Several transscleral suturing techniques require intricate surgical maneuvers and are associated with an increased risk for surgical trauma. We describe a new scleral fixation method that combines the advantages of temporary haptic externalization and injector implantation of an intraocular lens (IOL). After IOL insertion with the injector, each haptic is externalized through 2 adjacent corneal incisions. A 10-0 polypropylene suture loop is made at each exposed haptic and transscleral IOL fixation performed. This modified technique is a safe and effective way to fixate the IOL in eyes with ins

OBJECTIVE: To compare the mechanical properties and failure modes of a standardized short oblique distal radial metaphyseal osteotomy stabilized using either a transfixation pin cast (TPC), a modular-sidebar external skeletal fixator (ESF), or a solid-sidebar ESF (modular- or solid-ESF, respectively) using static or cyclic axial loading to failure.
STUDY DESIGN: In vitro study.
ANIMALS: Equine cadaver forelimbs.
METHODS: A 30 degrees oblique distal radial osteotomy was created and stabilized using 1 of the 3 fixation methods: (1) TPC, (2) modular-ESF, or (3) solid-ESF. Limbs were tested usi

The management of tibial tuberosity fracture-avulsion after tibial tuberosity transposition can be challenging. Implants must be able to resist the strong distractive force of the quadriceps mechanism, yet implant size is limited by the amount of tuberosity bone stock available. Revision fixation is compromised further by fragmentation of the tubercle. Five stifle joints had temporary transarticular external skeletal fixators applied to manage complications of tibial tuberosity transposition. Fracture reduction was confirmed in four out of five tibial tuberosity fractures. Frames were in place

BACKGROUND: The details regarding bone formation and complication rates associated with tibial lengthening over an intramedullary nail with use of the Ilizarov external fixator have not been reported, to our knowledge. The purpose of this study was to review our experiences with this procedure, performed in individuals with idiopathic short stature. In addition, we compared the results of lengthening over a nail, in terms of the observed bone formation and the complications, with those of the conventional Ilizarov method.
METHODS: Eighty-eight tibiae in forty-four individuals with a mean preo

Open surgical treatment of comminuted tibial pilon fractures is associated with substantial complications in many patients. Indirect reduction and stabilization of fractures by means of distraction using a circular external fixator can be a useful method of achieving satisfactory joint restoration, and employs the principles of closed reduction to realign disrupted bones and joint structures. In this report, we describe the case of a patient with a comminuted tibial pilon fracture treated by means of closed reduction and stabilization using a circular external fixator.

A 72-year-old woman with periprosthetic femoral fracture after cementless total hip arthroplasty (THA) underwent external fixation using the Ilizarov method. Although open reduction and internal fixation with a condylar plate system were initially attempted, deep infection with methicillin-resistant Staphylococcus aureus at the fracture site occurred 2 weeks postoperatively. Six weeks after removal of the plating system, the fracture was stabilized with external fixation using the Ilizarov method and went on to successful fusion at 3 months. To our knowledge, this is the first report in which

PURPOSE: Joint-bridging external fixation is a minimally invasive treatment option for distal radius fractures. Although radial length can be restored easily the anatomic reduction of articular fragments and restoration of the normal volar tilt proves to be more difficult. A method of nonbridging hybrid fixation of distal radius fractures facilitates fracture reduction and allows for free wrist movement.
METHODS: Twenty-five consecutive patients with fractures of the distal radius were treated with nonbridging external fixation for 6 weeks. The stepwise surgical technique comprised a prelimin

PURPOSE: External fixation and open reduction and internal fixation have been the traditional techniques for surgical fixation of unstable distal radius fractures. The existing literature has not identified which is superior, primarily because of the lack of comparative trials. We performed a comprehensive systematic review and meta-analysis of the current literature on external fixation and internal fixation of distal radius fractures to determine the dominant strategy based on available scientific evidence.
METHODS: We searched MEDLINE and EMBASE for English-language articles published betw

PURPOSE: To evaluate the clinical results of post traumatic complications treated by the author's own technique using an AO/ASIF conventional external fixator (without special distraction device).
MATERIALS AND METHODS: There were 70 patients (77 limbs) with an average of 26.8 years (range, 4-54). There were 33 femurs, 43 tibias and one ulna. The following post traumatic complications were treated: 14 limb shortening, 20 nonunion, 28 malunion 14 infected open fractures with bone loss and 1 chronic osteomyelitis. Linear lengthening was performed in 29 limbs, acute de-rotation and subsequent le

We report the outcome of 19 children aged 5.2 to 13.2 years with 20 fractures of the femoral shaft requiring surgery, who were randomly assigned to have external fixation (EF) or flexible intramedullary nailing (FIN) (10 fractures each). The duration of the operation averaged 56 minutes for the EF group with 1.4 minutes of fluoroscopy, compared with 74 minutes and 2.6 minutes, respectively, for the FIN group. The early postoperative course was similar, but the FIN [corrected] group showed much more callus formation. The time to full weight-bearing, full range of movement and return to school w

The pinless external fixator, introduced into clinical practice for open tibial fractures, suggested itself for use as a traction substitute because of its pinless frame. The aim of this feasibility study was to replace the conventional calcaneal pin traction by a joint-bridging pinless fixator, inserted under local anesthesia. 10 patients with 6 malleolar dislocation fractures, 3 pilon tibial and 1 open distal tibial fracture were immobilised by a joint-bridging pinless fixator during 10.4 days (5-16 days) till swelling had subsided and definitive fracture treatment, consisting of plate fixat

The authors successfully used an external fixator in 10 children affected by one of the following pathologies: fracture of the femur with poor consolidation and marked shortening of the limb; fracture in a hypoplastic segment; severe multiple trauma; delay in consolidation of pathologic bone; fracture in bone regeneration. On the basis of the results obtained, it is affirmed that the external fixator is the ideal means of osteosynthesis for use in any of the above-described situations.

Between 1977 and 1982, 17 pediatric patients ranging in age from six to 14 years were treated for injuries that required the application of a total of 20 external fixation devices. The indications for the use of external fixation devices in children include: open fractures with soft-tissue injury, unstable fractures (diaphyseal, epiphyseal, and intra-articular), "polytrauma" patients, and unstable pelvic fractures. All fractures and soft-tissue injuries healed. The usual reported complications of external fixation devices, such as pin tract infections, delayed union, and refracture, were not e

BACKGROUND: Open distal tibial fractures pose significant challenges regarding treatment options and patient outcomes. This retrospective single-centre study aimed to compare the stability, clinical outcomes, complications, and financial implications of two surgical interventions, namely the external locking plate and the combined frame external fixator, to manage open distal tibial fractures.
METHODS: Forty-four patients with distal open tibial (metaphyseal extraarticular) fractures treated between 2020 and 2022 were selected and formed into two main groups, Group A and Group B. Group A (19

BACKGROUND: The present study evaluated the effect of internal mesh fixation and external fixation (inguinal hernia truss) on postoperative complications in patients with inguinal hernia.
MATERIALS AND METHODS: This randomized clinical trial was performed on 64 patients that were candidates for inguinal hernia repair by laparoscopic method following the totally extraperitoneal procedure. These patients were randomly divided into two groups. In the first group, the mesh was fixed with a maximum of three absorbable tacks. In the second group, the inguinal hernia truss was used as the external f
