Potassium permanganate ingestion as a suicide attempt.

Potassium permanganate is a highly corrosive, water-soluble oxidizing antiseptic. A 68-year-old female patient was admitted to our Emergency Department after ingestion of 3 tablets of 250 mg potassium permanganate as a suicide attempt. The physical exam revealed brown stained lesions in the oropharynx. Emergency endoscopy was performed by the gastroenterologist after the third hour of ingestion. Emergency endoscopy revealed multiple superficial (Grade I-II) lesions on the esophagus and cardia, which were considered secondary to the caustic substance. The mainstay in the treatment of potassium

Pediatric trichotillomania: clinical presentation, treatment, and implications for nursing professionals.

Trichotillomania (TTM), or compulsive hair pulling, is a disorder that typically onsets in childhood. It is mistaken to believe that children will "age out" of this behavior, as pediatric TTM often has a chronic, debilitating course that does not remit without treatment, resulting in considerable psychological and physical impairment. Because most children with TTM will be seen initially by nursing professionals in the practices of dermatologists, pediatricians, gastroenterologists, and other disciplines, raising nurses' awareness of this disorder is of the utmost importance for accurate nursi…

[Diagnosis of intestinal Behçet's disease].

Due to similar manifestations of intestinal Behçet's disease (BD) to those of other colitis such as Crohn's disease or intestinal tuberculosis, it is still challenging for gastroenterologist to accurately diagnose intestinal BD in patients with ileo-colonic ulcers. Moreover, no reliable diagnostic criteria for intestinal BD have been developed yet. Therefore, IBD Study Group of KASID was formulated to establish the guideline for the diagnosis of intestinal BD using a modified Delphi process. The novel diagnostic criteria for intestinal BD were developed based on two aspects; colonoscopic findi

Balancing drug risk and benefit: toward refining the process of FDA decisions affecting patient care.

Several high-profile drug withdrawals for safety issues have brought into focus the FDA's process for approving drugs and monitoring adverse experiences with those agents after marketing has begun. Gastroenterologists and their patients have been affected adversely by removal from the marketplace of two licensed agents for irritable bowel syndrome (IBS): alosetron and tegaserod. The criteria used by the FDA for assessment of the risks and benefits of drugs used for functional bowel problems seem to be different than those used for the treatment of other conditions and have resulted in drastic …

Intra-abdominal abscess caused by toothpick injury.

We present the case of a 42-year-old female who presented to our emergency department (ED) complaining of epigastric pain for four days. She had been seen in the outpatient department and ED previously for evaluation, but continued to experience epigastric pain with fever. Emergency panendoscopy was performed and a toothpick was discovered impacted in the duodenal bulb. The gastroenterologist was unable to remove the toothpick endoscopically. Computed tomography of the abdomen revealed a long and straight hyperdense foreign body, and intra-abdominal abscess formation. An emergency laparotomy w

A case of severe hemorrhagic diarrhea.

A 50-year-old white female presented to the emergency department with diarrhea, abdominal pain, and hematochezia. The illness started four days prior to presentation and escalated to 10-15 bowel movements daily. She was tachycardic and hypotensive upon presentation with a diffusely tender abdomen. Her anemia and persistent hematochezia prompted endoscopy by a gastroenterologist. Flexible sigmoidoscopy was performed to the splenic flexure and revealed evidence of severe active colitis. The mucosa was friable and severely ulcerated. Numerous biopsies were obtained that revealed acute colitis wit

Approach to the patient who has suspected acute bacterial cholangitis.

Table 4 gives summary recommendations concerning the major decisions that are related to the diagnosis and management of suspected acute bacterial cholangitis. All of these decisions have to be made within the context of disease severity, degree of diagnostic uncertainty, and associated comorbidity. Although these recommendations are based on evidence, there are few randomized controlled trials. Antibiotics that cover gram negatives and anaerobes, along with fluid and electrolyte correction, frequently stabilize the patient. Imaging studies frequently confirm the diagnosis and identify the loc

Treatment of Fistulizing Crohn's Disease in Children.

Patients with Crohn's disease are at risk for developing both internal and external fistulae. These can be asymptomatic incidental radiologic findings or causes of incontinence, chronic pain, abscesses, and sepsis. They can have a devastating impact on quality of life. Careful prospective studies of therapy are few in adult medicine and entirely lacking in the pediatric age group. Assessment and management require a coordinated effort between gastroenterologist, radiologist, and surgeon. Principles of management include surgical drainage of infection combined with medical therapy. Only inflixi

[Secondary high-grade MALT lymphoma of the stomach in a 69-year-old patient with gastrocolic fistula].

