Biopsies of the fibrous capsule in 31 women undergoing explanation of gel-filled breast prostheses and in 3 women with silicone gel-associated granulomas were tested for the presence of IgG, IgM, IgA, C3 complement, and fibrin using fluorescent antisera. Of a total of 41 prostheses removed, 9 were found to be ruptured but contained within the fibrous capsule or immediately adjacent to it. Bone remodeling resulting from implant insertion has been attributed to changes in the bone's strain state. Since remodeling takes several months, it was this study's purpose to develop a long-term in vivo strain sensor. Porous surfaced metal tabs were attached to a standard strain gauge. A retrospective study was carried out in search of silicone deposits in 11 periodic hemodialysis patients (1 liver biopsy, 4 splenectomies and 6 autopsies) treated between 12 and 60 months with equipment containing a silicone pump segment. Silicone particles were found in the macrophage cells of all patients, especially in the liver and spleen, but also in lymph nodes, bone marrow and even an artery wall. BACKGROUND: The Military Health System recognizes the importance of analyzing "foreign bodies" removed from US service members through several policy documents. This activity focuses on detecting potentially toxic metals. Intra-ocular "foreign bodies" (IOFBs) represent a small, clinically important subset. The development of ocular metallosis with iron and copper fragments is a specific local reaction to IOFBs. The results of the compositional analysis of removed IOFBs can influence clinical management decisions aimed at optimizing the preservation of sight.
