrhaphy: modified Graham's patch with barbed suture.

INTRODUCTION: Peptic ulcers are caused by unbalanced acid production, and proton pump inhibitors (PPIs) in recent decades have helped to treat peptic ulcers effectively. Meanwhile, the incidence of perforated peptic ulcer (PPU) persists and has a high mortality rate if there is no adequate management. Primary closure with a modified Graham's patch was well performed in early detected PPU with a small size < 2 cm. A laparoscopic approach for PPU was prescribed for decades with proven feasibility and safety. We introduced an effective technique combined with barbed suture and modified Graham's p

Excision of Deep Endometriosis of the Rectosigmoid: Individualizing Care to the Presenting Pathology.

STUDY OBJECTIVE: To highlight different surgical approaches for managing deep infiltrating endometriosis involving the rectosigmoid colon. DESIGN: Demonstration of specific surgical techniques with educational narrated video footage. SETTING: Bowel endometriosis is reported in 3.8% to 37% of patients with endometriosis [1]. Most commonly, the rectosigmoid colon is involved. Pelvic ultrasound and magnetic resonance imaging may be useful in diagnosis and for surgical planning [2]. Treatment options include observation, medications, or surgery. There are various surgical techniques that can be

Comparison of oncological outcomes between low anterior resection and abdominoperineal resection for rectal cancer: A retrospective cohort study using a multicenter database in Japan.

BACKGROUND: It remains controversial whether the abdominoperineal resection (APR) procedure itself has a negative impact on prognosis compared with sphincter-saving surgery (SSS). The purpose of this study was to investigate whether the operation type affects the prognostic outcome in rectal cancer using a multicenter database in Japan. METHODS: The study involved 2533 patients who underwent APR or SSS and were registered in the Japanese Society for Cancer of the Colon and Rectum database, which includes data from 74 centers, between 2003 and 2007. The primary endpoints were overall survival

Impact of Objective Colonic and Whole Gut Motility Data as Measured by Wireless Motility Capsule on Outcomes of Antireflux Surgery.

BACKGROUND: Studies show higher rates of dissatisfaction with antireflux surgery (ARS) outcomes in patients with chronic constipation. This suggests a relationship between colonic dysmotility and suboptimal surgical outcome. However, due to limitations in technology, there is no objective data available examining this relationship. The wireless motility capsule (WMC) is a novel technology consisting of an ingestible capsule equipped with pH, temperature, and pressure sensors, which provide information regarding regional and whole gut transit times, pH and motility. The aim of this study was to
