A comparative assessment of the ultrasonic method of investigations and chromoduodenoscopy for diagnosis of patency of the cystic duct has been made on the basis of treatment of 84 patients. Their high informative value is shown. A tactical scheme of management of patients with acute cholecystitis using findings obtained by these methods allowed the amount of forced operations to be decreased and thus postoperative lethality was less.

A total of 160 adult Malawians with epigastric pain for longer than 2 weeks was investigated by endoscopy and serologically for evidence of infection with Helicobacter pylori. The organism was demonstrated histologically and/or by culture in 141 (88%) patients. With histological means and/or culture as the 'gold standard', the histological technique was 100% sensitive while culture was only 81% sensitive. The amino acid sequence (1-21) of glucagon has been shown to have full spasmolytic action in animal studies but no metabolic effect. The motor activity of the sphincter of Oddi was measured during ERCP with manometric recordings of sphincter pressure. Upper gastrointestinal endoscopy and radiologic examination were performed in 101 patients with symptoms strongly suggestive of gastroesophageal reflux (GER) disease. Hiatus hernia (HH) was found in 50 patients diagnosed by radiography or endoscopy, or both, in 22, 19, and 9 patients respectively. Perforation into the retroperitoneal space may occur during duodenoscopic sphincterotomy and can be recognized radiographically. While it is potentially serious, most patients should respond to conservative management provided that adequate biliary drainage is achieved. Five cases encountered during approximately 500 sphincterotomies are presented. Radiographs taken 24 hours after 50 additional procedures showed no evidence of perforation.
