Three-dimensional scanning electron microscopic study of keratoconus corneas.

OBJECTIVE: To examine the 3-dimensional collagen fibril organization in the Bowman layer of keratoconus corneas. METHODS: Eight keratoconus corneas, 8 corneas with other diseases, and 5 normal human corneas were studied. A cell maceration method in combination with scanning electron microscopy was used to examine the collagen network in the Bowman layer. RESULTS: In normal corneas, the surface of the Bowman layer was smooth and collagen fibrils were regularly arranged. By contrast, sharply edged defects in the Bowman layer were found in keratoconus corneas. Lattice-like configurations of the…

Automated topographic screening for keratoconus in refractive surgery candidates.

PURPOSE: We evaluated an automated corneal topography classification system developed as an adjuvant for screening patients prior to keratorefractive surgery. We screened for patterns suspicious for keratoconus by applying the system to the analysis of a series of patients who presented for evaluation for surgical correction of myopia. METHODS: Both eyes of 53 consecutive patients who were included in a previously reported prospective study were evaluated using the Expert System classification algorithm. This quantitative classification system incorporating eight indices was applied to the vi…

Histologic evidence of recurrent keratoconus seven years after keratoplasty.

PURPOSE/METHODS: A 42-year-old patient with keratoconus developed clinical signs of recurrent keratoconus in her right corneal graft seven years after the first keratoplasty was performed. She underwent successful corneal regrafting. RESULTS/CONCLUSIONS: The histopathologic examination of the corneal button disclosed typical findings of keratoconus, including breaks in Bowman's layer filled with connective tissue or epithelium. These histologic findings in the corneal graft support our clinical diagnosis of keratoconus. However, the pathogenesis of this complication is still not clear and fur…

Nebulae at keratoconus--the result after excimer laser removal.

Ten patients underwent excimer laser ablation due to nebula formation at keratoconus. The nebulae interfered significantly with contact lens fit or wearing time. The mean follow-up time in these patients was 16.5 months. Following surgery all patients could be successfully fitted with a contact lens and thereby obtain good visual acuity. Furthermore, contact lens wearing time was 8 hours or more in all cases. In 2 patients the nebulae recurred but were successfully retreated.

Synchrotron x-ray diffraction studies of keratoconus corneal stroma.

The aim of this study was to determine any differences in the collagen molecular and fibrillar packing, or the arrangement of the proteoglycans along the fibril axis, in the stroma of keratoconus and control corneas. High and low-angle x-ray diffraction patterns from the fibrillar and molecular packing of collagen in keratoconus and control corneas were obtained using a synchrotron radiation source. The results indicate no difference in interfibrillar spacing between keratoconus and control corneas at normal physiological hydration, or over a range of hydrations (H = 1-11). This unambiguously …

Penetrating keratoplasty for keratoconus: complications and long-term success.

A series of 100 penetrating keratoplasties for keratoconus performed between 1968 abd 1986 were reviewed for long-term results. The mean follow-up was 6.1 years with a range of 4-16 years. The systemic associations and the postoperative complications were analysed. Grafting in cases associated with Down's syndrome had a higher incidence of complications. 93% of grafts remained clear and 81% achieved a final corrected visual acuity of 6/12 or better. 21% of eyes developed a homograft reaction, with 50% of rejection episodes occurring in the first year after operation. Factors associated with hi…

Human basement membrane components of keratoconus and normal corneas.

Using immunofluorescence techniques, we analyzed the distribution of glycoproteins in normal and keratoconus corneas of humans. Laminin, bullous pemphigoid antigen, fibronectin, and fibrin/fibrinogen were all found in the epithelial basement membrane of normal corneas. Keratoconus corneas produced similar results, except that staining for fibrin/fibrinogen was weak. Fibrin/fibrinogen was absent from normal corneal basement membrane in animal models studied previously. Keratoconus may be the result of the lysis of fibrin or may involve impeded elaboration of fibrin.

Relationship between aberrometry and quality of life in patients with keratoconus.

BackgroundCorneal deformations caused by keratoconus induce high optical aberrations (HOAs), which affect the quality of life even with an appropriate optical correction. Quality of life is mainly assessed using questionnaires, but their use in clinical practice is limited by the time required and their subjectivity. The aim of this study was to test the correlation between quality of vision assessed by aberrometry and quality of life questionnaire.MethodsVision-related quality of life was assessed using the NEI-VFQ 25 (National Eye Institute Visual Function Questionnaire). Quality of vision w…

In Vitro Assessment of Nintedanib in Keratoconus Corneal Stromal Microenvironment.

Keratoconus (KC) is a degenerative corneal disease, characterized by stromal thinning and abnormal ECM remodeling, leading to fibrosis. Corneal fibrosis is a leading cause of visual impairment. Corneal stromal keratocytes differentiate into myofibroblasts, which alters extracellular matrix (ECM) protein deposition. Nintedanib (NIN) is an antifibrotic FDA-approved tyrosine kinase inhibitor, but its function in the cornea is largely unknown. This study examined the impact of NIN within the human corneal stromal microenvironment. Healthy corneal stromal fibroblasts (HCFs) and KC fibroblasts (HKCs…

Unraveling the immune axis of ocular surface microbiota in keratoconus.

PURPOSE OF REVIEW: This review's objective is to give readers an update on recent and developing information about the function of the ocular surface microbiome in keratoconus. RECENT FINDINGS: The microbiome's function in the pathogenesis of keratoconus is supported by recent research. Numerous metabolites that are produced by the ocular surface bacteria can affect the host tissue. A shift in the microbiota may influence the composition of these metabolites, which could have an effect on keratoconus by altering the cornea's metabolic environment. SUMMARY: Gaining insight into the function o…

Advancements and Innovations in Keratoconus Management: A Review of Current Practices.

Keratoconus is a potentially blinding condition characterized by progressive thinning and steepening of the cornea, leading to visual impairment due to irregular astigmatism and myopia. While the exact pathophysiology is still unknown, it is believed to involve genetic, environmental, and cellular factors. Treatment options for keratoconus have significantly expanded over the past few decades. Historically, glasses were the primary means of managing mild cases, whereas rigid gas permeable contact lenses were used in moderate to advanced diseases; yet the latter were rarely tolerated by patient…
