Out-of-pocket healthcare payments on chronic conditions impoverish urban poor in Bangalore, India.

BACKGROUND: The burden of chronic conditions is on the rise in India, necessitating long-term support from healthcare services. Healthcare, in India, is primarily financed through out-of-pocket payments by households. Considering scarce evidence available from India, our study investigates whether and how out-of-pocket payments for outpatient care affect individuals with chronic conditions. METHODS: A large census covering 9299 households was conducted in Bangalore, India. Of these, 3202 households that reported presence of chronic condition were further analysed. Data was collected using a s

Alcohol sales and risk of serious assault.

BACKGROUND: Alcohol is a contributing cause of unintentional injuries, such as motor vehicle crashes. Prior research on the association between alcohol use and violent injury was limited to survey-based data, and the inclusion of cases from a single trauma centre, without adequate controls. Beyond these limitations was the inability of prior researchers to comprehensively capture most alcohol sales. In Ontario, most alcohol is sold through retail outlets run by the provincial government, and hospitals are financed under a provincial health care system. We assessed the risk of being hospitalize

Adolescents' use of an online Q&A service to enhance their oral health literacy: a mixed-methods text mining and thematic analysis.

BACKGROUND: Health literacy is fundamental to healthy behaviour, defined as seeking, processing, and evaluating health information to make informed decisions. Access to health information has historically been linked to socioeconomic status (SES), with disparities in health persisting due to unequal access to new health technologies. Digital technologies offer opportunities for spreading health information but may also increase inequalities through a digital divide. The Norwegian government established the website ung.no in 2003 to provide health information to adolescents. This study aims to

Analysis of bilateral development assistance for surgery in low- and middle-income countries: trends, challenges and opportunities.

BACKGROUND: Surgical care in low- and middle-income countries (LMICs) remains underfunded despite representing a significant portion of the Global Burden of Disease. Understanding bilateral development assistance for surgery is crucial to address this disparity. METHODS: We conducted a narrative review and quantitative analysis of bilateral development assistance for surgery in LMICs from 2014 to 2022 using data from the Institute for Health Metrics and Evaluation and the Organisation for Economic Co-operation and Development Creditor Reporting System. Surgical projects were identified throug

Global challenges and perspectives in acute and emergency general surgery in low and middle-income countries: a scoping review.

BACKGROUND: Acute and emergency general surgery in low and middle-income countries (LMICs) faces critical disparities. Despite global and regional initiatives, surgical inequities persist due to fragile healthcare infrastructure, workforce shortages and insufficient policy integration. This study explores the systemic challenges and perspectives in acute and emergency surgical care in LMICs while evaluating global efforts to bridge these gaps. METHODS: We searched seven databases from inception to 26 November 2024, with an updated search through 12 June 2025. Records were screened in duplicat

Designing an Indicator‑Driven, Value‑Based Architecture for Pneumonia Prevention in Japan: A Formative Policy Viewpoint on Adult Vaccination and Oral Care.

Japan's aging society concentrates pneumonia burden across communities, long‑term care, perioperative pathways, and hospitals. Adult pneumococcal and influenza vaccination are supported by trials and meta‑analyses and are often cost‑effective; yet, realized value depends on targeting, measurement, financing, and pairing with bedside prevention such as oral care and hospital‑acquired pneumonia bundles. This viewpoint proposes a formative, indicator‑driven architecture linking cost‑effectiveness to operations by aligning vaccination with complementary oral‑care prevention and value‑based payment

RIGHTS principles for early intervention: right care at the right time for every infant, child and family in low and middle-income countries.

BACKGROUND: Early intervention programmes for infants and young children at developmental risk are implemented inconsistently and inequitably worldwide. Variation in access, surveillance, referral, follow-up and service quality means that children and families may receive very different opportunities depending on where they live, system capacity and resources, laws, policy and local pathways. Practical, evidence-informed guidance is needed to translate 'what should be done' into 'how to implement it' across diverse settings. METHODS: This conceptual, practice-oriented article used a narrative

Health-system challenges, resilience strategies, and actionable priorities for non-communicable disease care in humanitarian crises: A scoping review.

Humanitarian crises disrupt the continuous care required for non-communicable diseases (NCDs), yet evidence on effective health-system responses remains fragmented. We conducted a scoping review, reported in accordance with PRISMA-ScR, to map health-system challenges, implemented and recommended strategies, resilience functions, and evidence gaps for cardiovascular diseases, diabetes, cancer, and chronic respiratory diseases in humanitarian settings. PubMed/MEDLINE, Scopus, Web of Science, relevant organizational websites, and reference lists were searched. English-language empirical studies,

Barriers and facilitators to implementing an integrated electronic health records system to improve tuberculosis preventive treatment among people living with HIV: a content analysis study from Georgia.

INTRODUCTION: The integration of tuberculosis (TB) preventive treatment (TPT) into HIV services through electronic health records (EHR) can improve outcomes by enhancing care coordination, reducing redundancies, and supporting data-driven decision-making. In Georgia, despite close collaboration between TB and HIV programs, service delivery and data systems remain siloed, forcing patients to navigate between facilities and limiting the effectiveness of TPT among people living with HIV (PWH). METHODS: We conducted a qualitative study guided by the Consolidated Framework for Implementation Resea

Academic partnerships for family medicine and primary health care education: a global narrative review 7 years after Astana.

