The analysis of repeated-measures data on schizophrenic reaction times using mixture models.

Reaction times for schizophrenic individuals in a simple visual tracking experiment can be substantially more variable than for non-schizophrenic individuals. Current psychological theory suggests that at least some of this extra variability arises from an attentional lapse that delays some, but not all, of each schizophrenic's reaction times. Based on this theory, we pursue models in which measurements from non-schizophrenics arise from a normal linear model with a separate mean for each individual, whereas measurements from schizophrenics arise from a mixture of (i) a component analogous to

Effect of optimal control on SI epidemic model.

In this paper, we present an optimal control analysis of a susceptible-infected (SI) epidemic model governed by a single objective function. The main purpose of this study is to minimize the number of infected individuals while reducing the cost associated with the control strategy. A dynamic control variable [Formula: see text] is introduced to represent preventive measures, such as vaccination, awareness, or treatment. The model dynamics are formulated as a system of nonlinear differential equations with bilinear incidence rate. The existence of an optimal control is established using Pontry

Health sector reform in South Asia: new challenges and constraints.

In early 1990s, Jamison, Mosley and others concluded that a profound demographic and consequent epidemiological transition is taking place in developing countries. According to this classical model, by the year 2015, infectious diseases will account for only about 20% of deaths in developing countries as chronic diseases become more pronounced. These impending demographic and epidemiological transitions were to dominate the health sector reform agenda in developing countries. Following an analysis of fertility, mortality and other demographic and epidemiological data from South Asian and other

Narrative review of the development of an ischaemic heart disease prognostic scoring tool (i-IHD score) among patients with type 2 diabetes mellitus in Malaysia.

INTRODUCTION: Ischaemic heart disease (IHD) remains a major cause of mortality among individuals with type 2 diabetes mellitus (T2DM) in Malaysia. Conventional cardiovascular risk models, such as the Framingham risk score, often show limited calibration in Asian populations. Artificial intelligence (Al)-calibrated models have emerged as potential alternatives, yet their generalisability and clinical utility across different populations remain uncertain. This narrative review aimed to summarise existing prognostic models for IHD in patients with T2DM and identify methodological gaps relevant to

Validation of five hepatic steatosis algorithms in metabolic-associated fatty liver disease: A population based study.

BACKGROUND AND AIM: Non-invasive hepatic steatosis algorithms are recommended in detecting metabolic-associated fatty liver disease (MAFLD) in epidemiological studies. However, the diagnostic accuracy of these models is unclear. This study aimed to evaluate the diagnostic efficiency of five common models in a national survey population. METHODS: The Third National Health and Nutrition Examination Survey (NHANES III) datasets were used in this study. The fatty liver index (FLI), hepatic steatosis index (HSI), non-alcoholic liver disease-liver fat score (NAFLD-LFS), the steato text (ST), and vi

Estimating benchmark concentrations and other noncancer endpoints in epidemiology studies.

Methods for evaluating the hazards associated with noncancer responses with epidemiologic data are considered. The methods for noncancer risk assessment have largely been developed for experimental data, and are not always suitable for the more complex structure of epidemiologic data. In epidemiology, the measurement of the response and the exposure is often either continuous or dichotomous. For a continuous noncancer response modeled with multiple regression, a variety of endpoints may be examined: (1) the concentration associated with absolute or relative decrements in response; (2) a thresh

Salmonella epidemiology: A whirlwind of change.

The field of infectious disease epidemiology for Salmonella and other enteric pathogens is undergoing some of the most profound changes since the time of Kauffman and White. Rapid advances in "big data" technologies such as genomics and metagenomics are making it possible to monitor and control salmonellosis in new and exciting ways. Epidemiological methods are becoming increasingly robust through the routine use of standardized hypothesis-generating questionnaires, iterative open-ended interviewing, informational trace-backs and new modeling techniques for describing the attribution of diseas

Machine learning-based meta-analysis reveals gut microbiome alterations associated with Parkinson's disease.

There is strong interest in using the gut microbiome for Parkinson's disease (PD) diagnosis and treatment. However, a consensus on PD-associated microbiome features and a multi-study assessment of their diagnostic value is lacking. Here, we present a machine learning meta-analysis of PD microbiome studies of unprecedented scale (4489 samples). Within most studies, microbiome-based machine learning models accurately classify PD patients (average AUC 71.9%). However, these models are study-specific and do not generalise well across other studies (average AUC 61%). Training models on multiple dat

Can the prevalence of one STI serve as a predictor for another? A mathematical modeling analysis.

We aimed to understand to what extent knowledge of the prevalence of one sexually transmitted infection (STI) can predict the prevalence of another STI, with application for men who have sex with men (MSM). An individual-based simulation model was used to study the concurrent transmission of HIV, HSV-2, chlamydia, gonorrhea, and syphilis in MSM sexual networks. Using the model outputs, 15 multiple linear regression models were conducted for each STI prevalence, treating the prevalence of each as the dependent variable and the prevalences of up to four other STIs as independent variables in var

Fluorescent biomarkers demonstrate prospects for spreadable vaccines to control disease transmission in wild bats.

