Hypermucoviscous Klebsiella corneal abscess: Infection from inside out.

BackgroundCorneal ulcers are typically caused by external ("exogenous") factors such as trauma, contact lens use, or preexisting ocular surface diseases. Internal systemic ("endogenous") sources have not been reported as direct causes of corneal ulcers. Here, we report, to our knowledge, the first documented case of a corneoscleral abscess that resulted from an underlying bacteremia.MethodThis is a single case report.ResultA previously healthy 35-year-old male presented to the Emergency Department with left eye redness, pain, and blurred vision. Initial examination showed reduced visual acuity

Clinical Observation of Corneal Endothelial Plaques With Fungal and Bacterial Keratitis by Anterior Segment Optical Coherence Tomography and In Vivo Confocal Microscopy.

PURPOSE: Endothelial plaque is an important sign of fungal keratitis and is related to diagnosis, surgical indications, and prognosis. However, bacterial keratitis sometimes involves fibrin formation on the back corneal surface, similar to endothelial plaques. Because corneal infiltration interferes with precise observation of the posterior corneal plaque, distinguishing pathogens with a slitlamp is difficult. We hope to assist clinicians in early diagnosis and timely treatment by observing the connection state of endothelial plaques and the corneal endothelium through anterior segment optical

Microfiltration of brilliant blue G dye.

BACKGROUND: Brilliant blue G (BBG) is a safe and effective dye used to highlight the internal limiting membrane during macular surgery. The authors proposed that the utilization of a 0.22-μm filter intraoperatively can reduce the risk of inoculating an eye with contaminated BBG. METHODS: An in vitro model of contaminated BBG was prepared. Laboratory stock cultures of 7 organisms including, Staphyloccocus epidermidis, Streptococcus pneumoniae, Staphyloccocus aureus, Haemophilus influenza, Klebsiella pneumoniae, Fusarium species, and Candida albicans, were prepared in five 10-fold dilutions and

Photodynamic therapy (PDT) has been used in ophthalmology for the last 10 years particularly for disorders of the posterior pole. In recent years PDT has been increasingly applied for corneal cross-linking in progressive keratoconus. The classical PDT is performed with porphyrins as photosensitizers and illumination with visible light of 630-635 nm wavelength and cross-linking with the photosensitizer riboflavin and ultraviolet illumination at 370 nm. Illumination of photosensitizers generates free oxygen radicals, which damage the cell membrane or nucleic acids of eukaryotic cells or even mic

Endogenous Fungal Endophthalmitis Following Eyebrow Tattooing: A Case Report.

This case report describes a rare instance of bilateral endogenous fungal endophthalmitis in a 50-year-old healthy female patient, following a facial tattooing procedure. Initially misdiagnosed as iritis in the right eye due to presenting symptoms of blurred vision and ocular pain, the patient's condition worsened following treatment with corticosteroids. Ophthalmic examination revealed severe vitreous opacity in the right eye and a yellowish-white lesion in the inferonasal retina of the left eye. Metagenomic sequencing of the vitreous fluid confirmed infection with Aspergillus fumigatus. The

Infiltrating cells and IFNgamma production in the injected eye after uniocular anterior chamber inoculation of HSV-1.

PURPOSE: After uniocular anterior chamber (AC) inoculation with HSV-1, the anterior segment of the injected eye becomes inflamed and infected; however, virus does not spread from the anterior segment and infect the retina of the injected eye. The purpose of this study was to identify early infiltrating cells and to determine whether infiltrating cells produce interferon (IFN)gamma. METHODS: Euthymic, female, BALB/c mice were injected in one AC with 3 x 10(4) PFU of HSV-1 (KOS) in a volume of 2 microL. Mice from each group were killed at 12, 24, 36, 48, and 72 hours post injection (pi), the ey

Therapeutic lamellar keratectomy in the management of experimental keratomycosis.

Keratomycosis, a vision-threatening corneal infection, is a challenge to the ophthalmologist because it is often refractory to medical treatment. Surgical keratectomy may not only debulk the major infectious source but potentiate the intracorneal penetration of the antifungal agents. To evaluate the efficacy of keratectomy in managing the fungal corneal ulcer, three common keratomycosis models, Candida, Fusarium, and Aspergillus keratitis, were established in 46 eyes of 23 rabbits (each group had 14, 16 and 16 eyes respectively). Within each keratitis group, the infected eyes were randomly div

Endogenous mycotic endophthalmitis in a dog with candiduria and Evans syndrome.

This report describes the clinical presentation, diagnosis, histologic lesions, and outcome of endogenous mycotic endophthalmitis secondary to candiduria in a three-year-old female spayed Dachshund. The dog was being treated for Evans syndrome for one month prior to being diagnosed with candiduria and fibrinous uveitis OS. The left eye was enucleated due to secondary glaucoma, and the fungal urinary tract infection was treated successfully. Uveitis developed in the contralateral eye with relapse of the urinary tract infection in the following weeks. The right eye was medically managed until se

A possible port of entry into the eye of dog during erratic canine heartworm (Dirofilaria immitis) parasitism.

A dog was suspected of suffering from ectopic Dirofilaria immitis infection, because a large white nematode worm was detected in the anterior chamber of the left eye. A cylinder-shaped fibrin sac in the anterior chamber was found in the eye of the dog by slit lamp microscopy. After successful surgical removal of the worm, the corneal wound produced by the keratotomy healed in a short period. The worm was estimated to be extremely young, 5th-stage-immature male D. immitis, equivalent to a 90-120-day-old worm postinfection, by close morphological measurement and an experimental infection study.
