Vesicular hand dermatitis.

Vesicular hand dermatitis is a type of eczema involving the fingers, hands, and sometimes the feet. It is common in primary care, requiring prompt treatment to prevent chronicity. The cause is often unknown, making the condition challenging to treat. Management consists of avoiding irritants and using emollients and topical corticosteroids.

Atypical Herpetic Whitlow: A Diagnosis to Consider.

CONTEXT: Herpetic whitlow is caused by herpes virus (type1 or 2) during primary infection or as result of autoinoculation. Commonly, it is caused by HSV-2 in adults with positive history for genital infection. Laboratory exams including PCR on blood and cutaneous swab allowed to diagnosis it as a rare case of herpetic whitlow. CONCLUSION: The case here reported demon

Vesicular Hand Dermatitis in a Child: Allergy to Methylisothiazolinone.

A 7-year-old girl presented with a history of vesicular hand dermatitis that only partially improved with the use of steroids. Patch testing revealed a 1+ reaction to 0.2% aqueous methylisothiazolinone (MI) and 0.01% aqueous methylchloroisothazolinone/methylisothiazolinone (MCI/MI). Dyshidrosiform hand dermatitis is rare in children younger than 10 years of age, and this pattern has not been reported with MCI/MI allergy.

Syndrome in question: antisynthetase syndrome (anti-PL-7).

Antisynthetase syndrome is a rare autoimmune disease characterized by interstitial lung disease and/or inflammatory myositis, with positive antisynthetase antibodies (anti-Jo-1, anti-PL-7, anti-PL-12, ZO, OJ, anti-KE or KS). Other symptoms described include: non-erosive arthritis, fever, Raynaud's phenomenon, and "mechanic's hands." The first therapeutic option is corticotherapy, followed by other immunosuppressants. The prognosis of the disease is quite limited when compared to other inflammatory myopathies with negative antisynthetase antibodies.

Safety of alitretinoin for severe refractory chronic hand eczema: Clinical studies and postmarketing surveillance.

BACKGROUND: Alitretinoin is approved for the treatment of adults with severe chronic hand eczema (CHE) refractory to potent topical steroids. In the 6 years since launch, approximately 250 000 patients have been treated with alitretinoin. OBJECTIVE: To compare the postmarketing safety surveillance experience of alitretinoin with data from clinical trials and key safety issues with other retinoids. METHODS: An integrated safety analysis of the pivotal studies of alitretinoin and postmarketing adverse event (AE) reports received since approval for alitretinoin were analyzed. RESULTS: In the p

A case of cutaneous protothecosis in a polyarteritis nodosa patient and review of cases reported in Japan.

A 76-year-old woman farmer with a 20-year history of oral steroid treatment for polyarteritis nodosa, noted a rash consisting of numerous small papules on the dorsum of her right hand in February 2007. The rash worsened over time, and in April 2008 red papules and pustules as big as grains of rice fused to form plaques extending up her arm from the dorsum of the right hand to the extensor surface of the forearm. The plaques were accompanied by erosions, ulcers, and crusts. Skin biopsy specimens from the dorsum of the right hand showed a dense cell infiltration in the upper to middle layer of t

[Palmar hyperhidrosis in childhood: thoracoscopic sympathectomy].

BACKGROUND: Excessive hand sweating, known as palmar hyperhidrosis, is a disabling disorder that starts during the childhood, and can have a strong negative impact on the quality of life of affected children, as they feel rejected by others due to their permanently damp hands, suffering during school activities, contact sports and daily games. MATERIAL AND METHODS: Thoracoscopic sympathicolysis is easy to perform and has proven to be safe, with a short operating time. A review was performed on 48 children who underwent surgical treatment of palmar hyperhidrosis and answered a follow-up questi
