Initial experience with stylet-driven versus lumenless lead delivery systems for left bundle branch area pacing.

INTRODUCTION: Left bundle branch area pacing (LBBP) has emerged as an alternative method for conduction system pacing. While initial experience with delivery systems for stylet-driven and lumenless lead implantation for LBBP has been described, data comparing outcomes of stylet-driven versus lumenless lead implantation for LBBP are limited. In this study, we compare success rates and outcomes of LBBP with stylet-driven versus lumenless lead delivery systems. METHODS: Eighty-three consecutive patients (mean age 74.1 ± 11.2 years; 56 [68%] male) undergoing attempted LBBP at a single institution

Whole-heart electro-mechanical optimization: an integrative systems approach to modern cardiac pacing.

INTRODUCTION: Modern-day cardiac pacing is no longer limited to the treatment of bradycardia but can be tailored to optimize cardiac hemodynamics through coordinated modulation of heart rate, atrioventricular (AV) coupling, and atrial and ventricular activation patterns as an integrated system. While traditional approaches have addressed these components in isolation, this review summarizes the mechanisms and clinical evidence of a 'whole-heart electro-mechanical optimization' approach for the treatment of bradycardia. AREAS COVERED: Accelerated atrial pacing can lower left-sided filling pres

Complete heart block and subsequent sudden cardiac death following transcatheter tricuspid valve replacement in a heart transplant patient: a case report.

BACKGROUND: Tricuspid regurgitation (TR) is a common valvular complication following heart transplantation. Transcatheter tricuspid valve replacement exists as a novel treatment option. However, as with any procedure, important complications can occur and warrant discussion. CASE SUMMARY: A 46-year-old woman, status post-orthotopic heart transplant (HT), developed symptomatic severe TR. She underwent transcatheter tricuspid valve replacement (TTVR; EVOQUE system, Edwards Lifesciences) and was subsequently presented to the hospital following a syncopal event. She was found to be in cardiogenic

Left bundle branch-Purkinje system in patients with bundle branch reentrant tachycardia: lessons from catheter ablation and electroanatomic mapping.

BACKGROUND: The mechanism of bundle branch reentrant tachycardia has been described, and ablation of the right bundle branch (RBB) is a curative approach. OBJECTIVE: The purpose of this study was to evaluate the left bundle branch (LBB)-Purkinje system during sinus rhythm and to test the feasibility of ablating the LBB in patients with bundle branch reentrant tachycardia. METHODS: Thirteen consecutive male patients (age 62 +/- 12 years) with sustained bundle branch reentrant tachycardia were included in the study. RESULTS: Surface ECG before ablation showed left bundle branch block (LBBB) i

Dynamics of conduction blocks in a model of paced cardiac tissue.

We study numerically the dynamics of conduction blocks using a detailed electrophysiological model. We find that this dynamics depends critically on the size of the paced region. Small pacing regions lead to stationary conduction blocks while larger pacing regions can lead to conduction blocks that travel periodically towards the pacing region. We show that this size-dependence dynamics can lead to a novel arrhythmogenic mechanism. Furthermore, we show that the essential phenomena can be captured in a much simpler coupled-map model.

Clinical outcomes of His-Purkinje conduction system pacing.

His-Purkinje conduction system pacing (HPCSP) in the form of His bundle pacing (HBP) and left bundle branch pacing (LBBP) allows normal left ventricular activation, thereby preventing the adverse consequences of right ventricular pacing. HBP has been established for several years with centers from China, Europe, and North America reporting their experience. There is international guidance as to how to implant such systems with the differing patterns of His bundle capture clearly described. LBBP is a more recent innovation with potential advantages including improved pacing parameters. HPCSP ha

A Perforated Mitral Valve Aneurysm: A Rare but Serious Complication of Aortic Valve Endocarditis Resulting From a Regurgitant Jet Lesion.

Infective endocarditis has high morbidity and mortality rates. The aortic valve is most often affected in native valve endocarditis. Complications of aortic valve endocarditis range from local abscess and fistula formation, systemic complications secondary to thromboembolism and septic embolization, to congestive heart failure resulting from conduction system involvement and valve damage. A rare complication of aortic valve endocarditis is the occurrence of a 'jet lesion' on the mitral valve. Such a lesion, caused by an impinging regurgitant jet stream from a damaged aortic valve, can become d

Serial electrophysiologic studies in patients with chronic bundle branch block.

Serial His bundle recordings were obtained during 1:1 atrioventricular (AV) conduction in 90 patients with chronic bundle branch block over a mean interval of 30 months. Atrioventricular conduction time (AH) increased greater than or equal to 10 msec in 25 (28%) and infranodal conduction time (HV) increased greater than or equal to 8 msec in 29 (32%), but only 10 patients had parallel increases in AH and HV intervals. Increases in conduction times were independent of age, time interval between studies, cause of heart disease or initial AH or HV intervals. Women were significantly more likely t

Renal and cardiovascular effects of Bothrops marajoensis venom and phospholipase A2.

