[Hand-foot syndrome (palmar-plantar erythrodysesthesia].

The hand-foot syndrome (HFS) is an erythematous skin lesion of the palma and planta of the hand and feet is most often caused by cytostatic chemotherapy. The impact on the patients' quality of life depends on the extent of the disease. The pathogenesis of the hand-foot syndrome has not yet been sufficiently clarified and it can only be treated symptomatically. We performed an extensive literature search in Medline to evaluate the current state of knowledge concerning the hand-foot syndrome and conclude with practical advice to physicians treating patients with hand-foot syndrome.

[Conservative treatment of neurogenic foot deformities].

The lack of muscle control in neurological disorders often leads to deformities of the foot. The equinus (a contracture of the triceps surae muscle), the abduction and clubfoot deformity are the most common ones. The present paper describes the deformities and stresses the functional relevance for the patient. Therapeutic principles of the conservative orthopaedic means are described with special emphasis on the need of an adequate height of the aide and the foot position. Casting and the application of Botulinun toxine are described having a major side effect of muscle weakening. A combinatio…

Is prophylactic diabetic foot surgery dangerous?

Any treatment rendered to the deformed, insensate foot should be undertaken with the prime intentions of reducing the potential for future limb-threatening events and allowing the patient to continue as an ambulatory, productive member of society. The purpose of this article is to compare morbidity and outcomes of elective foot surgery among diabetics and nondiabetics with isolated toe deformities. We compared the prevalence of infection, wound complication, and recurrence of ulcers in 31 diabetics and 33 nondiabetics. All of these patients received a single proximal interphalangeal joint arth…

[Natural course of juvenile foot deformities].

The spectrum of normal variations of children's feet is extremely broad and often difficult to separate from pathological conditions. Especially the flexible flatfoot and the pes adductus normally disappear during growth, and even if they do persist up to adult life, they hardly have any pathological significance. It therefore appears proper to see foot deformities of children from the prognosis point of view, that is, to differentiate between a benign, pain-free course of development with no functional restriction even under load, and pathological deformities which systematically require cons…

[Skew foot--etiology, clinical appearance, management].

Thirty feet in 18 children diagnosed as skew foot (Z-foot, serpentine foot) have been assessed. Following types of deformity have been distinguished: 1. idiopathic congenital (21 feet), 2. congenital, associated with other defects or systemic disease (8 feet), 3. acquired (iatrogenic) after manipulations for club foot treatment (1 foot) and 4. neurogenic, not represented in this study. In the first group minor forefoot adduction that remained after cast treatment was generally accepted by the parents; 7 feet required surgical release. Eight feet were corrected operatively in group 2. In most o…

Posterolateral release for resistant club foot.

We treated 37 infants with 53 idiopathic club feet by posterolateral release alone at a mean age of 2.4 months. They were reviewed after a mean follow-up of 10 years 7 months. Both function and appearance were studied. Seventeen feet had required further surgery, at an average of four years after posterolateral release. In all cases hindfoot equinus had been well corrected; the mean ankle dorsiflexion at review was 15 degrees. Most feet showed subtalar joint movement of between 50% and 75% of normal. Four feet showed poor results: one had a stiff subtalar joint, two feet in one child showed fi…

[Trash foot following abdominal aortic surgery].

Peripheral embolism from dislodgement of intraarterial debris can convert successful surgery of the abdominal aorta into tragedy. An embolic shower from such surgery, often containing cholesterol, can result in forefoot or toe ischemia referred to as "trash foot." The incidence of this early complication is reported as 7-29%, but is believed to be even higher. Prevention and treatment includes heparin, embolectomy, lumbar sympathectomy, aspirin, rheomacrodex and prostaglandins. 3 such patients, 2 men aged 62 and 69, respectively, and a woman of 72, are reported. In 2 of the cases below-knee am…

Congenital flat foot: different clinical forms.

