BACKGROUND: Numerous surgical treatments have been described to address subluxation or dislocation of the patella, but many are not suitable for the skeletally immature patient, as they risk injury to the proximal tibial physis or tibial tubercle apophysis. The Galeazzi semitendinosus tenodesis is a soft-tissue reconstruction technique designed to stabilize the patella without altering the femoral or the bony structures about the knee. We sought to determine if this semitendinosus tenodesis is a safe and effective treatment for patellofemoral instability in skeletally immature patients.

BACKGROUND: The medial patellofemoral ligament (MPFL) is the primary restraint to extreme lateral displacement and is typically disrupted with an acute lateral patellar dislocation. Patients who fail a comprehensive nonoperative program and experience recurrent lateral patellar instability episodes are candidates for surgical treatment. We report the results of a retrospective analysis of 43 patients who received 56 Rotaflex total knee arthroplasties, with a mean follow-up of 55.7 months. The British Orthopaedic Association (BOA) knee assessment protocol was used in evaluating the clinical results. Two patients could not receive post-operative scores. In the remaining 54 knees, the mean pre-operative score was 25.6, improving to 30.8 post-operatively. Ten knees showed a decrease in knee score, two were unchanged and 42 improved. In this study we present the outcome for patients with the Rotaglide mobile meniscal knee prosthesis implanted for osteoarthritis. The knees of 210 institutionalized patients with Down syndrome were evaluated to determine the prevalence of patellofemoral instability and to assess the value of orthotic and surgical treatment. The cases of 151 non-institutionalized patients with Down syndrome were also reviewed. The patella was dislocatable or dislocated in thirty-five knees (8.3 per cent) in the institutionalized group and in twelve knees (4.0 per cent) in the non-institutionalized group. This interfered with walking in some patients, but only three of the patients were unable to walk because of patellofemoral instability.