A 69-year-old man was referred to our department with an exorbitant foetor ex ore, dysphagia and dyspepsia. Upper endoscopy had been performed prior by an outpatient gastroenterologist and the patient had received an eradication therapy for a Helicobacter pylori-induced gastritis. At admission upper endoscopy showed a gastric ulcer which drained a stinking fluid. Endosonography, computed tomography and an upper gastrointestinal series with water soluble media revealed a gastrocolic fistula. Multiple biopsies showed a low-grade gastric MALT lymphoma. Therefore, a surgical reconstruction with Ro

The relationship between gastro-oesophageal reflux disease and dental erosion.

It is well known that acid regurgitated from the stomach into the mouth will erode teeth. Conditions such as anorexia and bulimia nervosa, c … l and requires treatment and is termed gastro-oesophageal reflux disease (GORD). It is felt by many gastroenterologists that GORD is a failure of the anti-reflux mechanism, which is predominantly controlled by the lower oesophageal sphincter (LOS). Regurgitation is the reflux of gastric juice through the upper oesophageal sphincter and into the oral cavity. Once the …

Fatal iatrogenic iodine toxicity in a nine-week old infant.

BACKGROUND: Povidone-iodine has been used since the 1950s for various labelled uses as a topical antiseptic. The toxicity of an excessive dose in internal use is described in this case report. CASE REPORT: A 9-week old infant was treated for colic by a pediatric gastroenterologist with loperamide and the elimination of nonhuman milk. Without improvement he was hospitalized and given an enema of 50 mL of povidone-iodine diluted in 250 mL of a bowel irrigant. The enema was promptly expelled and 50 mL of the described solution was given hourly for three doses by nasogastric tube. The infant was

[A case of hypereosinophilic syndrome followed by bronchial asthma, intraventricular thrombus, and duodenitis].

We report a 43-year-old male with hypereosinophilic syndrome. The patient has had eosinophilia since the age of 25. He developed bronchial asthma at 41 years of age. In the following year, he consulted a gastroenterologist because of fever and abdominal discomfort. Upper gastrointestinal study showed duodenitis. At the age of 43 years, there was slight fever, watery diarrhea, and eosinophilia. Examination in our department disclosed hypereosinophilic syndrome associated with duodenitis, endomyocarditis with an intraventricular thrombus, and bronchial asthma caused by house dust. Both prednisol

[Reflux esophagitis. Diagnosis and treatment].

Refluxoesophagitis was the topic of a meeting of gastroenterologists in Bergen this year for the purpose of working out consensus guidelines on the diagnosis and treatment of this common disease. To day upper gastrointestinal endoscopy is the most important diagnostic procedure. There was a general agreement on using Berstad's staging scheme, which comprises only three stages of oesophagitis. Twenty-four hour pH-monitoring may provide useful information on the reflux pattern, but the practical consequences of the examination are still uncertain. H2-receptor antagonists are indicated in mild ca…

Differential Diagnosis Between Perianal Crohn's Disease and Hidradenitis Suppurativa: A Challenging Teamwork.

Hidradenitis suppurativa (HS) is a rare, debilitating skin disease characterized by the presence of recurrent tender subcutaneous nodules that develop into abscesses and fistulae. Isolated perineal Crohn's disease (CD) is unusual, diagnosis can be difficult, and distinction from HS is a challenge for the gastroenterologist. The aim of this work was to determine the criteria that distinguish perineal CD from perineal HS. Four patients with isolated perineal CD and three with perineal HS were included. Rectal or skin biopsies of all CD patients showed granulomas. No granulomas were found for HS.

Cytapheresis for pyoderma gangrenosum associated with inflammatory bowel disease: A review of current status.

Pyoderma gangrenosum (PG) is a neutrophilic dermatosis clinically characterized by the presence of painful skin ulcerations with erythematous. As it is frequently associated with inflammatory bowel diseases, including ulcerative colitis, gastroenterologists should be familiar with the disease including therapeutic options. Therefore, we have conducted a review focusing on the cytapheresis for PG in cases of inflammatory bowel diseases. A literature search was conducted to extract studies published in the last 20 years, with information on demographics, clinical symptoms, treatment, and the cli…

[Oral manifestations of Crohn's disease].

Gastrointestinal symptoms are predominant in Crohn's disease. Oral manifestations may also occur. The prevalence of oral manifestations varies between 0.5% and 37%. The manifestations may coincide with or precede gastrointestinal symptoms, and can be subdivided into specific and non-specific lesions. In most patients, lesions are asymptomatic but some patients experience serious discomfort. Oral manifestations can be classified as specific lesions, such as diffuse lip and buccal swelling and cobblestones, and non-specific lesions, such as aphthous ulcers, pyostomatitis vegetans, caries, gingiv