Bothrops marajoensis is found in the savannah of Marajó Island in the State of Pará and regions of Amapá State, Brazil. The aim of the work was to study the renal and cardiovascular effects of the B. marajoensis venom and phospholipase A(2) (PLA(2)). The venom was fractionated by Protein Pack 5PW. N-terminal amino acid sequencing of sPLA(2) showed amino acid identity with other lysine K49 sPLA(2)s of snake venom. B. marajoensis venom (30 microg/mL) decreased the perfusion pressure, renal vascular resistance, urinary flow, glomerular filtration rate and sodium tubular transport. PLA(2) did not

What is cardiac resynchronization therapy?

Cardiac resynchronization refers to pacing techniques that change the degree of atrial and ventricular electromechanical asynchrony in patients with major atrial and ventricular conduction disorders. Atrial and ventricular resynchronization is usually accomplished by pacing from more than one site in an electrical chamber--atrium or ventricle--and occasionally by stimulation at a single unconventional site. Resynchronization produces beneficial hemodynamic and antiarrhythmic effects by providing a more physiologic pattern of depolarization. Atrial resynchronization may prevent atrial fibrillat

Distribution of angiotensinogen in diseased human hearts.

Extrahepatic synthesis and localization of angiotensinogen (ATN) have been described in animals, thus establishing the tissue renin-angiotensin (RA) system. However, there had been no reports of tissue RA systems in human organs, including the heart. In earlier, we have reported the possibility of ATN synthesis in the human heart using ribonuclease protection assay system. ATN mRNA was detected not only in the liver, but also in both the atrial and ventricular heart tissues, suggesting that ATN is synthesized in the human heart. In this report, we looked for the distribution of ATN in diseased

Innovative Management of Rhythm Disorders in Adults with Congenital Heart Disease.

Adults with congenital heart disease (ACHD) represent a rapidly growing patient population that is uniquely predisposed to rhythm disturbances. Arrhythmia mechanisms are shaped by congenital anatomy, prior surgical repairs, and progressive chamber remodeling, necessitating lesion-specific strategies. Intra-atrial reentrant tachycardia remains the dominant atrial rhythm disturbance in repaired or surgically modified atria, whereas atrial fibrillation is increasingly encountered with advancing age. While pulmonary vein isolation using pulsed field ablation has entered clinical practice, extra-pu

Conduction System Pacing: Hope, Challenges, and the Journey Forward.

PURPOSE OF THE REVIEW: Cardiac pacing has evolved in recent years currently culminating in the specific stimulation of the cardiac conduction system (conduction system pacing, CSP). This review aims to provide a comprehensive overview of the available literature on CSP, focusing on a critical classification of studies comparing CSP with standard treatment in the two fields of pacing for bradycardia and cardiac resynchronization therapy in patients with heart failure. The article will also elaborate specific benefits and limitations associated with CSP modalities of His bundle pacing (HBP) and

Maternal hypertensive disorders of pregnancy and electrocardiographic findings among newborns: The Copenhagen Baby Heart Study.

BACKGROUND: Maternal preeclampsia is associated with both congenital heart defects and changes in left ventricular structure and function in the offspring. Whether preeclampsia and gestational hypertension also affect the offspring's cardiac conduction system is unknown. OBJECTIVES: This study assesses whether infants exposed to maternal hypertensive disorders of pregnancy (HDPs) exhibit changes in their electrocardiogram (ECG) compared with infants unexposed to HDPs. METHODS: This population-based cohort study included newborns from the Copenhagen Baby Heart Study who had an ECG performed w

Hemodynamics is a key epigenetic factor in development of the cardiac conduction system.

The His-Purkinje system (HPS) is a network of conduction cells responsible for coordinating the contraction of the ventricles. Earlier studies using bipolar electrodes indicated that the functional maturation of the HPS in the chick embryo is marked by a topological shift in the sequence of activation of the ventricle. Namely, at around the completion of septation, an immature base-to-apex sequence of ventricular activation was reported to convert to the apex-to-base pattern characteristic of the mature heart. Previously, we have proposed that hemodynamics and/or mechanical conditioning may be

Bradycardic onset of spiral wave re-entry: structural substrates.

AIMS: The least understood aspect of re-entrant cardiac arrhythmias is how they start spontaneously. The known mechanisms for re-entry induction involve the application of premature electrical stimuli or rapid pacing, whereas in a clinical setting, re-entry often occurs at normal heart rates. Here, we propose a physiological mechanism of re-entry onset at normal and slow heart rates, which is based on structurally determined heterogeneities. METHODS AND RESULTS: Using a two-dimensional tissue model with Luo-Rudy II kinetics, we study electrical propagation in the presence of macroscopic coupl
