Determinants of cardiac augmentation by elevations in intrathoracic pressure.

We studied the cardiovascular effects of phasic increases in intrathoracic pressure (ITP) by high-frequency jet ventilation in an acute pentobarbital-anesthetized intact canine model both before and after the induction of acute ventricular failure by large doses of propranolol. Chest and abdominal pneumatic binders were used to further increase ITP. Respiratory frequency, percent inspiratory time, mean ITP, and swings in ITP throughout the respiratory cycle were independently varied at a constant-circulating blood volume. We found that pertubations in mean ITP induced by ventilator adjustments

Chylothorax after congenital diaphragmatic hernia repair: a population-based study.

PURPOSE: Chylothorax is a recognized complication of congenital diaphragmatic hernia (CDH) repair. Our aims were to describe the frequency and outcomes of chylothorax and to seek predictors of chylothorax occurrence within a population-based CDH cohort. METHODS: Records for patients with CDH born between 2006 and 2010 were abstracted from a national database and were compared according to presence/absence of postrepair chylothorax. Univariate, and where appropriate, multivariate analyses were performed for group comparisons and chylothorax outcome prediction. RESULTS: Of 243 newborns with CD

Real-time monitoring and control of cooking fumes: Implementation strategies for school kitchens.

Cooking fumes in school kitchens contain hazardous pollutants, including particulate matter (PM), volatile organic compounds (VOCs), carbon monoxide (CO), and formaldehyde (HCHO), which pose significant health risks to staff. Conventional ventilation standards offer general guidelines but are insufficient for managing real-time fluctuations in pollutant concentrations. This study proposes a real-time monitoring and control strategy that integrates low-cost sensors, spatial correction for breathing zone exposure, and Short-Term Exposure Limit (STEL)-based algorithms. The system dynamically adju

Case report: severe microscopic polyangiitis successfully treated with extracorporeal membrane oxygenation and immunosuppression in a pediatric patient.

Severe microscopic polyangiitis can result in acute respiratory failure and renal failure and is commonly treated with mechanical cardiopulmonary support, along with immunosuppression. We report the use of continuous renal replacement therapy and rituximab in a 13-year-old boy with P-antineutrophil cytoplasmic antibody-positive microscopic polyangiitis, resulting in pulmonary hemorrhage and acute renal failure. The patient was treated with high-frequency oscillatory ventilation, extracorporeal membrane oxygenation, and continuous renal replacement therapy, in addition to plasmapheresis, cortic

Adult respiratory distress syndrome and silicone injection.

This report describes a case of adult respiratory distress syndrome (ARDS) after massive subcutaneous injections of silicone. The patient, a 23 year-old transsexual man, treated with mechanical ventilation, died on the 12th day of hospital, in septic shock and refractory hypoxemia. Autopsy findings revealed empty vacuoles surrounded by macrophages in various organs, especially in lung tissue. Toxicological analysis by emission by high frequency induced plasma showed silicone in the organs with histological lesions. The pathophysiology of this ARDS is considered.

A Delicate Balance: Anesthetic Management for Neonatal Congenital Diaphragmatic Hernia Repair.

Congenital diaphragmatic hernia (CDH) presents significant challenges in neonatal management, particularly in the context of anesthesia. This case report details the successful anesthetic management of a five-day-old neonate with left-sided CDH requiring thoracoscopic repair. A five-day-old neonate, delivered via emergency cesarean section due to breech presentation, presented with severe respiratory distress and was diagnosed with left-sided CDH. Initial evaluation revealed respiratory acidosis and moderate pulmonary hypertension. Preoperative stabilization included high-frequency oscillatory

Risk factors for fatal pulmonary interstitial emphysema in neonates.

Among 315 infants treated for respiratory distress syndrome (RDS) over a 2 year period, 32 prematures were studied retrospectively with the diagnosis of pulmonary interstitial emphysema (PIE). Eighteen died. In this group, birth weight below 1600 g, need for oxygen above 0.6 on the 1st day and appearance of bilateral pulmonary interstitial emphysema within the first 48 h of life were significant risk factors, with a mortality rate of 94%. In order to recognize one or more early criteria predictive of fatal PIE, we compared ventilation parameters on day 1 between neonates with fatal PIE and tho

Positional treatment without mechanical ventilation in a very preterm infant with unilateral pulmonary interstitial emphysema: case report and review of the literature.

BACKGROUND: Pulmonary interstitial emphysema (PIE) in very low birth weight infants is a rare but severe complication. Although most of these air leaks develop in mechanically ventilated infants, they have also been reported in infants exposed only to nasal continuous positive airway pressure (CPAP). The optimal treatment for PIE is still under discussion and includes different approaches such as unilateral intubation, high frequency oscillation ventilation and even surgical lobectomy. However, as yet, there has been no report on complete resolution of unilateral PIE by positioning therapy wit

Middle ear ventilation tubes and water precautions: A survey of otolaryngologists and pediatricians on their recommendations.

