The results of 37 consecutive cases of Hartmann's procedure over a 5-year period (1985-1990) were reviewed. The series consisted of 27 emergency procedures (11 diverticular disease, 13 carcinoma, two trauma and one sigmoid volvulus) and ten elective procedures (nine carcinoma and one peridiverticular abscess). The indications for emergency procedures were obstruction and perforation. All patients presented with faecal peritonitis owing to colonic perforation. The mean (range) age was 79.4 (34-90) years. The postoperative mortality rate was 30 per cent overall (11 of 37), 33 per cent (four of 1

Low-grade cystic mucinous neoplasm of the sigmoid colon has never been previously reported and a classification for such a tumor does not currently exist. Here, we present a case of low-grade cystic mucinous neoplasm of the sigmoid colon and discuss the differential diagnosis especially as it relates to clinical management. Our case is a 68-year-old male who presented with anemia and a history of a lower gastrointestinal tract bleed. He denied abdominal pain and weight loss. A colonoscopy was done for further investigation, and he was found to have a 4-cm nonbleeding submucosal mass in the des

Diverticular disease is one of the most common gastrointestinal pathologies, and its prevalence is rising in areas where diets are low in fiber and high in processed foods containing preservatives and emulsifiers. While this commonly affects the colon, in rare cases, chronic diverticulitis can lead to scarring and fibrosis that impact the small bowel. This case report describes the clinical course of a 56-year-old male with multiple comorbidities who presented to the emergency department for acute abdominal pain with worsening abdominal distention. Imaging revealed extensive intraperitoneal fr

74 colic perforations were surgically handled between 1975 and 1988. Among the non-traumatic perforations, which represent 74% of this series, 39 cases (71%) complicated the course of acute diverticulitis, 9 cases (15%) were due to cancers and 7 cases (13%) were of miscellaneous origin. Traumatic perforations (26%) were iatrogenically induced in 84% (16 cases). In 78% the perforation was located in the sigmoid or recto-sigmoid. The treatment consisted in performing either a diverting colostomy (30 cases--40%) or a colic resection (44 cases--60%) with or without immediate anastomosis (respectiv

The abdomen is a big place even in a small person. Gastrointestinal trauma can result in injury to the stomach, small bowel, colon, or rectum. Traumatic causes include blunt or penetrating trauma, such as gunshot wounds, stabbings, motor vehicle collisions, and crush injuries. Nontraumatic causes include appendicitis, Crohn disease, cancer, diverticulitis, ulcerative colitis, blockage of the bowel, and chemotherapy. The mechanism of injury will affect both the nature and severity of any resulting injuries. Treatment must address the critical and emergent nature of these injuries as well as iss

Enterovesical fistula caused by inflammatory bowel diseases is a rare disorder. Two male patients with recurrent cystitis had sigmoid colon diverticulitis causing enterovesical fistula. One female patient with Crohn's disease developed enterovesical fistula with abdominal and urinary symptoms. In each patient, a barium enema revealed causative bowel diseases. The precise diagnosis of enterovesical fistula was made either by cystography or cystoscopy. One-stage resection of the inflamed bowel with fistula and affected the bladder wall was proved to be an effective treatment for these three pati

Colocolic fistulas are usually a complication of an inflammatory or neoplastic process. Development of these abnormal bowel communications may lead to bacterial overgrowth. We report on a 71-year-old man with a one-year history of recurrent abdominal distension and irregular bowel habits. Abdominal X-rays and computed tomography showed multiple air-fluid levels and loops of distended bowel without evidence of mechanical obstruction or diverticulitis. Colonoscopy showed a fistulous tract between the sigmoid colon and cecum. Results of a lactulose breath test showed high fasting breath CH4 level

BACKGROUND: Whether inflammatory bowel disease patients are at increased risk of colonoscopic perforations remains controversial. Aim of this study was to investigate whether these patients are at increased risk of perforations.
METHODS: Electronic charts of all patients enrolled in the Nancy IBD cohort (1999-2012) were reviewed. All non-inflammatory bowel disease patients who underwent colonoscopy (1999-2012) were used as controls.
RESULTS: Of 17,992 colonoscopies, 2375 (13.2%) were performed for inflammatory bowel disease: 1547 for Crohn's disease and 828 for ulcerative colitis. Four IBD p

Since its first described case in 1991, laparoscopic colon surgery has lagged behind minimally invasive surgical methods for solid intra-abdominal organs in terms of acceptability, dissemination, and ease of learning. In colon cancer, initial concerns over port site metastases and adequacy of oncologic resection have considerably dampened early enthusiasm for this procedure. Only recently, with the publication of several large, randomized controlled trials, has the incidence of port site metastases been shown to be equivalent to that of open resection. Laparoscopic surgery for colon cancer has

INTRODUCTION: This study aims to investigate the current evidence regarding long-term outcomes using laparoscopic peritoneal lavage (LPL) versus primary bowel resection (PR) in Hinchey III diverticulitis.
METHODS: A systematic review was undertaken based upon articles published between January 1, 2000, and March 1, 2024. Databases Pubmed, Scopus, and Embase were used employing the key search terms "Diverticulitis" and "Peritoneal Lavage." Articles were selected according to the PRISMA guidelines and statistical analysis was undertaken. Cumulative analysis of diverticulitis recurrence and seco

The assumption that acute uncomplicated diverticulitis requires antibiotics has been challenged. In this issue, the Dutch Diverticular Disease Collaborative Study Group report the 2-year follow-up to their trial of observation vs. treatment with amoxicillin-clavulanic acid for acute uncomplicated diverticulitis. The new study provides useful insights on the natural history of acute uncomplicated diverticulitis. The most intriguing data compared rates of elective sigmoid resection between the groups. There was a trend toward more elective surgery in the observation group compared to those in th

INTRODUCTION: Modern management of severe acute complicated diverticulitis continues to evolve towards more conservative and minimally invasive strategies. Although open sigmoid colectomy with end colostomy remains the most commonly used procedure for the treatment of perforated diverticulitis with purulent/faeculent peritonitis, recent major advances challenged this traditional approach, including the increasing attitude towards primary anastomosis as an alternative to end colostomy and use of laparoscopic approach for urgent colectomy.
TECHNIQUE: Provided an accurate patients selection, hav

Pylephlebitis or septic portal thrombophlebitis is a rare but serious condition which may occur following intra-abdominal sepsis from any source. Sigmoid diverticulitis is one of the most common sources. Modern imaging modalities, particularly CT, have increased the recognition of this condition. Standard treatment consists of anticoagulation plus antibiotic therapy to cover anaerobic and gram negative organisms. The duration of anticoagulation therapy remains controversial. Sigmoid colectomy may be required in cases of perforated diverticulitis or failure of medical therapy.

A total of 31 patients with diverticulitis were analyzed who had both computed tomography and contrast enema. There was almost equal sensitivity to abnormality of approximately 90 percent. Contrast enema produced a specific diagnosis of diverticulitis in 61 percent, using stringent positive criteria, and an additional 29 percent with suggestive findings. Comparative computed tomography specific diagnoses in those 31 cases was made in 65 percent, and suggestive in 23 percent. Computed tomography was particularly useful diagnostically in cases of retrograde obstruction on contrast enema. The aut