Vaccines that autonomously transfer among individuals have been proposed as a strategy to control infectious diseases within inaccessible wildlife populations. However, rates of vaccine spread and epidemiological efficacy in real-world systems remain elusive. Here, we investigate whether topical vaccines that transfer among individuals through social contacts can control vampire bat rabies-a medically and economically important zoonosis in Latin America. Field experiments in three Peruvian bat colonies, which used fluorescent biomarkers as a proxy for the bat-to-bat transfer and ingestion of a

The Interdependence between Schistosome Transmission and Protective Immunity.

Mass drug administration (MDA) for control of schistosomiasis is likely to affect transmission dynamics through a combination of passive vaccination and reduction of local transmission intensity. This is indicated in phenomenological models of immunity and the impact of MDA, yet immunity parameters in these models are not validated by empirical data that reflects protective immunity to reinfection. There is significant empirical evidence supporting the role of IgE in acquired protective immunity. This is proposed to be a form of delayed concomitant immunity, driven at least in part by protecti

Soil-Transmitted Helminths: Mathematical Models of Transmission, the Impact of Mass Drug Administration and Transmission Elimination Criteria.

Infections caused by soil-transmitted helminthias (STHs) affect over a billion people worldwide, causing anaemia and having a large social and economic impact through poor educational outcomes. They are identified in the World Health Organization (WHO) 2020 goals for neglected tropical diseases as a target for renewed effort to ameliorate their global public health burden through mass drug administration (MDA) and water and hygiene improvement. In this chapter, we review the underlying biology and epidemiology of the three causative intestinal nematode species that are mostly considered under

Avoidable errors in the modelling of outbreaks of emerging pathogens, with special reference to Ebola.

As an emergent infectious disease outbreak unfolds, public health response is reliant on information on key epidemiological quantities, such as transmission potential and serial interval. Increasingly, transmission models fit to incidence data are used to estimate these parameters and guide policy. Some widely used modelling practices lead to potentially large errors in parameter estimates and, consequently, errors in model-based forecasts. Even more worryingly, in such situations, confidence in parameter estimates and forecasts can itself be far overestimated, leading to the potential for lar

Air pollution exposure assessment methods utilized in epidemiological studies.

The assessment of personal exposure to air pollution is a critical component of epidemiological studies associating air pollution and health effects. This paper critically reviewed 157 studies over 29 years that utilized one of five categories of exposure methods (proximity, air dispersion, hybrid, human inhalation, and biomarkers). Proximity models were found to be a questionable technique as they assume that closer proximity equates to greater exposure. Inhalation models and biomarker estimates were the most effective in assessing personal exposure, but are often cost prohibitive for large s

An Epidemiology-Constrained Framework for Mechanistic Hypothesis Restriction and Prioritization in Complex Disease.

RATIONALE: Many chronic diseases remain mechanistically unresolved despite major advances in molecular biology, systems biology and high-throughput biomedical investigation. Disorders such as autoimmune disease, neurodegeneration and chronic inflammatory syndromes frequently exhibit prolonged latency, developmental susceptibility windows, heterogeneous phenotypes and multiscale biological interaction that limit direct experimental reconstruction of initiating causal events. Under such conditions, causal interpretation may proceed without sufficient inferential constraint, contributing to fragm

Identifying memory mechanisms in Bayesian models of behavioural change during epidemics.

Accurate modelling of epidemic dynamics often requires accounting for how individuals modify their behaviour in response to perceived infection risk. While existing behavioural change epidemic models attempt to capture this feedback, they usually make simple, ad hoc assumptions about how memory affects responses. For example, they mostly focus on recent cases only, thereby overlooking how earlier experiences continue to shape current perceptions of risk. This study introduces a unified framework of Memory Mechanism Enhanced Behavioural Change (MEBC) models within a Bayesian SIR epidemic modell

Neural Network Assisted Estimation for the Structural Nested Accelerated Failure Time Models.

Time-varying confounding complicates the causal survival analysis for longitudinal data. Traditional survival models that adjust for time-dependent covariates fail to estimate the intervention causal effect unbiasedly. The Structural Nested Accelerated Failure Time Model (SNAFTM) can address this challenge effectively. This model estimates the intervention causal effect as the acceleration factor of the survival time while controlling for the time-varying confounders. However, the SNAFTM model usually relies on the G-estimation, which lacks power and suffers from computational burden, especial

Minimizing confounding in comparative observational studies with time-to-event outcomes: An extensive comparison of covariate balancing methods using Monte Carlo simulation.

Observational studies are frequently used in clinical research to estimate the effects of treatments or exposures on outcomes. To reduce the effects of confounding when estimating treatment effects, covariate balancing methods are frequently implemented. This study evaluated, using extensive Monte Carlo simulation, several methods of covariate balancing, and two methods for propensity score estimation, for estimating the average treatment effect on the treated using a hazard ratio from a Cox proportional hazards model. With respect to minimizing bias and maximizing accuracy (as measured by the
