Although rotator cuff disorder is by far the leading cause of shoulder pain, such pain can also reflect actual osteoarthritis. In shoulder pathology, non-inflammatory osteoarthritis should be kept in mind, as it is often the cause of symptoms resembling those of degenerative rotator cuff, with which it is most often associated. Among types of non-inflammatory osteoarthritis of the shoulder, the most often observed are acromioclavicular osteoarthritis and osteonecrosis of the humeral head.

The treatment of an irreparable rotator cuff tear is an enigma. There are so many treatment options, with different experts recommending dif … surgery. If I can do an adequate partial repair, where the residual defect is small enough that the humeral head does not button hole through the defect like a boutonniere deformity, that is my treatment of choice. I will only do grafts with Hamada stage 1 or 2, as stage 3 with a fixed humeral head against the acromion do not do well. I am intrigued by the bal …

OBJECTIVE: This article discusses the imaging findings, clinical findings, and conservative chiropractic management of 2 patients with glenoid hypoplasia.
CLINICAL FEATURES: Conventional radiographs of both patients revealed a hypoplastic glenoid bilaterally. Notch-like defects along with signs of degenerative disease were evident within the lower portion of the glenoid rims bilaterally in 1 patient and in the left glenoid rim of the other patient. Magnetic resonance imaging revealed a degenerative cyst or cortical defect in one patient along the anterior humeral head. The second patient show…

Assessment of posture is an integral component of patient evaluation with shoulder overuse injuries. However, the professional literature contains relatively few studies that have assessed the relationship between posture, function, and injury. The purpose of this study was to determine the relationship and differences in postural variables within and between subjects with overuse injuries to the shoulder of healthy subjects. Thirty patient subjects and 30 healthy subjects matched for age and gender were recruited. Scapular protraction and rotation, forward head position, midthoracic curvature

INTRODUCTION: The anterior and anterolateral approaches to the humerus describe splitting brachialis longitudinally, assuming its fibres run parallel to the shaft. Recent improvements in the understanding of brachialis anatomy however have demonstrated it has two distinct heads, with the bulk of its fibres running oblique relative to the humerus. Attempting to split brachialis longitudinally to the extent required for plate osteosynthesis invariably leads to transection of a significant number of muscle fibres. The authors present a less muscle destructive modification to the anterolateral app

BACKGROUND: Posterior shoulder dislocation accompanied by reverse Hill-Sachs lesions is relatively rare, whereas bilateral posterior shoulder dislocation is exceedingly rare. CASE PRESENTATION: We report two cases of bilateral posterior shoulder dislocations associated with reverse Hill-Sachs lesions. In Case 1, the lesions were managed using the McLaughlin procedure and the modified McLaughlin procedure. In Case 2, open reduction and internal fixation of the left shoulder was unsuccessful; consequently, reconstruction of the right humeral head was performed using bone cement. CONCLUSIONS: Bil…

Between 1996 and 2003 a total of 19 humeral head osteotomies were performed to treat post-traumatic malunion of the proximal humerus. Mean age was 46 years, ranging from 17 to 71 years. The period of time between traumas and surgeries ranged from 6 to 22 months. Osteotomy procedure was as follows: one after two-part fracture of the greater tuberosity, three after two-part fracture of the surgical neck, five after three-part fracture of the greater tuberosity, eight after four-part fracture, two after complex head splitting fracture, one after two-part fracture with anterior dislocation. Three …

CASE: Reported here is a single case of a highly comminuted fracture of the proximal aspect of the humerus with substantial humeral head displacement and a concomitant glenoid fracture encompassing >20% of the glenoid width. The fractures were treated simultaneously with reverse total shoulder arthroplasty and open reduction and internal fixation, respectively.
CONCLUSION: The 1-year follow-up demonstrated excellent pain control and a good functional outcome, with no signs of instability or implant complications.

For the treatment of large to massive rotator cuff repairs, not all partial repairs are equivalent. The technique of rotator cuff repair in large to massive rotator cuff tears matters. By using mobilization techniques to mobilize the rotator cuff to obtain anatomic or near anatomic coverage, double-row techniques to increase footprint coverage, and medial mobilization to decrease tension on the repair, exposure of the humeral head can be minimized, which can improve outcomes.

Complications of metallic hardware about the shoulder have been well documented in the literature. This case report presents a recreational athlete who presented to our office with a painful shoulder 4 years after a Bristow procedure. Radiographic imaging confirmed contact of the humeral head on the Bristow screw. In these unusual situations, arthroscopic evaluation, as this case suggests, should be considered. The patient's hardware was removed arthroscopically, obviating an open procedure and hospitalization.

Computed tomographic (CT) scans of the shoulders of 10 normal individuals and six patients with a history of recent or recurrent shoulder dislocation were reviewed. In normal individuals, the surface of the glenoid was smooth and was always within 10 degrees of being perpendicular to the body of the scapula. As scans progressed from cephalad to caudad, the glenoid surface twisted and faced more anteriorly. The center of the humeral head always projected within the central one-third of the glenoid fossa. In the patients with dislocations, CT scans depicted a variety of lesions, including: bony …

A hypokinetic model has been developed which attempts to simulate the weightlessness experienced during space flight. Male rats (approximately 130 g) were suspended from the model with a head-down tilt for a two-week period. Total mechanical unloading of the hind limbs and partial unloading of the fore limbs occurred. In comparison to pair-fed control rats, the skeletal alterations in the proximal tibial and humeral metaphyses of suspended rats were determined to be a diminished rate of longitudinal bone growth, a reduced mass of mineralized tissue, and an accumulation of marrow fat. Also, sus

BACKGROUND: Glenohumeral osteophytes (OPs) can adversely influence postoperative range of motion (ROM) following shoulder arthroplasty due to mechanical impingement. Though commercial three-dimensional preoperative planning software (3D PPS) is available to simulate ROM before and after OP resection, little is known about the magnitude of effect OPs and their subsequent removal have on simulated glenohumeral ROM.
METHODS: Included patients were 1) indicated for reverse total shoulder arthroplasty (rTSA) using 3D PPS and 2) presented with glenoid and/or humeral head OPs on preoperative two-dim…

BACKGROUND: Displaced proximal humeral fractures remain a challenge to orthopedic surgeons.
OBJECTIVES: The purpose of this study was to evaluate the functional and radiological outcomes of patients with comminuted proximal humeral fractures treated with closed reduction and percutaneous screw fixation (CRPF).
MATERIAL AND METHODS: The authors retrospectively reviewed 38 cases of displaced proximal humeral fractures (2-, 3- or 4-part fractures according to the Neer classification) that were treated using the CRPF technique from May 2009 to April 2013. From this group 26 patients were followe
