We present an unusual case of empyema of the gallbladder associated with a pyogenic liver abscess in a patient with Crohn's disease on Infliximab. It manifested by weakness, weight loss, and vague abdominal pain, which eventually localized to the right upper quadrant 4 days prior to admission. Infliximab was introduced along with methotrexate 8 mg/week to a female patient with intractable rheumatoid arthritis. Pyoderma gangrenosum is an extraintestinal manifestation of inflammatory bowel disease requiring meticulous medical and/or surgical treatment. OBJECTIVE: The aim was to evaluate the efficacy and safety of multiple infusions with achimeric, anti-tumor necrosis factor (TNF)alpha monoclonal antibody (infliximab) in patients with psoriatic arthritis (PsA) and psoriasis vulgaris.
METHODS: Over 22 weeks, nine patients with both active psoriasis and PsA received five infusions of 3 mg/kg infliximab. BACKGROUND: We aimed to clarify the efficacy, safety, and factors associated with remission on dose escalation in patients with Crohn's disease showing loss of response (LOR) to infliximab treatment of 5 mg/kg at 8-week intervals in a clinical trial.
METHODS: Thirty-nine patients with LOR to 5 mg/kg infliximab therapy started treatment with 10 mg/kg per 8 weeks. LOR was defined as both a Crohn's Disease Activity Index of ≥175 at 8 weeks after infusion of 5 mg/kg infliximab and a Crohn's Disease Activity Index increase of ≥50 from 4 to 8 weeks after infusion.

INTRODUCTION: Subcutaneous infliximab (SC-IFX) has emerged as an alternative maintenance therapy for inflammatory bowel disease (IBD), offering absorption and sustained systemic exposure. Although higher and more stable serum concentrations have been reported with SC-IFX, prospective real-world data evaluating pharmacokinetic evolution after transition from intravenous infliximab (IV-IFX) remain limited, particularly in Latin America. In patients with acute severe ulcerative colitis (ASUC) who have received infliximab rescue therapy, the need for and timing of further steroid therapy is unclear. This brief communication explores contemporary clinical practice across Australia regarding corticosteroid management following rescue therapy in ASUC. By describing variability in real-world practice patterns and potential risks and benefits of different approaches, this article highlights current evidence gaps and the need for further studies to better inform clinical practice.
