The use of bone grafts and substitutes in the treatment of distal radius fractures.

The interest in developing biomaterials to augment fracture healing continues to grow. New products promise early return to function with minimal morbidity; however, indications to use these products remain unclear. An ideal bone graft material stimulates bone healing and provides structural stability while being biocompatible, bioresorbable, easy to use, and cost-effective. This article reviews the biology of bone grafts and the clinical evidence in the use of bone graft substitutes for the treatment of distal radius fractures.

Bone metabolism and fracture risk in type 2 diabetes mellitus.

Osteoporosis and type 2 diabetes mellitus (T2DM), both prevalent in aging and westernized societies, adversely affect the health of elderly people by causing fractures and vascular complications, respectively. Recent experimental and clinical studies show that the disorders are etiologically related through the actions of osteocalcin and adiponectin. Meta-analyses of multiple clinical studies show that the hip fracture risk of T2DM patients is increased 1.4-1.7-fold compared with non-DM controls, even though the patients' bone mineral density (BMD) is not diminished. Vertebral fracture risk of

A child with bone fractures and dysmorphic features: remember of pycnodysostosis and craniosynostosis.

Accidental bony injuries are common in children. Children may also present with bony injuries following non-accidental injuries. Pathological fractures, though extremely rare, are an important entity and constitute fractures that occur in abnormal bones, usually after minor trauma. Pycnodysostosis is a rare skeletal dysplasia characterised by a clinical phenotype that includes short stature, skull deformities, osteosclerosis, acroosteolysis and bone fragility. Often the disease is diagnosed at an early age as a result of the investigation of short stature. However, the diagnosis is sometimes d

Comparison of ultrasonography with computed tomography in the diagnosis of nasal bone fractures.

OBJECTIVES: The aim of this study is to compare ultrasonography with CT in the diagnosis of nasal bone fractures. METHODS: 40 patients (9 female and 31 male) with mid-facial fractures, which were suspected nasal bone fractures, were included. All of the patients had mid-facial CT images. Ultrasonography with a 7.5 MHz transducer (Aloka 3500, Tokyo, Japan) was used to evaluate the nasal bone fractures. All of the sonograms were compared with CT findings for sensitivity, specificity and predictive values. A χ(2) test was applied to the data to assess statistical significance. RESULTS: CT diagn

Abnormal bone remodeling in patients with spontaneous painful vertebral fracture.

The application of tetracycline-based iliac bone histomorphometry to the study of the pathogenesis of osteoporosis has given conflicting results. Accordingly, we performed this procedure in 78 postmenopausal white women with one or more vertebral fractures identified according to rigorous criteria that excluded other causes of vertebral deformity and 66 healthy postmenopausal white women recruited from the same geographic region; the groups did not differ in age or weight. In each subject, measurements were made separately on the cancellous (Cn), endocortical (Ec), and intracortical (Ct) subdi

[Surgical treatment of patients with fractures of distal metaepiphysis of the radial bone].

Traumatologic emergency to patients with primary unstable, comminuted, intraarticular fractures of distal metaepiphysis of the radial bone (DMRB) should be provided in steady-state conditions, using reliable methods of reposition and retention of the fixed bone fragments. The strategy of treatments of fractures of DMRB first consisting in closed reposition and in surgery in case of failure or secondary displacement is thought to be outdated. In patients with symptoms of unstable fractures active surgical strategy should be used before complications appear.

[Modern approach to the rehabilitation of children with fractured long tubular bones].

An optimized therapeutic rehabilitation program involved 393 patients with fractured long bones (humeral, radial/ulnar, femoral, crural) admitted to the Orthopedic Department of the Stavropol Regional Children's Clinical Hospital and the Rehabilitation Centre for Handicapped Children and Adolescents. Rehabilitation was performed in three consecutive stages, viz. immobilization, functional recovery and training. The proposed approach ensured rather fast functional recovery of the affected extremity even in cases with compound fractures. The rehabilitation program included polyenzyme therapy wit

Evaluation of a single miniplate use in treatment of zygomatic bone fracture.

Different methods of internal fixation of simple displaced zygomatic fracture, such as wiring, miniplate, and screw fixation, were compared for postreduction rotational stability caused by muscular forces. This study was performed to evaluate the minimally invasive therapy in cases of zygomatic bone fracture. Seventeen patients were treated by percutaneous hook reduction and miniplate fixation along the frontozygomatic suture. Postoperatively, repositioning of aesthetic and stability and also bone ends approximation were assessed clinically and radiologically. The patients were followed up for

Development and validation of a predictive bone fracture risk model for astronauts.

