The prognostic value and reproducibility of the radiological features of the fractured scaphoid.

We investigated whether the radiological features of the fractured scaphoid could be reproducibly measured and used to predict the likelihood of union with conservative plaster cast immobilization. We found that the inter- and intra-observer reproducibility of the Compson, Herbert and Russe classification systems were only fair and that none predicted fracture union. Assessments of fracture level, comminution and displacement showed moderate inter- and intra-observer reproducibility but did not predict the likelihood of fracture union. We conclude that the radiological features of acute scapho

Transparency films: intraoperative templating to prevent limb deformity.

Operative management of fractures and malunions can be challenging when restoring native anatomy is not straightforward. Comminuted fractures and managing deformity correction in the setting of osteolysis, callus, and even complete fracture healing must include careful planning. Preoperative planning has been popularized and taught as an integral part of a surgeon's skill set, with critical evaluation and assessment of the implemented plan being the final step in the process. We present a robust, reproducible, and cost-effective technique for intraoperative fracture fixation assessment with ca

Pediatric odontoid fracture causing Brown-Sequard syndrome: a case report.

An odontoid fracture leading to Brown-Sequard syndrome (BSS) is an uncommon condition with only seven cases reported to date. To our knowledge, there are no reports of occurrence in paediatric patients. We report a paediatric odontoid fracture leading to Brown-Sequard syndrome (BSS) with satisfactory recovery following surgical treatment. Odontoid fractures may lead to Brown-Sequard syndrome in cases of high-energy trauma in paediatric patients. Direct anterior screw fixation may be a treatment of choice in paediatric odontoid fractures of the comminuted, displaced type when occurring in conju

[Post-traumatic ptosis].

Traumatic ptosis is most often secondary to a fasciomuscular lesion of the levator system. Less frequently, it results from the paralysis of the levator muscle due to an injury to the third pair of cranial nerves. This ptosis is observed in comminuted orbital fractures or fractures of the base of the skull involving the orbital apex, and it is then associated with oculomotor paralysis. In favorable cases seen early, repairing surgery of the levator system produces excellent results. When associated with ophthalmoplegia or corneal anesthesia, neurogenic ptosis requires a watch and wait policy.

Preventing iatrogenic injury from inadvertent intracranial migration of a urinary foley catheter while controlling profuse epistaxis after severe craniofacial trauma.

The massive nasopharyngeal bleeding that may accompany complicated comminuted fractures of the craniofacial bones can be controlled by pressure tamponade using an inflatable urinary Foley catheter. However, inadvertent intracranial catheter penetration poses a serious risk in such situations. Management of a relevant case is described, and a simple preventive measure is suggested.

[Treatment of upper metaphyso-epiphyseal fractures of the tibia by Ilizarov's method. Report of 4 cases].

The authors report four cases of comminuted metaphyso-epiphyseal fractures of the proximal end of the tibia treated by the Ilizarov method. Three of the cases were polytraumatisms, the fourth one had a severely contused skin. Due to this method early rehabilitation was made possible and treatment of associated lesions was made easy.

Distal ulna fractures.

In isolation, distal ulna fractures are rare. They are often found in conjunction with distal radius fractures, and the complexity of the interaction of the distal ulna with the radioulnar joint and triangular fibrocartilage complex makes understanding and treatment of distal ulna fractures challenging. Fixation of distal ulna fractures can be problematic owing to comminution making reduction challenging. A thin soft tissue can lead to hardware prominence and necessitate implant removal. In this Current Concepts article, we review the anatomy, pathology, and treatment of distal ulna fractures

One-point fixation of tripod fractures of zygoma through a lateral brow incision.

The aims of this report were to analyze the cases of lateral brow incision and 1-point fixation and to introduce the criteria for application of this selective approach.Among 70 patients with tripod fractures, 14 patients (20%) underwent 1-point fixation technique through lateral brow incisions. Preoperative and postoperative displacements of the infraorbital rim were radiologically measured.Of these patients, 7 cases (50%) were type III, 6 cases (43%) were type IV, and 1 case (7%) was type V according to the Knight and North classification. Simple fracture of the infraorbital rim was seen in

Treatment of complex proximal tibia fractures with the less invasive skeletal stabilization system.

OBJECTIVE: Proximal tibia fractures with metaphyseal comminution present a difficult treatment challenge. The Less Invasive Skeletal Stabilization (LISS) internal fixator system has theoretical advantages (minimally invasive fixed angle construct) for the treatment of these injuries. This report presents clinical results of the LISS system for treatment of complex proximal tibia fractures and illustrates the unique properties of the system. DESIGN: Prospective clinical trial. SETTING: Level I trauma center. PATIENTS: Twenty-eight consecutive patients with comminuted proximal tibia metaphyse

Operative treatment of volar intra-articular fractures of the distal end of the radius.

