Congenital cystic dilatation of the common bile duct, also known as choledochal cyst, is an uncommon malformation, observed mainly in asiatic children, with a 3:1 female--male ratio. The entity is classified into 4 different types according to the anatomy of the malformation, which effects or not the intrahepatic biliary tree. The records of 84 cows with cecal dilatation or cecal volvulus were reviewed. The diagnosis was made by exploratory laparotomy or necropsy in 21 cows with cecal dilatation, and in 45 cows with cecal volvulus. Eighteen of the cows with cecal dilatation responded to medical management and the diagnosis was made by physical examination alone. We are reporting a new kindred in which familial visceral myopathy was identified in four siblings. There was no other symptomatic case identified in this 75-member, four-generation family, suggesting an autosomal recessive inheritance. 38 stenoses of supra-aortic arteries were dilated with good success in 32 patients over a period of four years. They were 17 stenoses of the internal carotid artery, one of the common carotid artery, 16 of the subclavian artery, and four descending stenoses of the vertebral artery. Indication, method, and results of percutaneous transluminal dilatation of stenoses descending from the vertebral artery are described and discussed, with reference being made to case examples.

Dilatation of unobstructed extrahepatic bile ducts was observed in patients with conditions associated with intestinal hypomotility. For further investigation of this association, a prospective study was undertaken in which the common hepatic duct was measured in 15 patients before and 1 day after laparotomy, when all patients had postoperative paralytic ileus. A statistically significant (p less than 0.01) increase in the mean diameter of the hepatic duct was observed postoperatively. Laminaria japonica was used as pre-dilator of the pregnant uterine cervix prior to termination of pregnancy in the first and second trimester. In the first trimester (6-11 weeks, n=26) no other method was used, while in the second trimester pharmacological stimulation--prostaglandin F2 alpha or the prostaglandin analogue Sulprostone was used after or simultaneously with the laminaria insertion (12-18 weeks n=34 and 40, respectively). The dilatatory effect of laminaria was an obvious help in termination of both first and second trimester pregnancies.

Four patients with the fragile X syndrome including 3 males and one woman, were evaluated for cardiological abnormalities. One patient had an obvious murmur. All 4 were shown to have mitral valve prolapse by echocardiography. Two male patients also demonstrated mild dilatation of the ascending aorta. We recommend thorough cardiological evaluations of all fragile X patients.

Gastric dilatation was diagnosed in 156 dogs during a 5-year period. The recurrence rate was 10% and the overall mortality was 43%. Large-breed dogs of either sex and ranging in age from 2 months to 15 years (mean age, 5.2 +/- 2.9 years) were found to be susceptible to the problem. Cardiac arrhythmias were diagnosed in 48 of 115 (42%) affected dogs. Cardiac arrhythmias occurred most frequently between 12 and 36 hours after the onset of gastric dilatation-volvulus and were generally ventricular in origin.

Three cases of choledochal cyst associated with intrahepatic biliary dilatation are presented. Findings on sonography included a large cystic mass in the porta hepatis separate from the gallbladder; a dilated common hepatic or common bile duct entering directly into the cyst; the smaller cystic masses of dilated central intrahepatic ducts. During the first trimester of abortion, the Nelaton catheter can be used for the gradual, safe softening and dilatation of the cervix, just as the laminaria. Based on the results obtained from more than 1000 cases, we reached the conclusion that the effectiveness of cervical softening by the Nelaton catheter is almost equal to the laminaria and its efficiency in the dilatation of the cervix is similar or slightly inferior to the laminaria. Forty patients with symptomatic haemorrhoids had a Lord's procedure performed under general anaesthetic. Anal pressures were recorded before and after the dilatation. The patients were then randomly allocated to one of two regimens. Group 1 was given only a bulk laxative and group 2 was given a bulk laxative and a large anal dilator to use over the next few months according to Lord's regimen. When reviewed at 3 months and 12 months there was no difference in the results between the two groups. We conclude that the use of an anal dilator does not improve the result after a Lord's procedure.

During an 18-month period, gastric dilatation was recognized in six guinea pigs from a colony with a monthly average of 253 animals. All of the affected guinea pigs were breeders and ranged in age from 8 to 26 months. Clinical signs were tachycardia, dyspnea, cyanosis, and marked abdominal distention. Macroscopic lesions included gastric volvulus, gastric dilatation, and splenic congestion.