There are still many unknowns in the physiological response of human beings to space, but compelling evidence indicates that accelerated bone loss will be a consequence of long-duration spaceflight. Lacking phenomenological data on fracture risk in space, we have developed a predictive tool based on biomechanical and bone loading models at any gravitational level of interest. The tool is a statistical model that forecasts fracture risk, bounds the associated uncertainties, and performs sensitivity analysis. In this paper, we focused on events that represent severe consequences for an explorati

Hyoid bone fracture identified only with nasal Valsalva manoeuvre.

OBJECTIVE: We present two cases of a hyoid bone fracture identified through careful clinical examination with a Valsalva manoeuvre during nasendoscopy. METHOD: Case reports and review of the literature, with emphasis on technique during nasendoscopy. RESULTS: The first patient had sustained a blow to the neck with a stick, six months prior to presentation with a globus sensation. External examination and standard nasendoscopy were unremarkable. The second patient had been struck across the neck by a wire whilst riding a motorbike at low speed. Endoscopy revealed swelling of the supraglottis.

Bone health and fracture rate in individuals with neurofibromatosis 1 (NF1).

BACKGROUND: Patients with neurofibromatosis 1 (NF1) are shorter than expected and often have low bone mineral density (BMD), but the pathogenesis of these bony problems is poorly understood. METHODS: We performed an exploratory study of BMD, 18 laboratory measures of bone metabolism, and fracture history in 72 adult NF1 patients. RESULTS: Eight of the 18 clinical biochemical measures of bone health had at least 10% of NF1 patients outside the standard reference range. Serum 25-hydroxy-vitamin D concentrations were low in 56% of the NF1 patients, serum parathyroid hormone (PTH) concentrations

[Bone mineral density and fracture risk in patients with ankylosing spondylitis].

Ankylosing spondylitis (AS) is an inflammatory disease primarily affecting the spine. Bone mineral density (BMD) is reduced in AS, particularly in male patients. Syndesmophyte formation may cause falsely increased BMD of the spine. The risk of spinal compression fractures may be 7-fold increased compared with healthy individuals. An increased risk of peripheral fractures has not been described. Structural spinal changes in advanced AS predispose to unstable vertebral fractures. Antiresorptive treatment is recommended for osteoporotic patients with AS, but the effect on BMD has never been studi

SRS cancellous bone cement augmentation of calcaneal fracture fixation.

BACKGROUND: The proper treatment of intraarticular calcaneal fractures remains controversial. The goal of operative treatment is to achieve anatomic reduction with stable internal fixation. One method for stabilizing fracture constructs is the use of calcium phosphate bone cement. The goal of this study was to evaluate a series of patients who had open reduction and internal fixation of intra-articular calcaneal fractures with calcium phosphate bone cement augmentation. METHODS: Fifteen patients had open reduction and internal fixation through an extensile lateral approach with standard low-p

Intramedullary bone cementing for the treatment of Colles fracture in elderly patients.

Eighteen patients with Colles fractures, mean age 70 years (range 55-91), were treated by intramedullary bone cementing. The best indication for this technique was an unstable extra-articular Colles fracture with osteoporosis. Bone cement was packed into the canal that was made by curettage of intramedullary cancellous bone from the dorsal fracture site. Intramedullary cementing caused little bleeding from the medullary canal and no irritation of the extensor tendons. Because of rigid fixation, patients could use the affected hand for light activities without any external orthosis the day afte

New concepts for bone fracture treatment and the Locking Compression Plate.

The operative treatment of bone fractures using plates and screws is a standard successful technique. Internal fixation with plates and screws leads to additional trauma and disturbance of the bone blood supply, which increases the risk of delayed union and infection. However, problems also are encountered in the fixation of osteoporotic bone. The locked internal fixator technique is an approach to optimize internal fixation. It aims at flexible elastic fixation to imitate spontaneous healing, including induction of callus formation. The technology supports what is currently called "minimally

Quantitative ultrasound: use in the detection of fractures and in the assessment of bone composition.

Quantitative ultrasound (QUS) techniques have found widespread clinical use, but their specific role in clinical practice needs further refinement. This review discusses the ability of QUS approaches to predict the risk for prevalent vertebral fractures and the risk for future fractures. QUS approaches perform as well as central dual x-ray absorptiometry devices but with some disadvantages (at least for older QUS approaches) with regard to the predictive power for hip fractures. Technologic diversity of QUS approaches may lead to differences in performance. QUS also has the potential for asses

Orthopedic treatment of metastatic bone fracture from hepatocellular carcinoma.

We report on a 73-year-old female patient with a small hepatocellular carcinoma, which was detected by ultrasonography in September 1993. She was treated by ethanol injection and repeated courses of transcatheter arterial embolization for local recurrence. Five years after the initial treatment of the primary hepatocellular carcinoma in 1998, a solitary metastatic tumor from hepatocellular carcinoma was detected in the middle of the left humerus by fluorine-18-fluoro-2-deoxy-D glucose positron emission tomography and technetium-99m methylene diphosphonate bone scintigraphy. About one year late
