Cost Comparison of Kirschner Wire Versus Intramedullary Screw Fixation of Metacarpal and Phalangeal Fractures.

BACKGROUND: Intramedullary screw fixation is a relatively new technique for fixation of metacarpal and phalangeal fractures. The objective of this study was to compare health care-associated costs and outcomes for intramedullary screw versus Kirschner wire (K-wire) fixation of hand fractures. METHODS: A retrospective review of patients undergoing intramedullary screw fixation of hand fractures at a single center during 2016-2019 inclusive was conducted. Health care-associated costs were compared with age-matched and fracture pattern-matched controls who underwent K-wire fixation. RESULTS: Fi

Primary lymphoma of bone of the little finger: a case report and review of the literature.

Primary lymphoma of bone (PLB) is a rare, malignant lymphoid proliferation within bone accounting for less than 3% of all malignant bone tumors. In this case report, a 61-year-old female with past medical history of gout presented with pain and swelling in her right little finger. Initial radiographs demonstrated periostitis and soft tissue swelling about the right little finger. She returned three months later with progressive pain. Subsequent MRI and repeat radiographs demonstrated near complete destruction of the right little finger middle phalanx and periostitis with marrow infiltration at

Targeted disruption of Shp2 in chondrocytes leads to metachondromatosis with multiple cartilaginous protrusions.

Metachondromatosis is a benign bone disease predominantly observed in the hands and feet of children or young adults demonstrating two different manifestations: a cartilage-capped bony outgrowth on the surface of the bone called exostosis and ectopic cartilaginous nodules inside the bone called enchondroma. Recently, it has been reported that loss-of-function mutations of the SHP2 gene, which encodes the SHP2 protein tyrosine phosphatase, are associated with metachondromatosis. The purpose of this study was to investigate the role of SHP2 in postnatal cartilage development, which is largely un

Imaging of primary bone tumors in veterinary medicine: which differences?

Veterinary medicine is most often a mysterious world for the human doctors. However, animals are important for human medicine thanks to the numerous biological similarities. Primary bone tumors are not uncommon in veterinary medicine and especially in small domestic animals as dogs and cats. As in human medicine, osteosarcoma is the most common one and especially in the long bones extremities. In the malignant bone tumor family, chondrosarcoma, fibrosarcoma and hemangiosarcoma are following. Benign bone tumors as osteoma, osteochondroma and bone cysts do exist but are rare and of little clinic

[Fractures of the hamate bone and metacarpal bone: a rare combination].

BACKGROUND: Fractures of the hamate bone are rare, but are being seen more frequently due to the increasing popularity of racket sports and golf. CASE DESCRIPTION: A 43-year-old man presented at our emergency department with a swollen, painful left hand after hitting a concrete wall with his fist. X-rays and CT scans revealed comminuted fracture of the hamate bone and the base of the fourth metacarpal bone. Open reduction and internal fixation, using screws and Kirschner wires, were performed. The postoperative course was uneventful. At one-year follow-up the patient had regained full use of

Bone mineralization in the affected extremities of children with spastic hemiplegia.

To assess the impact of neurologic involvement on bone mineralization, dual-energy X-ray absorptiometry was used to quantitate bone mineral content (BMC), bone mineral density (BMD), fat and lean muscle mass in the limbs of 19 children with spastic hemiplegic cerebral palsy. The BMC in the affected limb was on average 26.5% lower (upper limbs) and 15.6% lower (lower limbs) than in the corresponding uninvolved limb. Lean muscle mass was reduced by 15% and BMD by 6% in the involved limbs and did not differ significantly between upper and lower limbs. The fat content of involved and uninvolved li

Fundamental ratios and logarithmic periodicity in human limb bones.

Fundamental mathematical relationships are widespread in biology yet there is little information on this topic with regard to human limb bone lengths and none related to human limb bone volumes. Forty-six sets of ipsilateral upper and lower limb long bones and third digit short bones were imaged by computed tomography. Maximum bone lengths were measured manually and individual bone volumes calculated from computed tomography images using a stereologic method. Length ratios of femur : tibia and humerus : ulna were remarkably similar (1.21 and 1.22, respectively) and varied little (<7%) between

[Use of the vascularized upper tibial crest at the wrist and hand].

The choice of a technique for reconstruction of bone loss to the lower end of the forearm is often difficult, especially in cases of nonunion. Vascularized bone is essential in these indications. We present cases of post-traumatic hand and wrist reconstruction with upper tibial corticocancellous transfer. It is a reliable reconstructive procedure for forearm bone loss to cover limited size bone defects, leaving no complication to the donor site.

Conservatively Managed, Displaced Scaphoid Fracture in a Young, Female Collegiate Basketball Player.

Fractures of the scaphoid bone occur in 60-70% of wrist bone fractures. It most often occurs after a fall onto outstretched hand injury. Many times, the diagnosis of a scaphoid fracture is missed due to the unique anatomy and positioning of this carpal bone. Treatment options vary depending on the type of fracture and degree of displacement. We present the case of a 19-year-old female patient. She reported to the urgent care facility after a fall on an outstretched hand while playing basketball. She was diagnosed with a wrist sprain. After having continued pain she sought care at the sports me

Recurrent aneurysmal bone cyst of the proximal phalanx treated with cryosurgery: a case report.