Introduction. Middle-ear ventilation tubes are commonly placed in pediatric patients because of the high frequency of otitis media. Although avoidance of water activity has been recommended to prevent otorrhea, studies indicate that exposure to water does not increase these episodes. However, a survey in the northwestern United States showed low adherence to these guidelines. This study evaluates the current recommendations of otolaryngologists and pediatricians on water exposure in patients with ventilation tubes. Population and methods. We surveyed 235 health professionals from Argentina, Br

Meconium Aspiration Syndrome: An Insight.

Meconium aspiration syndrome (MAS) is respiratory distress in a newborn baby caused by the presence of meconium in the tracheobronchial airways. The aspiration of meconium stained amniotic fluid by the fetus can happen during antepartum or intrapartum periods and can result in airway obstruction, interference with alveolar gas exchange, chemical pneumonitis as well as surfactant dysfunction. These pulmonary effects cause gross ventilation-perfusion mismatching. To complicate matters further, many infants with MAS have primary or secondary persistent pulmonary hypertension of the newborn as a r

The difficult airway: the use of subglottic jet ventilation for laryngeal surgery.

High-frequency subglottic jet ventilation was used in 352 cases of adults treated with suspension microlaryngoscopy. Of these, 89 had difficult airways, and these are the subject of this manuscript. All cases were managed by the senior anaesthesiologist (AP) and the senior surgeon (JR), between 2000 and 2004. There were minimal complications associated with this technique in this series. The subglottic catheter never proved impossible to insert, and was used on one occasion in a rescue mode after supraglottic jetting led to oxygen desaturation. Body habitus did not prevent use of the technique

[Acute respiratory distress syndrome in children].

The acute respiratory distress syndrome (ARDS) encountered in a child may be either due to a primary lung infection or may be secondary to a systemic inflammatory response of varying origin. Therapy is based on: 1) the mechanical ventilation strategy aimed at maintaining the functional residual capacity by alveolar recruitment using positive end expiratory pressure and to limit secondary pulmonary lesions by using small tidal volumes, 2) prone positioning as soon as sufficient stability is achieved; 3) optimizing tissue oxygen delivery by cardiac support; 4) correction of any other organ dysfu

Experimental pendulum air in the flail chest.

Conditions for occurrence of pendulum airflow under spontaneous ventilation were studied in adult dogs with flail chest experimentally constructed by removing three ribs and the chest wall. Pendulum air flow was recorded pneumotachometrically from outside the body by intubation to the bronchi. Despite objections to the occurrence of pendulum air by many investigators, we found that pendulum airflow occurs under various conditions. The main factors facilitating the occurrence included 1) Significant differences in airway pressure and ventilatory volume between the lungs on the injured and the o

Change in the electroencephalogram delta wave in the frontal cranial region of rats with the hyperventilation.

Hyperventilation is one way to cause activation on the electroencephalogram (EEG) to diagnose brain disorders. The hyperventilation is also known to affect on the delta power in EEG. This study divided the total delta wave into low, middle, and high bands corresponding to the wave frequency. The power in these three delta wave bands was examined in the frontal cranial region of adult male Sprague-Dawley rats hyperventilated with ventilation (VE) of 360, 540, and 720 ml/min for 5 min. The control group was ventilated normally with a volume of 160 ml/min. The results show that the relative power

Initial Observations on the Effect of Repeated Surfactant Dose on Lung Volume and Ventilation in Neonatal Respiratory Distress Syndrome.

BACKGROUND: Exogenous surfactant administration is an essential part of respiratory distress syndrome treatment in preterm infants. Current guidelines recommend the first dose to be given as early as possible, followed by an additional dose if symptoms persist. The effect of additional dosing on regional ventilation and lung volume has not been investigated so far. OBJECTIVES: The aim of this study was to assess changes in ventilation distribution, lung volume, and gas exchange following repeated surfactant dosing in invasively ventilated neonates. METHOD: Preterm infants requiring invasive

Natural surfactant for neonatal respiratory distress syndrome in very premature infants: a 1992 update.

Natural surfactant (Surfactant TA, Survanta, CLSE, SF-RI 1, Curosurf and human surfactant obtained from amniotic fluid) therapy for RDS in very premature infants has been evaluated in 17 controlled clinical trials. Uniformly intratracheal surfactant administration caused a decreased intensity of mechanical ventilation during the first hours (reduced inspiratory pressure, reduced oxygen requirements) as an immediate effect of surfactant administration. Metanalysis reveals barotraumatic pulmonary complications mainly, pneumothorax and pulmonary interstitial emphysema to occur less frequently in
