A case is presented of cecal appendicular diverticulum in a 52 year-old male, initially diagnosed as acute appendicitis. We emphasize the rarity or this pathology, its clinical presentation simulating a picture of acute appendicitis and its cure by appendectomy, as well as the diagnosis by pathologic study in most cases.

A 53-year-old nulliparous woman complained of terminal pain on urination and bended micturition. Previously, she had been treated unsuccessfully with antibiotics for chronic cystitis. The authors present the case of a 59 year-old patient with numerous stratified stercoliths within a large Meckel's diverticulum. An abdominal X-ray without contrast material, done while the patient was in acute abdominal pain showed a liquid-density mass. Clinical profiles were analysed of 18 children with congenital ventricular aneurysm (CVA) and diverticulum (CVD) (nine with CVA and nine with CVD). Of 18 children, only six had any symptoms, consisting of chest discomfort, palpitation, or convulsion. Heart murmurs were heard in nine of the 18 children, and a nonspecific systolic ejection murmur in one. A systolic click was heard in only four children. A Meckel's diverticulum was discovered in 89 patients during routine exploration at the time of an appendicectomy. Subsequent treatment and complications of surgical correction of a Meckel's diverticulum are discussed. A different attitude appears necessary depending on whether the patient is a child or an adult : apart from some particular circumstances, a healthy diverticulum should be removed in children and left untouched in adults.

Small-bowel obstruction in an old woman was the result of a twisted Meckel's diverticulum containing a large neoplasm. As neoplasms comprise only 1.2-1.4% of the complications of the diverticulum, and generally bleed or cause intussusception, we believe that the condition described is not common.

Perforation of a Meckel's diverticulum by a sharp object often presents with subtle nonspecific abdominal findings. A 53-year-old man had features resembling acute appendicitis and right ureteral obstruction. At laparotomy a wooden toothpick was found perforating a Meckel's diverticulum. The diagnosis of complicated Meckel's diverticulum should be considered when there is typical right lower quadrant pain or the patient is known to have ingested a sharp foreign body. The mortality resulting from perforation is low. The authors discuss other complications of Meckel's diverticulum.

Five patients under the age of 21 years are described in whom a Meckel's diverticulum was found in association with angiodysplasia of the caecum or ascending colon. Angiodysplasia has only been observed previously in patients over the age of 35 years and its demonstration in 5 young patients with Meckel's diverticulum points not only to an association between the two disorders, but suggests that angiodysplasia may have a congenital aetiology.

The analysis of 382 observations of the complicated Meckel's diverticulum in children and adults is presented. The clinical course of the diverticulum, ileus resulting from this defect, peptic ulcer of the diverticulum are described. It has been established that in people having the diverticulum there occur other congenital developmental abnormalities which may be used for making the diagnosis of the disease.
