Role of cardiac ATP-regulated potassium channels in differential responses of endocardial and epicardial cells to ischemia.

Epicardial cells are more susceptible to the electrophysiological effects of ischemia than are endocardial cells. To explore the ionic basis for the differential electrophysiological responses to ischemia at the two sites, we used patch-clamp techniques to study the effects of ATP depletion on action potential duration and the ability of ATP-regulated K+ channels in single cells isolated from feline left ventricular endocardial and epicardial surfaces. During ATP depletion by treatment with 1 mM cyanide (CN-), shortening of action potential durations was significantly greater in epicardial cel

Myocardial calcification after orthotopic heart transplantation.

Two cases with a remarkable similarity in their clinical and histopathologic findings are reported. Both cases involved myocardial calcification found in biopsy material obtained after orthotopic heart transplantation. Myocardial calcification after heart transplantation so far has been described in only one case in the medical literature and thus appears to be a rare entity. Its occurrence is associated with a certain constellation of clinical situations, which include repeated episodes of acute rejection, temporary uremia, periods of septicemia, alcoholism, and cyclosporine and/or steroid th

Clinical spectrum and endomyocardial biopsy findings in eosinophilic heart disease.

Fourteen cases of heart disease with hypereosinophilia were analyzed employing conventional cardiologic methods, including echocardiography, cardiac catheterization, and endomyocardial biopsy. The cases were divided into four types: Acute carditic (endocarditis, myocarditis, pericarditis; five cases); ventricular dilation (three cases); restrictive (three cases); electric disturbance (three cases). Biopsy revealed significant changes in all cases. In one case of the ventricular dilation type, endomyocardial fibrosis with myocardial degeneration was seen, and in another case mural thrombus form

Cerebral embolism in Libman-Sacks endocarditis.

Left homonymous hemianopia, hemiparesis, left-body clonic seizures, and progressive deterioration of consciousness complicated the clinical course of a 57-year-old woman with systemic lupus erythematosus (SLE). Autopsy documented the presence of multiple brain infarcts and cerebral arterial emboli that originated from the verrucae of Libman-Sacks endocarditis. Clinicians and pathologists should consider this unusual vascular mechanism of cerebral injury in patients with SLE who develop strokes.

Acute subendocardial myocardial infarction: its detection by Tc-99m stannous pyrophosphate myocardial scintigraphy.

Sixty-two patients hospitalized because of prolonged chest pain and initial electrocardiographic (ECG) changes of ST depression and T-wave inversion suggestive of acute subendocardial myocardial infarction were evaluated to determine the ability of Tc-99m-stannous pyrophosphate mycardial scintigraphy to detect the presence or absence of acute subendocardial myocardial necrosis. Three groups of patients were designated. Group A consisted of eight patients (13%) who developed reduction of R-waves of more than 25% or new Q-waves broader than 0.03 seconds; of these patients with acute transmural m

Carcinoid Heart Disease Presenting as Cardiogenic Shock: A Case Report.

Carcinoid heart disease (CHD) is a complication of carcinoid syndrome that results from chronic exposure of the endocardium to circulating vasoactive substances, particularly serotonin. It leads to progressive right-sided valvular dysfunction and right heart failure. Presentation as cardiogenic shock is uncommon and may delay recognition of the underlying disease. We present the case of a 44-year-old woman with no prior medical history who developed progressive dyspnea, ascites, and lower extremity edema over four months and was admitted with cardiogenic shock. Transthoracic echocardiography r…

Conserved roles of cardiac endothelial ETS factors in ventricular development and disease.

We previously identified ETS1 as a candidate gene in the rare chromosomal deletion disorder, Jacobsen syndrome (JS, OMIM#147791). Here, we p … creased numbers of endocardial cells that failed to contact the myocardium. Genetic ablation of the endocardium in mice also caused a hypoplastic ventricle with loss of the trabecular myocardium. In Drosophila , loss of the ETS1 ortholog pointed ( pnt ) affected specification of cardiac precursors and decreased Notch signaling, a pathway previously implicated …

Biomarkers and genetic determinants of cardiac sarcoidosis: current status, the unmet needs and future perspectives.

Sarcoidosis is a systemic disorder driven by genetic predisposition, environmental exposures, and immune dysregulation, resulting in the formation of noncaseating granulomas across multiple organs. In cardiac sarcoidosis (CS), immune cell infiltration of the myocardium, epicardium, and endocardium may lead to conduction disturbances, ventricular arrhythmias, and heart failure. While overt cardiac involvement was historically considered rare, affecting only 5% of sarcoidosis patients, the wider availability and improved sensitivity of contemporary cardiac imaging have revealed a substantially h…

The intersection of infectious diseases and cardiovascular disease in Africa: A narrative review.

In sub-Saharan Africa (SSA), the cardiovascular system can be significantly impacted by infections and infestations with the heart being a t … vement or a broader systemic illness with sequelae that can affect the pericardium, myocardium, and endocardium, as well as the blood vessels supplying the heart. Some parasitic infections, such as schistosomiasis, can cause pulmonary hypertension. While these infections are prevalent in many African countries, it is difficult to provide an exact estimate of …

Use of AngioVac device in debulking endocarditis vegetations before lead extractions.

BACKGROUND: Systemic infection of cardiac implantable electronic devices (CIEDs) usually manifests as endocarditis, with vegetations involving the lead and cardiac valves. Cardiac implantable electronic device systemic infections involve the transvenous or epicardial portion of the lead in addition to adjacent endocardium. History, blood cultures, and imaging are the cornerstone of diagnosis. The treatment of CIED infections involves intravenous antibiotics, complete removal of the CIED, and reimplantation after clearance of bacteraemia. AngioVac is used for the removal of right atrial masses,…

A multi-scale finite element method for investigating fiber remodeling in hypertrophic cardiomyopathy.

A significant hallmark of hypertrophic cardiomyopathy (HCM) is fiber disarray, which is associated with various cardiac events such as heart … cardiac muscle, significantly higher fiber disarray is observed near the epicardium compared to the endocardium across all perturbed left ventricle (LV) models. This regional difference in fiber disarray, irrespective of perturbation severity, aligns with previous DT-MRI studies, highlighting the role of regional myocardial mechanics in the development of fib …

Signaling pathway alterations in hearts of a porcine model harboring a β-myosin heavy chain (MYH7-R403Q) gene variant.

The disease-causing myosin variant (MYH7-403Q) is linked to hypertrophic cardiomyopathy (HCM). We carried out a research study of signaling pathways in heart samples from control wild-type (WT) GE Yucatán mini-pigs and their littermates harboring the gene variant, MYH7-R403Q. Our approach permits the determination of adverse signaling pathways involved in different regions of a translationally relevant heart without the effects of intervention. We examined the left ventricular free wall (LV), endocardium (EN), and coronary arteries (CA) from 5 transgenic and 5 wild-type mini-pig littermates to…

Cryptococcal Endocarditis in Humans-A Narrative Review.

BACKGROUND: Cryptococcus species constitute opportunistic fungi that seldom cause infections in individuals with competent immune systems. In the rare case of cryptococcal endocarditis, the fungus infiltrates the endocardium. This disease occurs almost exclusively in patients with active immunosuppression, implanted cardiac devices, or prosthetic valves. OBJECTIVES: This study aims to analyze all documented cases of Cryptococcus spp. endocarditis in humans, emphasizing the epidemiology, microbiology, clinical manifestations, therapeutic approaches, and infection outcomes. METHODS: A comprehe…
