Tracking intraoperative fluoroscopy utilization reduces radiation exposure during ureteroscopy.

PURPOSE: Recent studies have demonstrated deleterious effects of ionizing radiation from diagnostic and therapeutic imaging procedures. One of the barriers to minimizing patient exposure is physician awareness. We prospectively studied whether providing surgeons with feedback on their fluoroscopy utilization would affect intraoperative fluoroscopy times. MATERIALS AND METHODS: In 2007, we prospectively began to track fluoroscopy usage for all urology cases. Nine months later, surgeons started to receive periodic reports with their mean fluoroscopy time compared with their peers. We reviewed a

Laparoscopic management of peripelvic renal cysts: University of California, San Francisco, experience and review of literature.

OBJECTIVES: To report our experience and review published reports on the laparoscopic management of peripelvic renal cysts. Peripelvic renal cysts represent a unique subset of renal cysts, as they are rare, commonly symptomatic, and more difficult to treat than simple peripheral renal cysts. Minimally invasive methods for the treatment of peripelvic renal cysts, including laparoscopic decortication, have recently become more common. METHODS: Four patients who presented with symptomatic peripelvic cysts underwent laparoscopic decortication at our institution. All four were men aged 47 to 65 ye

Unusual late vascular complications of sagittal split osteotomy of the mandibular ramus.

Intraoperative or early postoperative vascular complications are not uncommon problems in sagittal split osteotomies of the mandibular ramus; however, reports of late complications are considerably rarer. Here, we present two patients who sustained late vascular complications after the sagittal split osteotomy. The first patient had a delayed bleeding, which presented itself as a rapidly expanding swelling of the left cheek from the left external carotid artery 18 days postoperatively. During exploration, a 2 mm laceration of the external carotid artery located just proximal to the bifurcation

Design and validation of a stereotactic frame for independent bilateral multi-trajectory targeting.

Objective.Modern stereotactic neurosurgery increasingly relies on bilateral and multi-trajectory workflows (e.g. deep brain stimulation (DBS), stereoelectroencephalography (sEEG)), which can exceed the reach and flexibility of conventional frame-based systems. Building on our skull-mounted Navinetics D1 platform, we present a stereotactic frame (Navinetics E1) intended to expand the reachable work envelope, enable independent bilateral targeting, and preserve mechanical accuracy and patient comfort.Approach.We designed the Navinetics E1 stereotactic frame using a center-of-arc architecture wit

Prevalence and risk factors of medial meniscal ramp lesions in patients undergoing primary anterior cruciate ligament reconstruction at a tertiary center.

BACKGROUND: Ramp lesions are injuries of the peripheral meniscocapsular attachment of the posterior horn of the medial meniscus and are commonly associated with anterior cruciate ligament (ACL) tears. Failure to identify these lesions may compromise knee stability and outcomes after ACL reconstruction (ACLR). PURPOSE: The primary objective of this study was to determine the prevalence of ramp lesions among patients undergoing primary ACLR at a tertiary center in Saudi Arabia. The secondary objective was to identify patient- and injury-related risk factors associated with ramp lesions. STUDY

Total Hip Arthroplasty in Legg-Calve-Perthes Disease: Mean Nine-Year Outcomes of 201 Hips.

BACKGROUND: Legg-Calvé-Perthes disease (LCPD) is an uncommon, but challenging indication for total hip arthroplasty (THA). We aimed to report one of the largest single-center contemporary series of THA associated with this condition. METHODS: We reviewed 201 THAs for LCPD performed at a single institution between 2000 and 2020. The mean age at surgery was 47 years, the mean body mass index was 32 and 74% were men. Bearing surfaces included ceramic-on-highly cross-linked polyethylene in 95 hips (47%), metal-on-highly cross-linked polyethylene in 83 hips (41%), ceramic-on-ceramic in 17 hips (9%

Age as a Risk Factor for Intraoperative Periprosthetic Femoral Fractures in Cementless Hip Hemiarthroplasty for Femoral Neck Fractures: A Retrospective Analysis.

BACKGROUND: Understanding the risk factors and outcomes of intraoperative periprosthetic femoral fractures (IPFF) during hip arthroplasty is crucial for appropriate perioperative management. Previous studies have identified risk factors for IPFF in total hip arthroplasty patients, but data for hip hemiarthroplasty (HA) is lacking. The aim of this study was to determine the age associated with increased rates of IPFF in patients undergoing HA. METHODS: We retrospectively reviewed patients aged 65 years and above who underwent a cementless HA for a displaced femoral neck fracture and had a mini

Novel radiation-sparing imaging techniques in transcarotid stenting.