A recurrent aneurysmal bone cyst of the proximal phalanx treated with curettage, cryosurgery, and bone grafting is presented. There is no evidence of recurrence after 5 years. Although cryosurgery is commonly used as an adjuvant for intralesional treatment for aneurysmal bone cyst, there have been no reports of its use for the treatment of a lesion arising in the hand.

A modified position for optimized skeletal maturity assessment of AIS patients and its impact on 3D spinal and pelvic parameters.

PURPOSE: A hands-on-wall (HOW) position for low-dose stereoradiography of adolescent idiopathic scoliosis (AIS) patients would allow for skeletal maturity assessment of the hand and wrist. Our aims were twofold: confirm the reliability and validity of skeletal maturity assessment using the HOW radiographs and compare the spinal and pelvic 3D parameters to those of standard hands-on-cheeks (HOC) stereoradiographs. METHODS: Seventy AIS patients underwent two successive stereoradiographs and a standard hand and wrist radiograph on the same day. Patients were randomly assigned to begin with HOW a

Family history correlates of digit ratio abnormalities in schizophrenia.

BACKGROUND: The differences in digit ratio are proposed to arise due to differential effects of sex steroids on the growth of finger bones. In this study, we sought to examine the sex differences and the influence of family history of psychosis on digit ratio in patients with schizophrenia compared to matched healthy controls (HCs). MATERIALS AND METHODS: Digit ratio (2D: 4D) was examined for a large sample of schizophrenia patients (n=200) and HC (n=177) to evaluate the potential effects of family history. RESULTS: The right hand 2D: 4D digit ratio was lesser in schizophrenia patients compa

Fibrous osteodystrophy in two Northern Royal albatross chicks (Diomedea sanfordi).

CASE HISTORY: In February 2004, two Northern Royal albatross chicks aged 20 and 25 days old were presented for necropsy. Both chicks had been hand-fed in situ at a breeding colony, from 2-3 days post-hatch. The hand-rearing diet consisted of boneless hoki fillets (Macraronus novaezelandiae), electrolytes, and sooty shearwater (Puffinus griseus) proventricular oil obtained as a by-product of cultural harvest. PATHOLOGICAL FINDINGS: Routine necropsies on the affected chicks revealed many bones were soft and easily bent. Radiography and histopathology revealed decreased bone density, pathologica

Imaging Traumatic Wrist and Hand Injuries in Children.

The pediatric wrist and hand are anatomically and functionally complex regions that undergo continuous changes throughout growth and maturation. This review opens with a discussion of normal skeletal development, emphasizing typical patterns of ossification and growth plate maturation in the distal forearm, carpus, and phalanges. Physeal anatomy is explained, followed by a review of acute physeal injuries (such as Salter-Harris fractures), and common nonphyseal fractures including buckle, incomplete, and complete fractures. Finally, repetitive stress injuries affecting the distal radius of gym

The effects of Scaphoid and Colles casts on hand function.

Although necessary for bone healing, immobilisation temporarily prevents hand function and may necessitate corrective physiotherapy later. Scaphoid and Colles casts are both commonly used to immobilize scaphoid fractures. Non-union rates are comparable with both casts. The Scaphoid cast incorporates the thumb, whereas the Colles cast leaves the thumb free. We compared the effect of the two casts on hand function in 20 healthy right-hand-dominant volunteers using the Jebsen-Taylor Hand Function Test. Data were obtained through a mixed between and within subject design. Both casts prolonged the

Digital lengthening in congenital hand deformities.

12 hands with congenital short finger in 11 patients were treated with various types of metacarpal bone lengthening. These included three patients with brachydactyly, seven with transverse deficiency, and one with constriction ring syndrome. All cases involved metacarpal lengthening and surgery was performed 16 times in 15 digits. Single-stage lengthening was performed in seven cases, on-top plasty in three cases, and distraction lengthening in six cases. The length gained ranged from 2 to 10 mm in single-stage lengthening, 3 to 17 mm in on-top plasty, and 12 to 30 mm in distraction lengthenin

Sports stress syndromes of the hand and wrist.

Many injuries to the hand and wrist may be seen in the practice of sports medicine. Most of these injuries are well known and the details of their diagnosis and treatment are well documented. However, even among this group, certain injuries frequently escape detection or are not monitored carefully enough to detect their frequent residual complications. These injuries are particular sources of problems. Some little known diagnoses are seldom considered, but any diagnosis, even a presumptive one, is better than applying vague, descriptive, but nondefinitive terms to an athlete's injury. Althoug

Cross-Cultural Teachings from Cremation Practices: Atmaram for the Departed, Shiva Linga for the Living, and Odontoid Fracture for the Neurosurgeons!

Atmaram bone (C2 axis vertebra) is usually handed over to the family of the deceased on the next day after cremation during the ''Asthi sanchaya '' commemoration. ''Asthi visarajan'' involves the practice of immersing the bones and ashes of the deceased in the Holy Ganges river as per Hindu beliefs. Atmaram bone, which usually does not burn during cremation, is handed over to the family of the departed (asthi sanchaya) after cremation which is then immersed in the holy Ganges river ( asthi visarajan). Atma means soul, Ram means Lord and Atmaram combined means the one who is Lord of his own sou