The 2018 Declaration of Astana reaffirmed Primary Health Care (PHC) as the foundation for universal health coverage and the health-related Sustainable Development Goals, yet the workforce, curricula, and research infrastructure required to deliver that vision remain unevenly developed across regions. This narrative review, prepared in line with the SANRA reporting standard, examines the global field of academic partnerships for family medicine (FM) and PHC education during the 7 years that followed Astana. We synthesize peer-reviewed and gray literature from 2010 to April 2026 across PubMed, S

Cost-Effectiveness Analysis of Universal Rotavirus Vaccination Schedules in Syria.

Background: Rotavirus (RV) continues to be the leading cause of acute gastroenteritis (AGE) globally among children under five. National RV vaccination efforts have lowered morbidity and mortality. Vaccination is a key public health tool to alleviate this substantial burden of RV in middle- and low-income countries. In Syria, RV morbidity accounts for 27% of severe GE. We conducted a cost-effectiveness analysis of introducing rotavirus vaccinations (RVV) into Syria's National Immunization Program. Methods: A decision tree model was developed to assess the cost-saving of two-dose rotavirus vacc

Slovakia: Health System Review.

This analysis of the Slovak health system reviews developments in governance, organization, financing and delivery of care, health reforms and health system performance. Slovakia, a central European country with a population of 5.4 million, continues to face significant health and health care system challenges. Slovakia's health system is founded on universal coverage with compulsory health insurance, a broad benefits package and a competitive insurance model. Although life expectancy improved between 2000 and 2019, the COVID-19 pandemic reversed gains, and in 2023 Slovak life expectancy remai

Capacity Planning (Capital, Staff and Costs) of Inpatient Maternity Services: Pitfalls for the Unwary.

This study investigates the process of planning for future inpatient resources (beds, staff and costs) for maternity (pregnancy and childbirth) services. The process of planning is approached from a patient-centered philosophy; hence, how do we discharge a suitably rested healthy mother who is fully capable of caring for the newborn baby back into the community? This demonstrates some of the difficulties in predicting future births and investigates trends in the average length of stay. While it is relatively easy to document longer-term (past) trends in births and the conditions relating to pr

Health and tuberculosis systems resilience, the role of the private sector and pandemic preparedness: insights from a cross-country qualitative study with policy-makers in India, Indonesia and Nigeria.

INTRODUCTION: The COVID-19 pandemic was an unprecedented challenge to health systems worldwide and had a severe impact on tuberculosis (TB) case notifications and service delivery. India, Indonesia and Nigeria are high TB-burden countries where the majority of initial care-seeking happens in the private health sector. The objectives of this study were to (1) explore policy-makers' perspectives on the impact of the COVID-19 pandemic on private sector TB service delivery in India, Indonesia and Nigeria and (2) identify cross-cutting insights for pandemic preparedness with respect to TB service d

Spain: Health System Review.

This review of the Spanish health system analyses recent developments in health organization and governance, financing, health care provision, recent reforms and health system performance. Overall health status continues to improve in Spain, which presents the highest life expectancy in the European Union - although some socioeconomic inequalities in health persist and risk factors such as overweight, tobacco and alcohol consumption and illegal drug use remain a concern. The Spanish national health system (SNS) provides universal coverage, and it is mainly funded by taxes. Health competences a

Private sector engagement for immunisation programmes: a pragmatic scoping review of 25 years of evidence on good practice in low-income and middle-income countries.

INTRODUCTION: Many National Immunisation Programmes attempt to leverage the private sector ; however, there is limited consolidated and synthesised documentation on good practices, gaps and lessons learnt. A 2017 WHO guidance document recommended best practices for private sector engagement (PSE) in immunisation. We conducted a pragmatic scoping review to identify gaps, update and consolidate evidence on promising practices in PSE for vaccination. METHODS: Building on two previous reviews published in 2011 and 2017, we conducted a pragmatic scoping review of peer-reviewed publications from lo

Investment case for primary health care in low- and middle-income countries: A case study of Kenya.

BACKGROUND: Primary healthcare (PHC) systems attain improved health outcomes and fairness and are affordable. However, the proportion of PHC spending to Total Current Health Expenditure in Kenya reduced from 63.4% in 2016/17 to 53.9% in 2020/21 while external funding reduced from 28.3% (Ksh 69.4 billion) to 23.9% (Ksh 68.2 billion) over the same period. This reduction in PHC spending negatively affects PHC performance and the overall health system goals. METHODS: We conducted a cost-benefit analysis and computed costs against the economic benefits of a PHC scale-up. Activity-Based Costing (AB

Perceptions and drivers of healthcare provider and drug dispenser practices for the treatment of malaria in pregnancy in the context of multiple first-line therapies in western Kenya: a qualitative study.

BACKGROUND: Emergence of Plasmodium falciparum resistance to artemether-lumefantrine in Africa prompted the pilot introduction of multiple first-line therapies (MFT) against malaria in Kenya, potentially exposing women-of-childbearing-age (WOCBAs) to anti-malarials with unknown safety profiles in the first trimester. This qualitative study explored knowledge and perceptions among healthcare providers providing malaria treatment to WOCBAs and pregnant women.