We retrospectively reviewed the results of operative treatment of forty-nine volar marginal intra-articular fractures of the distal end of the radius. According to the Comprehensive Classification of Fractures, there were two B3.1 fractures (characterized by a small volar fragment, with the sigmoid notch intact), three B3.2 fractures (characterized by a large volar fragment that included the sigmoid notch), and forty-four B3.3 fractures (characterized by comminution of the volar fragment). Although all fractures healed and only nine patients had evidence of osteoarthrosis on follow-up radiogra

Preexisting lesions associated with complete diaphyseal fractures of the third metacarpal bone in 12 Thoroughbred racehorses.

We characterized features of complete diaphyseal fractures of third metacarpal bones in Thoroughbred racehorses. Given that stress fractures are known to occur in the third metacarpal bone, an additional aim was to determine if complete fractures are associated with signs of a preexisting incomplete stress fracture. Bilateral metacarpi from 12 Thoroughbred racehorses euthanized because of complete unilateral metacarpal diaphyseal fracture were examined visually and radiographically. Open, comminuted, transverse or short oblique fractures occurred in the middle of the diaphysis or supracondylar

Diplopia following midfacial fractures.

Over a period of 2 years, 363 patients who had sustained a total of 438 midfacial fractures due to blunt trauma received a full ophthalmological examination within 1 week of injury. Of these, 72 patients (19.8%) developed diplopia. Diplopia was most common following road traffic accidents (31%) and least common with simple falls (10%). Blow-out fractures of the orbit led to double vision in 58% of cases. Eighty two percent of patients recovered from diplopia within 6 months of injury; only 1 patient required squint surgery for double vision. The principal risk factors for diplopia comprise roa

Fixation of metacarpal and metatarsal fractures in greyhounds.

Over a 6-year period a total of 23 greyhounds and 10 dogs of other breeds were presented with metacarpal and metatarsal fractures. While in other breeds trauma was the usual cause, in greyhounds the injuries were stress or fatigue fractures. In the greyhounds the fractures occurred mainly in young dogs and were often comminuted. Fifteen cases involved the fifth metacarpal bone of the left foreleg. The fractures were managed by either external immobilisation (8 cases) or internal fixation, mainly with lag screws (14 cases). Of 16 dogs which recovered sufficiently to be trialled, 6 were retired

Open Reduction and Internal Fixation of Femoral Head Fracture After Firearm Injury: A Case Report.

CASE: A 21-year-old man presented with a femoral head (FH) fracture from a firearm injury. The FH had a comminuted, minimally displaced fracture with intertrochanteric extension and a retained bullet. This patient was treated with surgical hip dislocation (SHD) and open reduction internal fixation (ORIF). This case describes the operative management of a fracture pattern of the FH not covered within the Pipkin classification. The patient was noted to be progressing well at 1-year follow-up. CONCLUSION: FH fractures from firearm injuries may present with unique fracture patterns. SHD and ORIF

Repair of supracondylar femur fracture and unilateral knee replacement at the same surgery.

In patients who are candidates for a total knee arthroplasty and suffer a periarticular fracture of the femur, the arthroplasty may be performed after bony union of the fracture or immediately, in conjunction with the fracture repair. Herein we present the case of a sixty-year-old female with rheumatoid arthritis and a supracondylar fracture of the right femur in whom total knee arthroplasty and retrograde nail insertion were addressed at one surgery; the outcome was favorable. The transverse extraarticular fracture pattern in this patient was advantageous for simultaneous procedures; had the

Severe osteoarthrosis after fracture of the mandibular condyle: a clinical and histologic study of seven patients.

The factors that induce complications subsequent to fractures of the mandibular condyle have long been controversial. A clinical and histologic study of seven patients (nine temporomandibular joints [TMJs]) who suffered from severe TMJ problems subsequent to condylar fractures showed that the complications were related to comminution of the condyle, sagittal fracture of the condyle, and subcondylar fracture dislocation. Alterations in these TMJs all involved secondary osteoarthrosis. Additionally, aseptic necrosis, bifid deformity of the condyle (two of the three sagittally fractured condyles)

The functional outcome of direct lateral approach for fixation of proximal humeral fractures: A case series from a tertiary care hospital.

Displaced proximal humeral fractures warrant surgical fixation for early rehabilitation and better functional outcome. These fractures are traditionally fixed by delto pectoral surgical approach. Direct lateral approach has recently gained interest as it involves less soft tissue dissection and is particularly helpful in certain fracture patterns. However, there have been concerns of axillary nerve damage with this approach. We report a case series of proximal humerus fractures fixed by direct lateral approach from our institution. All displaced Type 2 and 3 fractures were included in our stud
