Optimizing imaging in the pediatric trauma patient, part 1: head and neck trauma.

Trauma is the leading cause of death in the pediatric population and is among the most common reasons for ED visits by children. Imaging is an important tool for the diagnosis and management of pediatric trauma, but there are risks associated with exposure to ionizing radiation. In pediatric head and neck injuries, clinical findings and clinical decision tools can help inform selection of the most appropriate imaging modalities for the trauma patient, while also reducing unnecessary radiation exposure. This supplement reviews evidence-based recommendations for imaging decisions and interpretat

Facial fractures.

Fractures of the facial skeleton can result in the loss of an aesthetically pleasing appearance and basic function, and many cases subsequently require an operative intervention. The surgeon managing these facial fractures must, at the same time, be cognizant of concomitant injuries, including neurologic, ophthalmologic, and cervical spine issues. For most situations, early stabilization in anatomical position using rigid fixation will give the most accurate reduction for the optimal return of preoperative appearance and function, while reducing long-term soft-tissue contracture.

An unusual complication of condylectomy: fracture of the temporal bone and intratemporal facial paralysis.

The incidence of temporomandibular joint (TMJ) ankylosis appears to be decreasing with the increased socioeconomic status of society. The intraoperative complications were reported to be few, the most dangerous of which are facial nerve injury during the extended preauricular incision and maxillary artery injury during condylectomy. The authors report a case of fracture of the temporal bone tearing the intratemporal portion of the facial nerve, resulting in a total facial nerve paralysis.

Heads Up Play: Acute Assessment and Management of Basketball-Related Craniofacial Injuries by On-Court Personnel.

INTRODUCTION: With a following of over 825 million people, basketball currently ranks amongst the world's most popular sports. Despite a number of concussion awareness and player safety protocols that have been implemented at the professional level of organized play, a standardized, layperson-friendly algorithm for the acute management of basketball-related craniofacial injuries does not appear to presently exist. METHODS: This 10-year retrospective cohort study was conducted using the National Electronic Injury Surveillance System database to examine basketball-related craniofacial injuries …

Pearls of nasoorbitoethmoid trauma management.

Nasoorbitoethmoid fractures account for ∼5% of adult and 15% of pediatric facial fractures. The appropriate management of these injuries requires an understanding of the anatomic features of the region, the classification of injury severity, assessment, and treatment methods. The purpose of this article is to provide a general overview of the topic, with a more specific focus on the pearls of managing these fractures. Prompt and proper management of these injuries can achieve both adequate functional and aesthetic outcomes.

Orofacial manifestations of child maltreatment: A review.

The recognition and reporting of child abuse and neglect continues to be a challenge in the healthcare setting. All healthcare providers, including dentists, should be aware of the high prevalence of orofacial injuries and conditions that are concerning for abuse and neglect. Sentinel injuries may appear trivial and do not require medical attention, but are unlikely caused by accidental mechanisms and, if not properly identified, often precede more severe abusive injuries. Concerning orofacial findings can manifest as bruising, eye injuries, intraoral injuries, pharyngeal perforations, facial …

Dental Implants in Rehabilitation of Patients with Facial Trauma: a Review of Most Current Practices.

Facial trauma can cause complex hard and soft tissue defects that significantly affect function, appearance, and quality of life. Dental implants are a useful tool that can offer dramatic improvement in reconstructive options for patients who have sustained orofacial trauma and are key in rehabilitating these injuries, helping restore chewing, speech, and facial esthetics. Successful outcomes depend on careful timing, consideration of tissue integrity, and a multidisciplinary approach, including surgical optimization including bone and soft tissue grafting, orthodontics, and prosthodontics.

[Microsurgical reconstruction after traumatic lesion of the peripheral facial nerve].

In this case report, a 17-year-old female underwent microsurgical exploration and neurorrhaphy after a glass pane accident resulting in traumatic lesion of the right temporal facial nerve branch. Nine months after reconstruction she had regained function of the frontalis muscle. When patients with traumatic facial nerve injuries are admitted to hospital, it is crucial to consult with a microsurgical center to ensure that reconstruction is completed within 24-72 hours from the time of accident to regain mimetic function.

Car seat palsy.

Soft tissue trauma causing peripheral paralysis of the facial nerve is a rarely described event in children. The author describes such a case where a boy developed paralysis of the buccal division of the facial nerve in the absence of any bony injury, through collision with the cushioned side of a child seat during a road traffic accident. He had no other injuries. This reinforces the importance of thorough clinical examination in the detection of subtle neurological abnormalities in young children.

Maxillofacial fractures associated with electric scooter trauma: a cohort study from a national level I trauma center.

PURPOSE: Electric scooters (e-scooters) have become a widely used mode of urban transportation, but their increasing popularity has been accompanied by a rise in trauma-related emergency department visits. Craniofacial injuries represent a particularly relevant subset due to the exposed riding position and the limited use of protective equipment. This study analyzed the epidemiological characteristics, fracture patterns, associated injuries, and treatment requirements of patients presenting with facial fractures after e-scooter accidents. METHODS: A retrospective observational study was condu…

Concussion in facial trauma patients: a retrospective analysis of 100 patients from a UK major trauma centre.

Concussion is a common and potentially debilitating condition. Research has shown that one-third of patients admitted with facial trauma have concurrent concussion. This study aimed to investigate the burden and management of concussion in patients presenting with acute facial trauma, and to identify potential risk factors within this population. A retrospective observational study was conducted at a UK major trauma centre between 1 January 2019 and 1 February2020. One hundred randomly selected patients who attended the acute clinic responsible for managing facial trauma were identified. No pa

Retrofacial approach of cochlear implantation in inner ear malformation with aberrant facial nerve: a case report.

Cochlear implantation is regarded as a safe surgery for young children with minimal complications. However, inner ear malformations and aberrant course of facial nerves may impede electrode insertion via the round window approach and increase the risk of iatrogenic facial nerve injury. We report a case of cochlear incomplete partition in a patient with anomalous facial nerve anatomy. The anterior and inferior displacement of the facial nerve obscured the round window. A retrofacial approach was used to expose the round window and the electrode was inserted successfully. No surgical complicatio

Extratemporal facial nerve paralysis after blunt trauma.

Presented are two patients in whom facial palsy developed after blunt trauma to the side of the face. Both patients made a complete and uneventful recovery. The etiology of a facial nerve lesion can be established through knowledge of the possible sites of injury to the facial nerve, good history taking and physical examination, together with appropriate tests. The prognosis of an injury secondary to blunt trauma is generally good.

Ear trauma.

Understanding basic ear anatomy and function allows an examiner to quickly and accurately identify at-risk structures in patients with head and ear trauma. External ear trauma (ie, hematoma or laceration) should be promptly treated with appropriate injury-specific techniques. Tympanic membrane injuries have multiple mechanisms and can often be conservatively treated. Temporal bone fractures are a common cause of ear trauma and can be life threatening. Facial nerve injuries and hearing loss can occur in ear trauma.