Transcarotid artery revascularization is increasingly prevalent, but relies on fluoroscopic guidance for placement of carotid stents. With increasing awareness about the hazards of radiation exposure, we aimed to explore the prior literature regarding alternative and adjunct imaging methods in carotid stenting. These have included fusion with preoperative computed tomography angiography, fully contrast-sparing procedures with preoperative magnetic resonance angiography and fusion using plain fluoroscopy, and ultrasound-assisted transfemoral carotid stenting. All methods demonstrated excellent

Thoracic Packing Failure due to Occult Diaphragmatic Injury after Empyema Surgery.

INTRODUCTION: Intrathoracic gauze packing is an established damage control technique for life-threatening thoracic hemorrhage in trauma settings. However, its role in nontraumatic coagulopathic bleeding remains unclear, and the mechanisms underlying packing failure have not been well characterized. We report a case in which intrathoracic packing failed due to an unrecognized diaphragmatic injury, highlighting an important limitation of this strategy. CASE PRESENTATION: An 82-year-old man with chronic kidney disease, diabetes mellitus, and hypoalbuminemia underwent emergency video-assisted tho

Questionnaire-based Survey on the Prevention of Surgical Site Infection after the Publication of Its Guidelines for Neurosurgery in Japan.

In recent years, the Japanese neurosurgical field has been added to the guidelines for the appropriate use of antimicrobial agents for the prevention of surgical site infection; however, the awareness of neurosurgeons and specific methods for surgical site infection prevention in the Japanese neurosurgical field remains unclear. Therefore, we report a repeat survey conducted after the addition of guidelines on the appropriate use of antimicrobial agents for the prevention of surgical site infection and compare it with our previous survey on surgical site infection prevention conducted in 2018.

The amnesic effects of propofol on functional connectivity in the hippocampus determined by functional magnetic resonance imaging in volunteers.

BACKGROUND: One of the primary actions of general anaesthetic agents, apart from inducing a state of unconsciousness, is reversible impairment of memory formation during the period of administration. Failure to induce and maintain amnesia can result in recall of accidental intraoperative awareness and contribute to adverse psychological health outcomes. The precise mechanisms of action by which general anaesthetics achieve their amnesic effects are not fully understood. To this end, we focused on the hippocampus, a region critical for the formation of new episodic explicit long-term memories o…

Splenic Artery Aneurysm in Gaucher Disease: A Hybrid Study Combining Case Report, Scoping Review, and Clinical Survey.

Gaucher disease (GD) is a rare lysosomal storage disorder caused by pathogenic variants in the GBA gene. Splenic artery aneurysm (SAA) is an uncommon and underrecognized complication of GD, particularly in the absence of traditional risk factors. SAA carries a high risk of rupture and significant mortality. This hybrid study investigates the clinical features and impact of SAA in GD through a three-part approach: a case report, a scoping review with individual patient data (IPD) analysis, and a clinical survey conducted across two GD centers in Naples, Italy. We report the case of a 46-year-ol

Salmonella Infection Arising From a Wound in an Infant: A Case Report.

Bilateral hand cellulitis is an uncommon manifestation of Salmonella infection, particularly in pediatric patients. While Salmonella is typically associated with gastrointestinal symptoms, extraintestinal infections such as cellulitis are rare and pose diagnostic challenges. These atypical presentations highlight the clinical diversity of Salmonella species and underscore the need for timely and targeted management. A previously healthy one-year-old girl presented with progressive swelling, redness, and warmth in both hands, accompanied by fever. There was no history of trauma, insect bites, o

Defining a danger zone for iatrogenic long thoracic nerve injury in gender-affirming mastectomy.

The long thoracic nerve's (LTN) superficial location on the chest wall renders it vulnerable to iatrogenic injury. Plastic surgeons' gender-affirming mastectomy volumes are rapidly increasing. This operation involves lateral chest contouring placing the distal LTN at risk of injury along the chest wall. Although proximal LTN injury can cause debilitating shoulder dysfunction, more distal injury can cause chronic postoperative shoulder pain and dysfunction without frank scapular winging, making diagnosis and treatment difficult. Therefore, injury is best avoided. In this study, the course of th