Using standard lateral x rays of 32 congenital flat feet (CFF) treated in our institutions, we studied the alignment in the sagittal plane of the tarsal and metatarsal bones. All feet presented with a complete dislocation of the talonavicular joint and a vertical talus. In some cases, however, the alignment of the calcaneocuboid joint was abnormal. In these, both the talus and the calcaneus appeared verticalized with no apparent subtalar joint subluxation (type I). Other cases, by contrast, had a normally aligned calcaneus thus implying subluxation of the subtalar joint (type II). Between the

Diabetic foot amputations. Part III: Midfoot.

Recently, the approach in management of diabetic foot disorders has undergone critical re-evaluation. Major foot infections, such as plantar space infections and osteomyelitis do not automatically result in Syme's or below-the-knee amputations. When indicated, midfoot amputations should be attempted because of the functional and psychologic advantages compared with more proximal amputation levels of the lower extremity. In Part III of this series of articles concerning diabetic foot amputations, the authors present case studies along with midfoot amputations and muscle-balancing procedures.

Foot deformity in hereditary onycho-osteodysplasia.

A 30-year-old woman with hereditary onycho-osteodysplasia was examined. In addition to the classic tetrad of fingernail and toenail dysplasia, patellar aplasia, iliac horns, and radial head hypoplasia and dislocation, she also had scoliosis, proteinuria, and distinctive bilateral foot anomalies. The foot deformity consisted of a ball-and-socket ankle joint, valgus ankle, forefoot supination, and lateral subluxation at the tarsal-metatarsal joints. The literature showed that the radiographic findings of foot deformity in this dysplasia have never been reported in detail before. Evidence suggest…

Giant dermoid associated with foot duplication.

After resection at birth of an accessory left foot, a white male infant was followed up for a slowly enlarging posterior calf mass of the same leg. When the patient was 8 years old, the mass was thought to have enlarged sufficiently to warrant excisional biopsy. Histologic examination of the tumor was consistent with the diagnosis of a dermoid cyst, revealing a lesion lined with stratified squamous epithelium and containing cutaneous adnexal structures represented by eccrine sweat glands. A congenital dermoid in the extremities is unusual. The prognosis following removal is good, and recurrenc…

Biped gait stabilization via foot placement.

It is shown that stable biped gaits can be achieved by discrete foot placement based on feedback of information available at the time of foot placement. The model, developed by Townsend (1981, J. Biomechanics 14, p. 727) to evaluate the coordinations of torso motions, subsumes most of the salient body members and motions. The modeling yielded a generalized inverted pendulum with a movable support point which physically defines lateral foot placement. The principal result is that stable gaits can be defined by foot placements which are a linear function of the system center of mass position and…

Sural nerve neurocutaneous cross-foot flap.

Managing heel ulcers has proved frustrating because of the tendency for recurrence despite satisfactory use of grafts and flaps. This predisposition for continual breakdown is invariably associated with the loss of sensation. A neurocutaneous cross-foot flap was used to provide coverage and sensation to a heel after a degloving injury. Initially a full-thickness skin graft was used but resulted in a chronic, infected ulcer. Local tissue was not available for coverage. The neurocutaneous flap was based over the distribution of the sural nerve of the opposite foot. After proper delay, the flap w…

Mid-foot disassociation in rheumatoid arthritis.

The mid-foot is commonly involved in rheumatoid arthritis with subchondral sclerosis and erosions at the talonavicular joint. There may be a mid-foot disassociation whereby the hindfoot is usually subluxed. Occasionally the forefoot may be subluxed with mid-foot disassociation. In long-standing cases of rheumatoid arthritis there is a loss of the normal plantar arch.

Cross-foot flap for ankle coverage.

The usual cross-leg flap procedures to cover composite wounds involving exposed ankle joint or bone require awkward positioning of the extremities with resultant complications. This is especially true in young, muscular patients with thick legs. A dorsalis pedis flap from the uninjured foot used as a cross-foot flap has been reliable and successful in our experience. A representative case history and the multiple advantages of this technique are described. We believe that this flap is an excellent alternative to the free-flap technique for coverage around the ankle area, especially in hospital…
